{"title":"[Subjective well-being and ability to work after SARS-CoV-2 immunization with the vector vaccine ChAdOx1-S (AstraZeneca COVID-19 vaccine)].","authors":"Johannes Kalbhenn, Feline Gabler, Sebastian Heinrich, Daniel Steinmann","doi":"10.1007/s40664-021-00448-4","DOIUrl":"https://doi.org/10.1007/s40664-021-00448-4","url":null,"abstract":"<p><strong>Background: </strong>The SARS coronavirus 19 vaccine ChAdOx1‑S (Vaxzevria, AstraZeneca) has been licensed since January 2021 by the Paul Ehrlich Institute for Germany. In several campaigns, healthcare workers and medical students were offered this vaccine on a voluntary basis.</p><p><strong>Aim: </strong>The primary endpoint of the study was to assess the rate and duration of the incapacity to work as a result of initial immunization with ChAdOx1‑S. Secondary endpoints were type and severity of adverse events and self-perceived tolerability.</p><p><strong>Material and methods: </strong>Anonymized online questionnaire to be completed once by all vaccinated individuals after receiving the first dose of ChAdOx1‑S. The severity of side effects was queried using an ordinal numerical rating scale with values ranging from 0 to 10. Other key data points were age, sex, and occupational group. Ability to work in the days following the injection was also assessed by self-reporting.</p><p><strong>Results: </strong>Data from 1988 respondents were analyzed. The mean age was 37.13 years (standard deviation 13.7 years). Of the respondents 69.8% were female, 48.1% belonged to therapeutic and technical professions with patient contact, 38% were students, 10.6% were nursing personnel and 4% were physicians. Only 14.4% of respondents reported having tolerated the vaccination without side effects. The most common side effect was fatigue, followed by pain at the injection site. This was followed in descending frequency by headache, aching limbs, and chills. After vaccination 18% of respondents felt able to return to work immediately, 51% of all respondents had to report themselves unfit for work for at least 1 day after vaccination. Side effects were more prevalent in male and younger respondents.</p><p><strong>Conclusion: </strong>Vaccination with ChAdOx1‑S frequently resulted in side effects. These resulted in 37% of respondents reporting sick. As a result of the lockdown, training possibilities were mostly limited to nonspecific home-based training methods. It is unclear whether the lack of sport-specific stimuli led to a reduction in the physical performance of soccer players.</p><p><strong>Methods: </strong>For the narrative review, the PubMed, Google Scholar and BISp-Surf databases were selectively searched for studies examining the effects of the lockdown on physical performance parameters in adult professional soccer players.</p><p><strong>Results: </strong>In this review six prospective longitudinal studies were included. In all studies home-based training was performed during quarantine. Of the studies four compared the performance of the soccer players with data from previous seasons and two studies assessed the performance of players immediately before and after the lockdown period.</p><p><strong>Discussion: </strong>While general strength and endurance performances can be maintained through home-based substitute training programs, the studies indicated that the lack of football-specific stimuli could have an impact on the rapid power and speed performances of soccer players. How well OHS is organized and how measures are actually implemented are not yet empirically known. This gap was closed with an epidemiological study on the status quo.</p><p><strong>Material and methods: </strong>In the second half of 2020 a total of 120 managers of day care centers in Germany, mostly church-run, which are member companies of the German Statutory Accident Insurance and Prevention in the Health and Welfare Services (BGW), were randomly selected and interviewed. This was done by members of the BGW prevention services using a standardized survey instrument. The surveyed aspects (mostly concerning the OHS organization) were summarized in a standardized sum index between 0 and 1 (worst to best occupational health and safety) and analyzed descriptively.</p><p><strong>Results: </strong>The requirements for the organization of OHS were fulfilled in many cases. There is still some potential left for improvement in the actual implementation of organized OHS requirements. The standardized sum index is 0.82 (standard deviation 0.16). Of the respondents two thirds rated OHS in their day care center as good/very good in a global item. In free texts, the desire for more information on OHS and better communication between the day care center and the respective provider was frequently expressed.</p><p><strong>Discussion and outlook: </strong>The results confirmed a higher formal quality of OHS as found in other cross-sectional studies. To continuously improve the actual implementation of organized measures, OHS management systems, which are still rarely used in day care centers, could have a supporting effect. Corresponding instruments are available and their use should be more strongly promoted in the future.
