Os resultados clínicos foram avaliados pelo Score AOFAS Hallux MTP-IP. Os desfechos radiográficos incluíram ângulo do hálux valgo (AVH), ângulo intermetatarsal (AIM), ângulo articular distal do primeiro metatarso (AADM) e deslocamento do sesamóide (DS). Todos os pacientes tiveram seguimento mínimo de 6 meses. \u0000Resultados: Não houve diferença estatisticamente significativa entre os três grupos no escore AOFAS (p<0,001). Houve redução nas médias do AVH, AIM, DS e AADM do pré para o pós-operatório, em todas as técnicas em magnitudes semelhantes. Do total da amostra, 17 apresentaram algum tipo de complicação. Kainuma","doi":"10.30795/jfootankle.2024.v18.1752","DOIUrl":"https://doi.org/10.30795/jfootankle.2024.v18.1752","url":null,"abstract":"Objective: Evaluate the efficacy of a new orthotic-made super-elastic alloy brace to correct hallux valgus (HV) deformity.Methods: Five female patients diagnosed with HV deformity wore the newly developed orthotic daily for six weeks. The orthotic wasdeveloped using a super-elastic alloy brace to correct great toe lateral deviation, first metatarsal medial deviation, and fixation belts.The correction was evaluated by measuring the load at which the big toe begins to move and the distance moved by 15 gf force whenpulled from the side. Results: The orthotic was worn by all patients without causing any sleep disruptions. In the initial state, a strong force was required to move the big toe, but after two weeks, it was significantly improved to be mobile in all patients. Conclusion: The developed HV orthotic has a high wearability without causing pain or discomfort. Long-term use can provide a consistent and continuous correction force to the big toe, resulting in the successful release of the big toe metatarsophalangeal joint contracture within two weeks. Methods: An electronic search was conducted in the following databases up to June 2023: MEDLINE, EBSCO, Embase, CINAHL Plus, and PubMed. The PICO framework was employed to define search terms. Results: Despite considerable methodological heterogeneity among studies, a stronger inclination was found to investigate etiological and prognostic factors. There is a trend for a higher prevalence in boys with a positive family history. Novel classifications have been proposed to improve differential diagnosis. Among conservative treatment options, there is stronger evidence for gait improvement using serial casting followed by orthoses. Severe cases may benefit from surgical zone III gastrocsoleus lengthening. Conclusion: Further studies with standardized methodologies are required to clarify questions about this condition’s etiology, classification, and treatment. Nonetheless, there is a higher level of evidence supporting conservative treatments with serial casting and orthoses and zone III gastrocsoleus lengthening for severe cases in the second decade of life. Symptomatic cartilage lesions in the ankle pose a significant challenge and often require surgical intervention to alleviate pain, improve joint function, and restore the integrity of the articular surface. Possible treatment options, including operative and nonoperative management, havebeen widely discussed in the literature, with substantial heterogeneity regarding treatment approaches and resulting patient-reportedoutcomes and joint function metrics. A general problem regarding OLT is that this entity is not clearly defined, nor its description,location, or best treatment for each type that may be required in some symptomatic presentations. The focus of our paper was to show the proper treatment for each lesion, including location, deepness, vascularity, and capability of healing. O teste T-Student Pareado foi utilizado para comparar o American Orthopaedic Foot and Ankle Society Score e o Foot and Ankle Outcome Score entre os momentos pré e pós-operatório. Constatou-se que a média aumentou de forma significativa, no American Orthopaedic Foot and Ankle aumentou de 25,8 para 79,8 e no Foot and Ankle Outcome Score dor de 7,2 para 69,8.Ao correlacionar a Escala Visual Analógica de Dor e o Foot and Ankle Outcome Score Esportes, notou-se que quanto maior for o escore do Foot and Ankle Outcome Score no momento pós, menor será o Escala Visual Analógica da Dor e vice e versa. Baumfeld","doi":"10.30795/jfootankle.2024.v18.1775","DOIUrl":"https://doi.org/10.30795/jfootankle.2024.v18.1775","url":null,"abstract":"Resumo \u0000Objetivo: Apresentar os resultados clínicos e radiográficos de pacientes com hálux valgo (HV) tratados por técnica minimamente invasiva de 4° geração com guia para correção da rotação do primeiro raio. \u0000Métodos: Foram incluídos 22 pacientes nesse estudo. Todos os pacientes eram do sexo feminino, sendo 13 pés direitos e 8 esquerdos, com tempo médio de seguimento de 15 meses (12-18). Os parâmetros radiográficos avaliados foram o ângulo de hálux valgo (AHV), o ângulo intermetatarsal (AIM), a cobertura dos sesamoides (Hardy Clapham) e o formato lateral da cabeça do primeiro metatarso (FLCPM). O escore AOFAS, a EVA e as complicações foram avaliadas. \u0000Resultados: O AHV teve uma melhora do período pré para o pós-operatório de 26,8o para 4,2o e o AIM de 13,2o para 2,7o. Com relação aos sesamoides, no pré-operatório 03 pacientes apresentavam grau 06, 10 o grau 05 e 09 o grau 04. No pós-operatório, 09 pacientes apresentaram o grau 02 e 12 pacientes o grau 01. Antes da cirurgia, o FLCPM era tipo intermediária em 17 pacientes e tipo redonda em 04 pacientes.  No pós-operatório, todos os pacientes foram classificados como tipo angular. O AOFAS médio aumentou de 45 para 91 pontos e a EVA média teve diminuição de 06 para 01. A complicação mais comum foi aderência da cicatriz cirúrgica em 4 pacientes. \u0000Conclusão: A técnica minimamente invasiva de 4° geração com controle rotacional apresentou correção triplanar da deformidade do HV. Muitos questionários são encontrados apenas na língua inglesa e é necessária uma adaptação linguística e cultural para o uso preciso da ferramenta pelos pesquisadores. O objetivo deste estudo é realizar a tradução, adaptação cultural e avaliação da reprodutibilidade do Questionário “American Orthopaedic Foot And Ankle Society (AOFAS) Hallux Metatarsophalangeal-Interphalangeal Scale” para a língua portuguesa. \u0000Métodos: Foram aplicados os questionários AOFAS e SF-36 em 50 pacientes com patologias de hálux. A metodologia seguiu os critérios definidos por Reichenheim & Moraes para tradução e adaptação cultural de questionários. \u0000Resultados: O coeficiente alfa de Chronbach dos domínios foi de 0,99, considerado excelente. Na primeira e segunda avaliação dos domínios os valores foram 65,5 e 64,2 respectivamente. A concordância inter-observador foi elevada (65,5 x 65,3). We present a 59-year-old male patient with diabetes mellitus and an extensive and infected lesion in his right ankle region, suggestive of a diabetic foot. Urgent surgical treatment was performed with debridement of the devitalized tissues, drainage of a large amount of purulent secretion, and exhaustive washing with 0.9% saline solution. After a new debridement, abundant washing with 0.9% saline solution and coverage of the extensive bloody area with a polypropylene prosthesis were performed. He remained under outpatient follow-up, and subsequently, twice, new cleanings were performed, and a small diameter mesh was applied due to the good evolution of healing. A good range of motion was maintained in the right ankle and foot, with the medial wound completely healed, leaving only a small area of the lateral wound to complete the healing process. Figueiredo’s technique, described in 2017 for fingertip lesions, uses a flexible thermoplastic polymer, which can be obtained from a small portion of silicone from a saline bag, to form a semi-occlusive dressing that allows the drainage of secretions and provides a favorable environment and temperature for healing. The exact limits of this technique are not yet well established, but the practice has shown excellent results in larger lesions. Figueiredo’s technique is a simple, low-cost, and effective option for treating extensive infected skin lesions. Automated analysis of weight-bearingcomputed tomography (WBCT) images has been investigated to diagnose various pathologies. However, it has not been studied forLisfranc ligament injuries. The objective of the study was to examine whether automated WBCT analysis could demonstrate diagnosticutility for these injuries. Methods: Serial sectioning of Lisfranc complex ligaments was conducted on 24 cadaveric limbs to simulate Lisfranc injuries. WBCT images were collected at each dissection condition under three loading conditions. Images were automatically segmented, and automated measures of specific angles and distances in the midfoot were calculated using digitally reconstructed radiographs. These were analyzed using repeated measures ANOVA and paired T-tests to identify significant differences between dissections at each loading condition. Results: Overall, minimal differences between dissection conditions were observed in automatically generated measures. Differences in axial angles of the metatarsals in severe dissections were observed, and there were fewer differences in angular measures across dissection conditions in fully loaded than unloaded conditions. Conclusions: Automated analysis of WBCT images may indicate severe Lisfranc ligamentous injury but is insufficient to diagnose ligament injuries without full capsule disruption. This lack of injury markers may be due to the imaging conditions, automated analysis, or biomechanics of Lisfranc injuries. More alignment differences were seen under unloaded conditions, suggesting that weight-bearing imaging may not be appropriate for this injury. Overall, automated analysis shows only minimal changes in alignment measures, and additional study is necessary to improve diagnostic tools for Lisfranc injuries.
