Chauhan, Dr Priyanka Goswami","doi":"10.56018/bjkines20220618","DOIUrl":"https://doi.org/10.56018/bjkines20220618","url":null,"abstract":"A new COVID-19 related clinical syndrome in children called Multisystem inflammatory syndrome in children (MISC) has been reported, with significant inflammation and similarities to Kawasaki disease (KD) with concurrent SARS-CoV-2 in Europe and in USA since 7 April 2020. On 25th April, concerns were initially raised in the United Kingdom regarding a cluster of children of various ages, presenting with a Multisystem inflammatory state who required intensive care, and who all displayed \"overlapping features of toxic shock syndrome and atypical Kawasaki disease with blood parameters consistent with severe COVID 19 in children. Clinical presentations were variable, with significant gastrointestinal (GI) symptoms, cardiac disease, mild or absent respiratory symptoms, and variable incidence of rash, red eyes, oral mucous membrane changes. Later on this was diagnosed as part of a post covid Multisystem inflammatory disease (MISC). We report one patient admitted in Civil hospital, Ahmedabad with high grade fever for more than three days along with manifestations of multisystem disease involving cardiovascular system with myocarditis,and gastrointestinal system in form hepatitis, widespread blanchable rashes and respiratory distress due to pleural effusion.The clinical features had similarly to features of Kawasaki disease. Patients also had raised inflammatory markers in form of increased CRP, ESR, PT, APTT. Covid IgG antibody was found positive and patient was treated as per MISC protocol with steroid, IVIG and aspirin together with proper supportive management in form of oxygen through High flow nasal cannula and ionotropes for shock. Multiple theories on pathogenesis have been suggested in form of Antibody dependent enhancement precipitating kawasaki like illness due to cytokine storm provoked by type 1 and 3 interferon. Role of STING (Stimulator of interferon gamma) pathway has been found to be involved in MISC. With proper treatment as per protocol it is possible to successfully discharge patients with MISC admitted in decompensated states. Material and Method: The study is a prospective comparative study which was conducted on 50 patients of diabetic limb ulcers admitted in General Surgery Department at Tertiary care hospital between June 2017 to June 2019. Patients were followed up until discharged from the hospital or completion of 8 weeks assessment period from the date of admission whichever was earlier. Result: Ulcer due to diabetes is common in males. 4% of the patients had hba1c level between 6.5 to 9.5. In 64% of patients granulation tissue appeared at the end of 2 weeks after VAC dressing while in only 40% of patients granulation tissue appeared after 2 weeks of conventional dressing. In 80% of patients wound was completely covered by granulation tissue at the end of 4 weeks after VAC dressing while only in 52% wound was completely covered by granulation tissue after 4 weeks of conventional dressing. Conclusion: VAC therapy is better than conventional therapies in terms of wound healing, less complications, reducing the hospital stay, reducing the bacterial infection load and is cost effective. We report a case series of 12 patients who were bitten and had atypical clinical presentation affecting CNS. We evaluated 12 patients of Indian red scorpion sting by taking history, routine blood investigations, CT scan of brain, and CNS examination at a tertiary care hospital in Gujarat. We observed that if patient takes longer time after sting to visit hospital, there are more chances of development of CNS signs and symptoms and indicates poor prognosis. Indian red scorpion venom is a potent sympathetic stimulant and cardio-toxic agent. It can also cause haemorrhagic and thrombotic stroke, cerebellar stroke and cerebellitis. Prazosin is the treatment of sting as there is no availability of anti-venom for sting. Prognosis is directly related to duration between scorpion sting and initiation of treatment. Material and Methods: It was a Prospective randomized comparative interventional study on total 111 patients, aged 25-60 years of either sex with ASA physical status I and II who were randomly allocated into three groups (37in each group). Group A: Received inj. Etomidate 0.3 mg/kg IV, Group B received inj. Propofol 1.5 mg/kg IV + inj. Ketamine 0.5 mg/kg IV, Group C received inj. Thiopentone 3 mg/kg + inj. Ketamine 0.5 mg/kg IV; at induction of anaesthaesia. Haemodynamic parameters (SBP, DBP, MAP, and HR) were noted at baseline, after induction, just before intubation, and 1, 3, 5, 10 and 15 min after intubation. Side effects like myoclonus, post-operative nausea and vomiting (PONV) and hallucinations were also noted. Statistical analysis was performed using ANOVA test and chi-square test (P significant). Results: Hemodynamic parameters were increased in all groups after intubation and was minimum in Group B compared to Group A and C at one min post intubation. Myoclonus and PONV were observed more in Group A than Group B and C (statistically nonsignificant in all three groups). Conclusion: Propofol-ketamine combination is a safe and effective induction agent with better hemodynamic stability as compared to Etomidate and thiopental-ketamine in general anaesthesia. The greater part of it is situated under cover of morphological variations of the human liver and its clinical implications. Materials and method: A total of 32 formalin-fixed adult human livers, irrespective of the sex, were studied over a period of three years from Dr. M. K.Shah Medical College. These livers were specifically observed for any riedels loab morphology. Results: out of 32 specimens, 22 were considered normal without any accessory lobes, 10 Conclusion: Riedel's lobe is no longer mistaken for an abdominal tumor by using imaging. However, the condition of \"Riedel's lobe\" or the extreme case of a downward elongated right hepatic lobe still seems important since the recognition of its existence leads to the correct diagnosis of right abdominal palpable mass, or correct depiction of the tumors within the lowest part of the elongated normal liver. Present study aimed to analyse demography, epidemiology, clinical features and outcome in children infected with COVID-19 admitted at a tertiary care institution. Material and Method: A retrospective study was carried out evaluating data of 209 cases, admitted at a dedicated COVID hospital from March 2020 to February 2021. Results: Data of 209 children, from age 1 month to 12 years with confirmed diagnosis of COVID-19 with a mean age of 4.7±3.6 years and malefemale ratio of 1.09:1 were studied. 177(84.68%) patients were from identified containment zone. Overcrowding was observed in 115 (55.02%) cases. Close contact with positive family members was observed in 166 (79.42%) cases with majority coming from lower middle socio economical class. 60 (28.71%) patients were asymptomatic and 149 (71.29%) were symptomatic. Among symptomatic patients, 118 (79.19%) patients had fever as the main presenting complaint. Positive findings on chest radiograph were found in only 25 (11.96%) patients. In asymptomatic and mild cases, chest radiograph was found to be normal. Vitamin D deficiency was observed in 170 (81.34%) patients. A total of 206 patients (98.56%) recovered while 3 patients (1.43%) died. Conclusion: A milder disease pattern is observed in majority of children with COVID 19. There is higher disease burden observed in lower middle socio-economic class with majority of children having positive household contact. Majority of cases recover with supportive treatment while treatment modalities like oxygen therapy, mechanical ventilation, steroids and low molecular weight heparin are required only in moderate to severe case. Mortality is observed in patients with severe disease and havingco-morbidities. Biceps brachii anomalies may play an important role in neurovascular compression syndromes in the arm and also affect shoulder joint movements. Three significant morphological variations in Biceps brachii were noted during routine dissection of upper limbs used for undergraduate teaching purposes. An extra articular origin of tendon of long head of Biceps brachii which was passing through the subpectoral tunnel formed by Pectoralis major was observed in one right upper limb. Nonfusion of two bellies of Biceps brachii with an accessory bicipital tendon was observed in one right upper limb. Presence of one supernumerary head of Biceps Brachii with a communicating branch between Median and Musculocutaneous nerves was observed in one right upper limb. Knowledge of these variations is essential for Surgeons and Orthopaedicians during surgery involving shoulder, arm and elbow regions. Thus it is in global interest to provide them social security. Indian society is witnessing a gradual withering of joint family system leading to a large number of neglected parents and their exploitation. A number of social security measures have been taken by Indian government. This study was conducted to know the distribution for various social security schemes among elderly utilizing them at Ahmedabad. Material and Method: It is community based cross sectional study conducted from June 2019 December 2020 with the total 1078 sample using purposive sampling method. A self- developed semi-structured proforma was prepared in English and vernacular languages to collect information regarding distribution about social security schemes among the elderly. Result: Distribution of social security schemes was found to be varied with major inclination towards LIC. Friends are major source of information & helping hands to avail them. Conclusions: The data obtained suggest that more marketing strategy is needed for all the government social security schemes similar to LIC as found in our study. Most are sporadic tumors that present in the fourth and fifth decade of life. Classic triad of episodic headaches, sweating and tachycardia present in about 30%, while hypertension is present in almost 90 % of cases. Clinical suspicion with laboratory testing and imaging for confirmation. Histopathological examination is gold standard, but no single biomarker or histologic feature predicts malignancy. Pheochromocytoma of the Adrenal Gland Scaled Score (PASS) provides prognosis based on histologic features. Five patients of pheochromocytoma with PASS score are described with the aim to represent a rare neoplasm. They are positive for chromogranin, synaptophysin and S100.
