When analyzing a sample of 543 patients with purulent-destructive lung diseases in the Penza Regional Clinical Hospital, 45 (8.3 %) had purulent-inflammatory complications. The aim of the study is to create a neural network system for predicting the risk of surgical complications in patients with purulent-destructive lung diseases. As a result of this study, the technology of constructing neural network models for predicting complications in thoracic surgery was developed. Intraorgan application of Haemoblock allows endoscopists to identify significant features of biotissue defects and to optimize further actions to achieve reliable hemostasis and to prevent secondary bleeding. Clinical study of the diagnosis and treatment of 84 patients with gastroduodenal bleeding was presented. Letina","doi":"10.33920/med-15-2103-02","DOIUrl":"https://doi.org/10.33920/med-15-2103-02","url":null,"abstract":"The results of the analysis of medical records of 148 patients with acute appendicitis who were treated in the surgical departments of an infectious diseases hospital and in a general surgical hospital are presented. The patients were divided into 3 groups: I (main) — patients with a confirmed infectious disease, II (occult) — patients with excluded infectious disease, III (comparison group) — patients of a general surgical hospital. Patients treated in an infectious diseases hospital were admitted mainly within 24 hours from the onset of the disease, while patients with a general surgical profile — in the interval from 6 to 24 hours from the onset of symptoms. In the structure of morphological forms of acute appendicitis in infectious patients, gangrenous appendicitis prevailed, and phlegmonous appendicitis among general surgical patients. The gold standard for the treatment of muscle invasive bladder cancer (MIBC) is radical cystectomy with bilateral pelvic lymph node dissection. The high incidence of intra- and postoperative complications, as well as a large number of repeat visits (up to 25 %) in the first 30 days after open radical cystectomy necessitates a search and implementation of new methods of surgical treatment, one of which is robotic-assisted radical cystectomy (RARC). The robotic method reduces the need for intra- and postoperative blood transfusion, but does not significantly improve the patient’s quality of life compared to open surgery. The main disadvantage of the RARС is the long operation time. Urinary drainage is a necessary component of radical cystectomy. The most modern methods of derivation are «dry» stoma placement and orthotopic bladder surgery with urination along the native urethra. GSV hypoplasia is frequently observed anatomical type with twice higher incidence than GSV aplasia. Proximal GSV aplasia including sapheno-femoral junction (SFJ) is the rarest anatomical type that is always accompanied by anterior accessory saphenous vein (AASV) acting as the alternative drainage route in such cases. In the case of SFJ absence the AASV connects common femoral vein at the level of typical SFJ location. In this case report we present the situation of complete GSF and AASV absence with the subsartorious perforating vein as the proximal junction between superficial and deep veins. At the same time this perforating vein is the source of pathological venous reflux towards the varicose veins of the thigh and leg. There is no information about phlebectomy of GSV in this case but it is known that the accident of the knee trauma with a subsequent operation and also the operation on the GSV tributaries on the leg (puncture without avulsion according to the patient memories) took place in the past. As such events are associated with the risk of thrombotic complications the postthrombotic involution of GSV could be contemplated in this case as the reason of GSV disappearance. Foam sclerotherapy of the incompetent thigh perforator vein with miniphlebectomy on the thigh was performed. Reconstruction of the lower urinary tract, based on the Mitrofanov principle, improves the quality of life of patients. However, this operation has a significant number of early and late operational complications. We represent a small group of patients — 6 people who underwent continental cutaneous urine diversion in our modification, the purpose of which was to reduce incontinence. It negatively affects the quality of life of patients, especially in the elderly, which can lead to exhaustion, dehydration, aspiration pneumonia and, ultimately, death. Dysphagia and its severity should be assessed accurately and in a timely manner. Many questionnaires have been proposed to monitor the severity and effectiveness of dysphagia therapy. This paper analyzes ten different self-assessment questionnaires for quality of life. Almost all currently available questionnaires need to be finalized. This article describes a new method of treating a hernia of the esophageal opening of the diaphragm with laparoscopic access, using a combined xenopericard endoprosthesis as an implant. A clinical observation was carried out. Rodoman","doi":"10.33920/med-15-2102-05","DOIUrl":"https://doi.org/10.33920/med-15-2102-05","url":null,"abstract":"Clinical observations have shown that COVID-19-associated pneumonia is often accompanied by coagulopathy, an increase in C-reactive protein and D-dimer. Early laboratory assessment may represent a promising approach for predicting the development of thrombotic complications and the severity of COVID-19. Thrombotic complications often cause a rapid deterioration in the patient’s condition, the development of ARDS and a negative outcome of the disease. In this study, the authors sought to identify synthetic indicators that may be associated with the development of acute respiratory distress syndrome (ARDS) requiring treatment in an intensive care unit in patients with viral pneumonia caused by COVID-19. Differences in D-dimer levels and the ratios of D-dimer and C-reactive protein (CRP) levels to the number of days (Days) elapsed from disease onset to hospitalization were investigated between COVID-19 patients who were transferred to the intensive care unit, within two weeks after admission to the hospital (n = 78), and patients with COVID-19 with a stable course of the disease (n = 70). Multiple comparisons showed statistically significantly higher values of CRP / Days at admission in patients with COVID-19 transferred to the ICU (median 22,57) compared with stable patients (median 7,19), p < 0,0001. D-dimer levels on admission did not have statistically significant differences in these groups, while comparison of D-dimer / days ratios in patients transferred to the ICU and in stable patients revealed statistically significant differences: median 44,57 and 41,27, respectively (p = 0,0048).
