A prospective study of 37 patients aged 28 to 59 years, who underwent gastroplication according to the original technique, was conducted on the basis of the Clinic of the Bashkir State Medical University. The indication for surgery was morbid obesity (body mass index 40 kg/m² or more). The quality of life of patients who underwent laparoscopic gastric plication for morbid obesity was studied by comparing the results of questionnaires and special research methods. Weight loss achieved as a result of laparoscopic gastric plication, correction of concomitant disorders in organs and systems, and normalization of metabolic parameters allow us to consider the intervention effective, and the results obtained good. Long-term results of treatment were monitored for up to 2 years. The assessment of the parameters of the quality of life of patients indicates that after bariatric interventions, positive changes are more related to the physical indicators of the quality of life and health status in general. At the same time, changes in mental health indicators, such as vitality, social and emotional functioning, were not statistically significant. In addition to the ongoing surgical correction, obviously essential in achieving stabilization of the psycho-emotional sphere of patients should be given a multidisciplinary approach using developed diets snd psychotherapeutic techniques. The study highlights the features of the regimen and structure of nutrition in patients after bariatric surgery, which can improve long-term results and improve the quality of life in patients with morbid obesity.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"22 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125141980","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There has been a gradual increase in the use of minimally invasive approaches in comparison with traditional approaches for aortic valve replacement. The advantages of using ministernotomy access in terms of faster recovery in the postoperative period and improving the patient’s quality of life in the long term after surgery are shown. The use of mini-accesses for the treatment of patients with cardiovascular pathology is a promising and safe direction.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"1548 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130213277","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Emergent surgery in such cases are associated with a high frequency of hernia recurrence (up to 30 %), and difficulties in diagnosing hernial strangulation lead to a long delay in operations and dramatically increase postoperative mortality up to 4–12 %. The review of the literature in the historical perspective reflects the use of videolaparoscopic techniques with diagnostic and therapeutic purposes for acute hernial incarceration. The experience of diagnostic videolaparoscopy for strangulated hernias of all kinds of localization accumulated in the world practice indicates a very high sensitivity of the method that allows recognizing hernial strangulation, differentiating it from other diseases, verifying organs that have undergone incarceration, and assessing their viability. However, targeted studies of the diagnostic capabilities of videolaparoscopy for strangulated hernias have not been conducted. By 2009, 7 articles had been published in the world press on the treatment of strangulated inguinal hernias by laparoscopic (TAPP) or endosurgical (TEP) methods. A meta-analysis of these publications showed that laparoscopic hernioplasty for strangulated hernias is a performable operation with acceptable results. Further studies have identified a number of physiological and socio-economic advantages of endosurgical operations compared to open methods of prosthetic hernioplasty. However, such operations remain the lot of a narrow circle of specialists. To date, there are no randomized trials or systematic reviews on hernioplasty techniques for TARR and TER in patients with strangulated inguinal hernias. The question about optimal method of eliminating a strangulated inguinal hernia is not covered in modern literature and is still pending.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"582 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-04-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"131891438","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It has been established that the morphogenesis of tuberculous peritonitis directly depends on such factors as the initial immune status of the patient, the virulence of the pathogen, the type of tissue reaction, and some genetically determined features of the coagulation and anticoagulation systems and fibroblast activity. A direct correlation of the morphological variant of peritonitis with the form of the clinical course of the disease and the development of surgical complications was noted. Based on the data obtained and the patterns identified, a functional classification of tuberculous peritonitis is proposed, taking into account the prevailing morphological variant of peritoneal tuberculosis, the phases of activity and the development of complications requiring surgical interventions. The developed classification has a clear practical focus, allows you to visually determine the morphological variant of the pathological process, its activity (according to the totality of indirect factors), as well as assess the presence of complications requiring surgical treatment.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"48 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-04-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"130082994","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The medical records of patients who were hospitalized in the past four years to the surgical departments were analyzed. In all the cases, the cause of the formation of pancreatic pseudocysts was biliary destructive pancreatitis. There were 86 such patients in total; all were female, their age ranged from 42 to 78 years. The procedure for draining pancreas cysts was performed under ultrasound guidance, using a GE Logik P 5 ultrasound scanner with attachments for color Doppler mapping with convex sensor 3.5–5 MHz. With formed postnecrotic cysts, patients underwent cystogastroanastomosis (CGA) under the control of ultrasound and EGD. The duration of the stay of patients in the hospital was 7–14 days. In 14 cases, patients were discharged from the hospital with drains under the supervision of a surgeon at the place of residence. Such complications of drainage of pancreas pseudocysts as bleeding, peritonitis and suppuration, were not observed. Cyst recurrence was registered in 2 cases after primary external drainage of the cyst cavity.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"10 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-02-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126736554","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

One of the most common manifestations of chronic pancreatitis is the development of postnecrotic pancreatic cysts of various localization with a corresponding clinical picture. Currently, the main types of surgical treatment for postnecrotic pancreatic cysts are: drainage and resection methods of treatment. Topographic and anatomical features and the presence of concomitant pathology in the anamnesis necessitate the development of new minimally invasive, safe and effective methods of surgical treatment of postnecrotic pancreatic cysts.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"93 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-02-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"129513698","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Despite the advances in medicine in the prevention, diagnosis and treatment of this disease, the PE epidemiological thresholds have not changed significantly over the past twenty years. Today, despite the routine use of mechanical and pharmacological thromboprophylaxis, the 30-day mortality rate after major surgeries ranges from 16.9 % to 31 %. Considering that more than 300 million surgical interventions are performed in the world, and more than 10 million operations per year in the Russian Federation, the scale of the problem can be easily imagined. The results of numerous studies confirm that at present, the problem of postoperative PE remains extremely urgent and requires further study. This manuscript provides a review of modern literature highlighting the key issues of the studied pathology.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"15 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-02-14","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127482585","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
