Patel, UV Shah","doi":"10.56018/bjkines2023062","DOIUrl":"https://doi.org/10.56018/bjkines2023062","url":null,"abstract":"Background: The main aim of this study was to monitor the antimicrobial resistant pattern of Salmonella isolates to select proper antibiotic & prevent drug resistance in Salmonella species. Material and Methods: Total 250 blood culture bottles were collected from patients clinically suspected enteric fever and loaded to BACTEC 9050. Out of them 200 signalled positive samples were inoculated on chocolate and Mac Conkey agar plates and incubated overnight at 370 C in the incubator and identified by colony characteristics, procedures like Gram staining, wet preparation for motility and bio-chemical reactions like oxidase test, catalase test, triple sugar iron agar, Citrate test, using Salmonella antisera like Poly O, O-9, and H-d. The clinical isolates were subjected to antibiotic sensitivity test on Mueller-Hinton agar, using modified Kirby Bauer disc diffusion method as per Clinical Laboratory Standard Institute (CLSI) guidelines. Results: Total 61 Salmonella were isolated. 49 were Salmonella typhi and 12 were Salmonella paratyphi. Overall rate of resistance of 49 S. typhi isolates was 2 % to ampicillin, 4 % to Azithromycin, 4 % to fluoroquinolones and 2 % to co-trimoxazole. S. paratyphi isolates were 100% sensitive to ampicillin, co-trimoxazole, chloramphenicol and 3rd generation cephalosporins. Conclusion: Enteric fever is one such infection which poses challenges in antimicrobial resistance. Comprehensive blood utilization review and reporting to management ensures that corrective actions are taken when needed. Meeting is held annually in our setting to discuss practices within the hospital and scope of improvement. Materials and Methods: A total of 23 patients with recurrent pneumothorax (15) and recurrent pleural effusion (8) were enrolled. Pleurodesis with either talc or povidone iodine was performed via intercostal drainage tube. Treatment outcome along with complications was studied by routine follow up of patients for 6 months. Results: A total of 23 patients (19 males and 4 females) were included in the study. 16 patients were subjected to povidone iodine pleurodesis while 7 patients underwent talc pleurodesis. Mean age of patients was 38.68+19.6 for the povidone iodine group and 43.85+16.32 for the talc group. Eight (34.78%) patients had recurrent pleural effusion whereas 15 (65.21%) patients had recurrent pneumothorax. Post procedure pain was experienced by 5 (31.25%) patients in the povidone iodine group and 4 (57.14%) patients in the talc group. Two (12.5%) and one (14.28%) patient had fever following povidone iodine and talc pleurodesis respectively. Pleurodesis was successful in 13 (81.25%) patients in the povidone iodine group and 7 (100%) patients in the talc group, during the follow up period of 6 months. Materials such as autologous cartilage (tragal or conchal) or allograft (homograft) of the same tissue and synthetic materials like polyethylene TORP, ceramic and hydroxyapatite PORP or Titanium prosthesis have been used. But the latter synthetic materials are expensive and have high extrusion rates. Autologous cartilage grafts are selected for Ossiculoplasty as they are easy to harvest at the same site of operation, non-toxic, has less extrusion rate, minimum shrinkage and lateralization. Aims and Objective: The aim of this study is to know the efficacy of autologous graft and prosthesis in ossicular reconstruction and to evaluate hearing outcome along with extrusion rate among them. Methodology: It is a prospective study done on patients attending ENT outpatient department in civil hospital Ahmedabad affiliated to BJ medical college Ahmedabad. 60 patients were included in the study who underwent ossicular reconstruction either with canal wall up or down mastoidectomy with tympanoplasty. Conclusion: On follow up of 60 patients over six month we followed there is significant air bone gap reduction with prosthesis but there is risk of extrusion which cancel out beneficial effect of prosthesis. Whereas cartilage and incus remodelling give good result in air bone reduction and better take up compare to prosthesis with less extrusion rate. According to earlier studies serum folic acid and vitamin B12 were found to be reduced in such patients. Therefore, the present study was conducted to estimate and compare the serum folic acid and vitamin B12 values of diabetic mellitus patients with and without nephropathy. Materials and methods: A single institution based cross sectional study was conducted in Kolkata from March 2018 to September 2019.203 adult with at least five years history of type 2 diabetes mellitus were subdivided into two groups’ nephropathy and non-nephropathy. The cut off value for inclusion into nephropathy group was urinary albumin-creatinine ratio of more than equal to 30gm/dl. Serum folic acid and vitamin B12 values of both groups were estimated by Chemiluminescence assay and later statistically compared for result. Result: Overall 127 patients were grouped in non-nephropathy while rest 76 belonged to nephropathy. Both folic acid (4.68±3.64 versus 7.02±3.89; p<0.0001) and Vitamin B 12 (219.54±56.53 versus 343.98±32.1; p<0.0001) values were found to significantly lower in the nephropathy group. Non-nephropathy patients also displayed better glycemic profile in terms of HbA1c.Conclusion: Patients with diabetic nephropathy displayed lower levels of serum folic acid as well as vitamin B12 in comparison to the patients who did not develop nephropathy. It is potentially life-threatening and can cause serious maternal morbidity and mortality if undetected or misdiagnosed. The aim of this case series is to study clinical presentation, diagnosis, and treatment of caesarean scar pregnancy. We studied 10 confirmed cases of cesarean scar pregnancy treated in department of OBGYN at a tertiary care hospital, in western India over a period of 2 years. Maternal age ranged between 25 years to 38 years and gestational age ranged between 6 weeks to 10 weeks. Seven patients presented with symptoms like bleeding per vaginum and/or lower abdominal pain and three patients were asymptomatic. Transvaginal sonography was done for confirmation of diagnosis. In all patients, beta hCG levels were done on the first day. 4 patients were initially treated by medical management but was unsuccessful. Surgical management was done by laparotomy in 9 patients and in 1 patient laparoscopy. Thyroid dysfunction prevalence in diabetics patient was objective of the present study conducted at Jhalawar medical college and SRG Hospital Rajasthan. Materials and Methods: Retrospective study conducted in patients admitted between July 2017 to December 2018 at department of medicine Jhalawar medical college and SRG Hospital, Jhalawar Rajasthan. 130 patients (age group ≥ 30 yrs.) of diabetes mellitus and 125 of same age group & having glucose levels normal were taken for the study. The Ethical committee clearance was taken. Fasting plasma glucose (FPG) levels of more than 126 mg/dl twice at different occasions were labeled diabetics and subjects having FPG levels less than 126 mg/dl at two occasions were controls. Result: Prevalence of thyroid dysfunction in case group (diabetic mellitus patient) was 24.6% and prevalence of thyroid dysfunction in control group was 6.4%. Among thyroid dysfunction, prevalence of subclinical hypothyroidism was highest. Aims: To study the correlation of ocular manifestations & systemic COVID-19 status with clinical risk factors, during first & second outbreaks (FO & SO) of COVID19 pandemic in western India. CS RT-PCR results of diagnosed COVID-19 patients with positive nasopharyngeal swab (NPS) RT-PCR with or without ocular manifestations to be evaluated. Material and Methods: Prospective study conducted with confirmed COVID-19 cases with positive NPS RT-PCR during FO & SO of COVID-19 pandemic in year 2020 & 2021 in western India. Ocular manifestations & CS RT-PCR results were evaluated with systemic COVID-19 status. CS was collected within 48 hours of admission for RT-PCR analysis with repeat test within 72 hours upon increasing severity of COVID-19 stage. Result: Thirty-two (20.51%,13 FO,19 SO) patients had uniocular complaints, redness being the commonest during both the outbreaks. Out of 32 patients with ocular symptoms (OS), 87.5% had refractive error, 75% had diabetes & 53.1% had moderate COVID-19, (P=<0.0001, significant). 25% & 18.8% patients with OS had history of use of protective eye gears & covid vaccine respectively. 103 & 80 CS from 103 & 53 patients of FO & SO respectively were tested. All primary (183) & repeat (93) CS were negative for RT-PCR.Conclusion: Ocular involvement in diagnosed COVID-19 cases is variable. HPV virus is a major infectious etiological factor of cervical cancer. It has been proven that HPV vaccine, if taken before initiation of sexual life, is highly effective in HPV and cervical cancer prevention. Despite its availability, deaths due to cervical cancer has increased rapidly. Healthcare workers, especially the young medical students can play a significant role in recommendation of HPV vaccination among people. So, this study was conducted to study the knowledge, attitude and practices of HPV infection and vaccination among medical students. Methodology: A cross -sectional observational study was conducted in medical students of Narendra Modi Medical College. Three hundred medical students were requested to fill a questionnaire which covered demographic details, knowledge, attitude and practice towards HPV vaccination for cervical cancer prevention. Results:82.66% of students were aware about the HPV infection out of which 71% of the participants knew that cervical cancer can be prevented by HPV vaccine, 66.66% of studentes aware of the availability and 35% were well aware about the correct schedule/dosage of vaccination. Material And Methods: A prospective cross-sectional study on 116 eyes of 58 patients (20 normal patients, 28 POAG patients and 10 OHT patients) attending the out-patient’s department of a tertiary eye care hospital was carried out. All patients underwent a complete ophthalmic examination including visual acuity measurement, visual field examination, contrast sensitivity measurement, colour vision testing and stereopsis. Results: 58 patients with a mean age of 44.2 (± 9.17) years were enrolled in the study. Mean visual acuity was 0.0181 log unit (± 0.027); contrast sensitivity 1.79 log unit (± 1.14) and stereopsis 117 secs of an arc (± 59.92) in the normal group with normal colour vision and visual fields. The same was 0.09 log unit (± 0.07), 1.36 log units (± 0.16) and 270 seconds of an arc (± 106.45) respectively, colour vision defective in 60.71% and visual field defects in 87.50% amongst the POAG group. Mean visual acuity was 0.0291 log unit (± 0.03), contrast sensitivity 1.537 log unit (± 0.06), steropsis 204 seconds of an arc (± 57.96), defective colour vision in 25% cases and no patient with visual field defects amongst the patients having OHT.
