The common complaints are discomfort due to itching and a picture of lesions on the skin in the form of papules, pustules, or vesicles. The study aims to analyze the comparative efficacy of using a combination therapy of 5% permethrin cream and precipitated sulfur (ointment 2-4) compared with single therapy of 5% permethrin cream in pediatric scabies patients at the Outpatient Clinic Dr. Soetomo General Academic Hospital in the 2019-2020 period.\u0000Methods: This study is an analytical study using a cross-sectional retrospective method, which aims to compare the effectiveness of using a combination of 5% permethrin cream and precipitated sulfur(ointment 2-4) compared to 5% permethrin cream as a single therapy. Study data were processed using SPSS, followed by univariate and bivariate analyses using statistical methods and comparative tests.\u0000Results: This study found 88 new scabies patients, with the number of patients who received permethrin single therapy as many as 43 patients (51.1%), while those who received combination therapy were 45 patients (48.9%). Scabies treatment using combination therapy has better effectiveness than single therapy 5% permethrin cream when seen from the absence of new complaints and improvement of skin lesions after receiving treatment. Analysis of therapy results for the emergence of new complaints with a p-value of 0.010 showed significant results. However, many people have dental caries as one of the main diseases in the oral cavity. Dental caries can occur due to pH falling in the oral cavity by acid-producing bacteria, such as Streptococcus gordonii, which metabolizes sucrose as an energy source and produces lactic acid. Then, the environmental pH decreases. Chemicals are widely used as antibacterial agents, but long-term use can cause negative effects, hence antibacterial agents with minimal side effects are needed. Red ginger is one of the options that appears as a promising antibacterial agent. Even though a lot of research has been investigated regarding red ginger, there is limited information on the effect of fractionated red ginger extract on pH in bacterial metabolism, especially Streptococcus gordonii. This study aims to analyze the effect of the other components in red ginger besides its essential oil on the pH level of Streptococcus gordonii metabolism.\u0000Methods: The groups in this study are control group 1 (bacteria, BHIB), control 2 (bacteria, BHIB, 5% sucrose), and treatment groups (bacteria, BHIB, 5% sucrose, extract red ginger 1.56% and 0.78%). Generally, no special therapy is required because spontaneous resolutions occur. However, in some cases, therapy is required due to the potential of complications. During therapy, the side effects and effectiveness of therapy must be evaluated. One of the assessment tools is the Hemangioma Activity Score (HAS). HAS is a simple and objective assessment to evaluate the successful treatment of IH by observing swelling, color, and ulceration. The use of HAS in Indonesia is still minimal. This study aims to evaluate HAS before and after therapy in IH patients at Dr. Soetomo General Academic Hospital and Surabaya Skin Centre.\u0000Methods: Twenty-seven patient medical records were selected based on the inclusion and exclusion criteria. This study is an analytic cross-sectional retrospective using consecutive sampling. The collected data were tested for suitability using Cohen's Kappa. Pre- and post-tests were carried out using the Shapiro-Wilk and Wilcoxon tests afterward.\u0000Results: Among 27 IH cases occurred in females (81.4%), with the youngest age of the patient being 1 month and the oldest being 8 years. The onset of lesions in all patients appeared before 1 year of age. The most frequent location for IH lesions was the facial area (51.8%), with a clinical manifestation being plaque (37%). The most common therapy used in this study was pulse dye laser (PDL) (48.1%). Paraquat causes direct cell damage by producing free radicals, which can cause pulmonary fibrosis and kidney failure. This study aims to analyze the effects of paraquat toxicity on the lungs and kidneys on histopathological features.\u0000Methods: This research is an experimental research post-test only control group design. The research subjects used stored biological material in the form of 12 lung and kidney organ samples from white rats (Rattus norvegicus) that had been treated in previous research, which were divided into 2 groups, namely the negative control group (P1, given distilled water), the positive control group (P2; given paraquat 0.01 gr/kgBW [single dose intraperitoneal]). All samples of stored biological material were made for lung and kidney preparations. The lung condition was assessed using the American Thoracic Society criteria, and the kidneys were evaluated using the Mitchell criteria with the help of the ImageJ application. Data were analyzed using the Kruskal-Wallis test and the Mann-Whitney test.\u0000Results: The Kruskal-Wallis test showed a significant difference in lung damage scores between groups with a p-value of 0.01 (p <0.005). The results of the Kruskal-Wallis test of the kidneys showed substantial differences in glomerular and proximal tubular damage scores between groups with p values of 0.004 and 0.009 (p <0.005). It has guaranteed locoregional control of the disease, guided adjuvant therapy, and functioned as a staging tool. However, over the past 25 years, a more conservative approach has become the standard for surgical management of the axilla. Because sentinel lymph node biopsy (SNB) is less invasive and less morbid than axillary lymph node dissection (ALND), it has become the standard surgical procedure for axillary staging of clinically node-negative breast cancer. This study aimed to compare the mortality of breast cancer patients receiving ALND and SNB procedure.\u0000Methods: Our review includes randomized controlled trials of breast cancer patients undergoing ALND or SNB surgical techniques. A systematic literature search was performed from PubMed on December 2023 to search for articles reporting on survival rates between ALND vs SNB among breast cancer patients. The data obtained then underwent statistical evaluation, and a meta-analysis was conducted using Review Manager 5.4. In order to represent primary outcomes, the risk ratio was estimated using an inverse variance technique with a random-effects model. Heterogeneity was presented with total values, 95% confidence intervals (CI), and I values.\u0000Results: As many as 208 articles were found in the PubMed database search, and only six studies were included in this study. The six included trials all used chemotherapy and radiotherapy for systematic treatment. The length of follow-up varied from 33 months to 15 years. The total number of patients in the trials was 3,585, and the study period was from 2011 to 2023. The pooled analysis of the mortality outcomes showed that the heterogeneity of the data was low (I2 21%, p = 0.4). In the pooled analysis, the mortality of patients with SNB for surgical treatment was not significantly different compared to patients with ALND (OR:0.88; 95% CI:0.65-1.19).\u0000Conclusion: The mortality of patients undergoing SNB and ALND does not differ significantly, according to this pooled study. Following the principles of Morrison's research, we assessed its effectiveness in calming users with four distinct vibration patterns.\u0000Method: Ten male participants without ASD history, aged 19-25 and weighing 48-73 kg, provided subjective evaluations using the Comfort Rating Scale (CRS).\u0000Result: Results showed that participants reported increased calmness after using the device, as evidenced by the overall ratings of the \"calming\" term, with the \"down\" pattern receiving the highest ratings. The study advocates for future work involving physiological sensors to measure the device's effectiveness objectively. While promising, this innovation has limitations, such as fixed vibration frequency and reliance on an adapter for power. The findings of diffuse type of systemic sclerosis, moderate restriction in pulmonary function test, interstitial pneumonia in chest x-ray, and ground glass opacity in thoracic CT-scan establish the diagnosis of interstitial lung disease associated with systemic sclerosis. We present a case of interstitial lung disease in a systemic sclerosis case, which was approached and treated comprehensively to slow the progression.\u0000Case Presentation: A 40-year-old female was admitted to the rheumatology polyclinic of Dr. Soetomo Hospital Surabaya with persistent dry cough for two years in June 2021. The patient also complained of breathlessness, painful joints, swollen knees, and morning stiffness three months prior to admission. The positive results of antinuclear antibody and rheumatoid factor were identified in 2016, which suggested autoimmune disease of scleroderma and being treated with azathioprine 50 mg twice a day, methylprednisolone 4 mg once a day, chloroquine 200 mg once a day, and calcium lactate once a day. The examinations in 2020 showed moderate restriction from the pulmonary function test, bilateral lung parenchymal infiltrates from the chest x-ray, and ground-glass opacity from the chest computed tomography scan (CT-scan). The patient was monthly prescribed cyclophosphamide 500 mg intravenously for six months as the substitution of azathioprine, methylprednisolone 4 mg once a day, and aspirin 100 mg once a day. The latest worsening condition led the patient to receive the substitution of cyclophosphamide which was mycophenolate mofetil 360 mg twice a day, along with aspirin 100mg and methylprednisolone 4 mg once a day, respectively.\u0000Conclusion: As a rare disease, the complications of systemic sclerosis should be anticipated. In this case, interstitial lung disease occurred following nearly five years of systemic sclerosis course which required immunosuppressive agents to enhance the efficacy of treatment. {"title":"Invasive candida infection as the cause of seizure in a patient with systemic lupus erythematosus: a report of unusual case","authors":"Mirza Suryo Adi, A. Seizures in NPSLE might be caused by intracranial infection, with bacteria as the most common pathogens. This study reported a rare case of seizure in a SLE patient due to invasive Candida infection.\u0000Case Presentation: A 26-year-old female presented in the emergency department of Dr. Soetomo Hospital Surabaya, Indonesia with seizure, which she has experienced in the last 2 days prior to hospital admission. She experienced a convulsion that lasted 5-10 minutes followed by loss of consciousness. The patient also complained of severe headaches two days before hospital admission. She was diagnosed with SLE in November 2022 and was currently taking methylprednisolone. Physical examination showed Glasgow Coma Scale (GCS) of E3V4M5, fever, dyspnea, and rough rhonchi on the right lung. Chest X-ray indicated pneumonia, while brain computed tomography (CT) suggested subacute to chronic thromboembolism. The patient was diagnosed with NPSLE due to intracranial infection, and hospital-acquired pneumonia (HAP), and was given Levofloxacin. On the 3rd day, a reddish lesion with central healing was found on the cheeks, and the patient had dyspnea, suggesting fungi infection. Methylprednisolone was stopped while fluconazole and urine cultures were initiated. Urine culture suggested Fluconazole-sensitive Candida and consultation with a dermato-venerologist confirmed Candida, thus, Fluconazole was continued along with dexamethasone, and Levofloxacin. The patient showed clinical improvement and was discharged after 12 days.\u0000Conclusion: Seizure is one of the most common manifestations of NPSLE, due to intracranial infection. Although rare, we cannot rule out the possibility of fungal infection in NPSLE patients with intracranial infection. It is recommended to create AVFs as distally as possible. The snuffbox, situated at the far end of the wrist, is infrequently utilized for AVF insertion due to the prevalent smaller vessel calibers.\u0000 Case: This case series involves seven patients with chronic kidney disease scheduled for first AVF placement in the snuffbox region, with an average age of 56.71 years. All patients showed an average radial artery caliber of 20.3mm, a mean peak systolic velocity of 67.14 cm/s, and a cephalic vein caliber of 22.7mm, according to preoperative vascular ultrasound. The AVF placement involved an end-to-side anastomosis. All patient demonstrated anastomosis clinical maturation, the primary patency rate was 86%. Follow-up Doppler examinations revealed that patients achieving functional patency had an average caliber of 5.22mm and an average flow rate of 810.65 ml/s.\u0000 Conclusion: SAVF are created less frequently due to predominantly smaller vessel caliber. However, it can be a viable alternative site for fistula implantation. Vascular mapping is required to ensure that both vessels have minimum 2 mm caliber. This study aimed to determine the relationship between Leprosy Suspicion Questionnaire (LSQ) and anti-PGL-1 IgM antibody levels in household contacts of leprosy patients.\u0000Methods: This research was an observational analytical study with a cross-sectional design. The sample consisted of 30 subjects who were household contacts of leprosy patients. Anti-PGL-1 IgM antibody levels were examined using the ELISA method at the Cito Laboratory, Yogyakarta.\u0000Results: Regarding demographic characteristics, there were 18 female subjects (60%) and 12 male subjects (40%). LSQ+ results were found in 22 (73.3%) subjects, while LSQ- results were found in 8 (26.7%) subjects. The results of the anti-PGL-1 IgM examination were classified into three categories, namely seronegative (<605 U/ mL) totaling 25 people (83.3%), low titer seropositivity (605–1000 U/mL) totaling four people (13.3 %), and high titer seropositivity (>1000 U/mL) was one person (3.3%). The mean value of IgM anti-PGL-1 was 383.63 U/mL, with the lowest value being 0 and the highest being 1593 U/mL. Using Fischer's Exact showed no relationship between LSQ and IgM Anti-PGL-1 (p=0.287).\u0000Conclusions: No relationship was found between LSQ and anti-PGL-1 IgM antibody levels in household contacts of leprosy patients.
