The informativeness of the methods for predicting the outcome of peritonitis was performed according to the parameters of sensitivity and specificity (Se and Sp). The sum of the proportions of correct forecast results characterizes the accuracy of the method (Ac). We also calculated the predictive value of a positive result (PPV; hereinafter referred to as predictive value), which is the proportion (probability) of unfavorable outcomes among all patients with a high risk of death. The article also demonstrates the assessment of predictive ability using the example of four common scales for predicting the outcome of secondary peritonitis. The results of the study showed that none of the studied systems for predicting the outcome of peritonitis is universal and absolutely reliable. Despite the fact that all the studied systems (scales) are used in clinical practice, none of them can completely satisfy surgeons, primarily in the choice of access, the volume of surgery and the option for its completion.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"63 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133431298","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

NETs are comprised of DNA, histones and neutrophil granule enzymes. As the result of the combination of all those components’ effects NETs play the key role in immunodefence, thrombosis and cancer progression. Due to procoagulant activity NETs act as the inducers of venous thrombosis and can be detected in thrombus and in plasma. The high levels of NETs concentrations in plasma are associated with thrombotic complications in trauma, infectious diseases and cancer. DNase decreases the incidence of thrombus and improve the results of thrombolysis in experiments. These results are seem to be very promising in clinical practice.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"180 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121092887","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients and Methods: 162 patients were included. 69 (42,6 %) — received preoperative conservative treatment. Ileocecal resection was performed in 148 (91,4 %), in 5 (3,1 %) cases — part of jejunum resection, ileum resection — in 3 (1,8 %) patients and right hemicolectomy — 6 (3,7 %). Stoma formation was in 104 (64,2 %) patients. Complications were registered in 25 (15,4 %) cases. Results: in univariant analysis it was found that young age up to 40 years, male gender and short operative time ( 150 min) are the predictors (р = 0,03, р = 0,03 и р = 0,02, respectively) to noncomplicated postoperative period (reduce risk in 10, 5 и 10 fold, respectively). The absence of conservative treatment before surgery increased the complications’ rate more than 3 times comparing to patients who received therapy (OR 3,2 CI 95 % 0,1–11,45; р = 0,06), but we failed to get significance, that is why multivariant analysis was carried out to see the influence of all clinical factors on non-treated patients. Significance was found in all models. Conclusion: male gender (OR 0,2 CI 95 % 0,01–2,02; р = 0,02), the age younger than 40 (OR 0,1 CI 95 % 0,02–0,9; р = 0,03) and the duration of the operation less than 150 minutes (OR 0,1 CI 95 % 0,01–2,02; р = 0,03) is associated with the reduction of complications in the postoperative period. Preoperative conservative treatment during 3 months allows to decrease the rate of complications to 3,5 times (OR 3,5 CI 95 % 1,2–9,8; р = 0,01) and risk of the stoma formation — to 7 times (χ2 = 7,56; р = 0,006).","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"84 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"133400185","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In women, they occur at an older age and 3 times less often than in men. An essential point in the disease pathogenesis is rapid growth of leiomyomas and their tendency to ulceration with decay, which leads to the development of formidable complications in the form of bleeding, perforation of the hollow organ, followed by the development of peritonitis. The growth of leiomyomas occurs from the thickness of the muscle layer in the form of a single node, mainly endogastrically in 66.5 % of cases. Exogastric growth was noted in 24.6 %, mixed in 5 % and intramural in 3.9 % of cases. A rare representation of benign stomach tumors of non-epithelial origin is characterized by an extremely wide variety of their types and names. A clinical case of surgical treatment of gastric leiomyoma using minimally invasive technologies in a patient with concomitant pathology is presented. The neoplasm was detected during a screening examination of the respiratory system. The patient underwent laparoscopic removal of the neoplasm. A macro- and micropreparation of gastric leiomyoma is presented. After surgical treatment, a positive result was achieved, the patient was discharged from the clinic in a satisfactory condition. The modern development of medicine and the use of video endoscopic technologies significantly expand the possibilities of surgery for neoplasms of the gastrointestinal tract. This observation demonstrates the features of the clinical picture of benign gastric neoplasms and the possibility of using minimally invasive surgical technologies in their treatment against the background of severe comorbid pathology.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"32 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2021-02-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127545604","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Infectious agents such as mycobacteria, fungi, foreign substances such as talc and barium; meconium; intestinal contents; contents of a ruptured ovarian cyst; gall stones; nematode eggs or unknown agents can cause granulomatous peritonitis. Most of the listed nosological forms of peritoneal lesions are not described, whereas vessels, lung tissue, intestines, kidneys, liver, lymph nodes are often the target organs. So far there are very fewpapers in the medical literature covering questions such as the spreading of granulomatous peritonitis, methods of their diagnosis, differential diagnosis and treatment. This review is an attempt to fill this gap.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"16 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-11-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114633105","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The positive effects of a combination of neoadjuvant chemotherapy with surgery are shown. A clinical case of treatment of a 43-year-old patient with a diagnosis of: cervical cancer IB2 stage T2a2N0M0. At the first stage of treatment, the patient was prescribed 3 courses of neoadjuvant chemotherapy in a combination of paclitaxel and carboplatin. There was a decrease in the size of the cervical tumor by 25 % on the RECIST scale. At the second stage of treatment, extended uterine extirpation was performed with the removal of the upper third of the vagina, fallopian tubes, parotid tissue and regional lymph nodes with ovarian transposition. The patient’s prognosis is favorable, and further dynamic monitoring is recommended. The presented clinical case demonstrates the possibility of using neoadjuvant chemotherapy in combination with paclitaxel and carboplatin and subsequent extended extirpation of the uterus type III with ovarian transposition in the treatment of stage IIA2 cervical cancer in a woman of reproductive age. After the first stage of treatment 3 courses of neoadjuvant chemotherapy, patient S. had a decrease in the size of the cervical tumor by 25 % on the RECIST scale according to pelvic MRI data, which later allowed to proceed to the second stage surgical intervention. Taking into account the known prognostic factors, the patient has a favorable prognosis for life and requires further dynamic monitoring.","PeriodicalId":437500,"journal":{"name":"Hirurg (Surgeon)","volume":"108 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2020-11-26","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116662619","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
