This may undermine care commissioning and development to institutionalized elderly. Data to inform policy and practice are needed.</p><p><strong>Objective: </strong>To identify clinical diagnoses and dependency levels among elderly residents in long-term care institutions in Lebanon.</p><p><strong>Methods: </strong>Cross-sectional survey of long-term care institutions throughout Lebanon as identified by the Mihistry of Social Affairs.</p><p><strong>Results: </strong>Thirty-one out of 42 long- term care institutions have been included in this survey. Among them, 1371 elderly subjects were included in the analysis; 75.6% were over the age of 75, and female repre- sented 67%. Medical morbidities and associated disabilities have driven admission in 70.5% of residents; 45.6% of resi- dents reported dernentia, stroke or other neurodegenerativb disease. Overall, 60% of residents required locomotor assis- tance.</p><p><strong>Conclusions: </strong>Elderly residents in long-term carb institutions are predominantly females, with mental and/or locomotor disabilities resulting fror neurological and ar- thritic conditions. Targeting healthcare for such health condi- tions remains a challenge for the institutions.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. Its deficien- cy can cause rickets or osteomalacia. This is particularly im- portant in hemodialysis (HD) patients who are at icreased risk due to decreased sunlight exposure and deterioration of their mineral homeostasis.</p><p><strong>Objectives: </strong>To determine the prevalence of 25 (OH) vitamin D deficiency in HD patients at Rafic Hariri University Hospital (RHUH), compared to a sample of the general population matched for gender and age, and to evaluate the effectiveness of 25 (OH) vitamin D supplementation in HD deficient group.</p><p><strong>Methods: </strong>This is a cross sectional study conducted since December 2012, comparing the prevalence of 25 (OH) vitamin D deficiency in HD patients in the dialysis center at RHUH, with patients from the general population who sought medical attention at RHUH for purposes other than HD, matched for age and gender. 25 (OH) vitamin D levels were measured with radio- immunoassay method (LOINC) at CIC European Lab, Bar- celona, Spain. A pilot study was conducted with the 34 HD patients who turned out to be deficient or insufficient in 25 (OH) vitamin D. We supplemented them with cholecalciferol over 6 months. We then assessed their vitamin D levels, and biochemistry parameters.</p><p><strong>Results: </strong>The prevalence of 25 (OH) vitamin D deficiency in the sample of HD patients at baseline was 32% while that of insufficiency was 36%. The prevalence of 25 (OH) vitamin D deficiency in the sample of general population was 67%. No correlation was found be- tween 25 (OH) vitamin D levels and the studied parameters. In the pilot study, after six months of cholecalciferol supple- mentation, there was a significant improvement in 25 (OH) vitamin D levels in the deficient and insufficient groups.</p><p><strong>Conclusion: </strong>The sample studied in the general population showed high prevalence of 25 (OH) vitamin D deficiency (67%). The sample studied in HD patients showed a preva- lence of 25 (OH) vitamin D deficiency of 32% and insuffi- ciency of 36%. The pilot study showed that 25 (OH) vitamin D supplementation in the form of cholecalciferol is beneficial in HD patients.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. Strict preoperative planning is of paramount\u0000importance for this surgery. Thorough clinical history and\u0000physical exam, the assessment of limb deformity and knee\u0000range of motion as well as knee stability in flexion, extension\u0000and mid-flexion are crucial. Blood exam, standardized radiographic\u0000views, and CT scan are powerful tools for etiologic\u0000diagnosis of total knee arthroplasty failure. Templating is\u0000unique and mandatory to provide the surgeon with the\u0000critical data concerning the valgus position of the femoral\u0000component, the AP size of the femoral component, and the\u0000optimal position of the joint line; these three parameters are\u0000determinant for the final clinical outcome. A stepwise surgical\u0000technique with close adherence to the guiding principles of\u0000revision knee arthroplasty should be adopted from skin incision\u0000to closure. Femoral and tibia components with modular\u0000stem are ideally set at their optimal position as predetermined\u0000by templating. Any residual implant-bone gap is filled\u0000with metallic augment or bone graft. We evaluated the reliability of the sural\u0000flap in treating such defects among a diabetic population.</p><p><strong>Material and methods: </strong>This is a continuous retrospective\u0000series of 14 patients with type 2 diabetes treated with an\u0000ipsilateral sural flap for soft tissue defects around the rear\u0000foot (11 cases) and over the malleolar areas (3 cases).\u0000Three patients had an open tibia fracture (Gustillo IIIb),\u0000four had chronic osteitis and seven had a chronic heel\u0000ulcer.</p><p><strong>Results: </strong>The mean follow-up at 28 months showed\u0000healing of the flap at a mean of 24 days, donor site healing\u0000in two weeks, one case of total flap necrosis, three\u0000cases of skin edge necrosis, two cases of temporary\u0000venous congestion and 10 cases of hypoesthesia of the\u0000lateral border of the foot. No infection or recurrence of\u0000infection was encountered.</p><p><strong>Conclusion: </strong>We found the\u0000sural flap useful, reproducible and reliable in treating soft\u0000tissue defects in diabetic patients with a low frequency of\u0000serious complications.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. Minimally\u0000invasive techniques for reduction and internal fixation of\u0000many types of these fractures have recently been widely\u0000promoted, especially with the use of the newly developed\u0000locking plate systems. Intramedullary flexible nailing is one\u0000of the oldest techniques using minimally invasive reduction\u0000and fixation of proximal humerus fractures. Kapandji technique\u0000uses the “Deltoid V” landmark as entry point for intramedullary\u0000insertion of the flexible nails into the humeral\u0000head. The authors report their experience with this procedure\u0000in twenty-six, relatively young patients with good bone\u0000quality, presenting with displaced 2- or 3-part extra-articular\u0000fracture of the proximal humerus, treated with percutaneous\u0000reduction and intramedullary flexible nailing as described by\u0000Kapandji. Nineteen medical records were available for this\u0000retrospective review, with 9 to 12 months follow-up. In this technique the K-wires of the\u0000TBW are replaced by a cerclage wire while keeping the figure\u0000of eight wiring.</p><p><strong>Material and methods: </strong>We reviewed retrospectively\u0000our WTBW cases done between 2000 and 2015\u0000where we replaced the K-wires by a cerclage wire. In this\u0000technique no hardware migration is possible. Patients were\u0000evaluated clinically, radiographicaly and a DASH score was\u0000measured.</p><p><strong>Results: </strong>Seventeen patients were reviewed with\u0000a mean age of 58.5 years. The mean follow-up period was\u000058.5 months. The mean DASH score was 12 with 7 patients\u0000having a DASH score of zero. Joint mobility was near normal\u0000compared to the other side with loss of a mean of 4º in elbow\u0000extension and a mean of 3º in elbow flexion. In comparison\u0000with other series, in addition to good results, hardware removal\u0000for medical reasons was the lowest in our technique. It\u0000was needed in three patients for pain on elbow contact and in\u0000one with ulnar nerve irritation. This represents a rate of\u000023.5%.</p><p><strong>Conclusion: </strong>Undesirable events related to the use of\u0000K-wires in standard tension band wiring, such as wire migration,\u0000wire protrusion through the skin and wire impingement,\u0000are absent in the wireless tension band wiring. The high rate\u0000of patient satisfaction, good clinical results as well as low rate\u0000of needed hardware removal make this technique preferable\u0000for fixing Mayo Type II A olecranon fractures.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. Remplissage technique combined with Bankart repair\u0000have been proposed as a treatment option.</p><p><strong>Hypothesis: </strong>Early\u0000arthroscopic treatment for shoulder dislocation will result in better\u0000outcome and lower recurrence rate than nonoperative management.</p><p><strong>Methods: </strong>We retrospectively reviewed 60 cases from\u00002010 to 2015 treated by remplissage technique with Bankart\u0000repair or closed reduction for anterior shoulder dislocation. All\u0000surgeries and closed reductions were done by the same surgeon.\u0000Mean age of patients was 30 years, most of them males\u0000having experienced one or more recurrent dislocations; mean\u0000follow-up was 2 years. Patients with Hill-Sachs lesions < 40% on\u0000the articular surface and < 20% of bone defect in the glenoid\u0000cavity were included. Exclusion criteria were: glenohumeral\u0000arthritis or other inflammation, fracture around the shoulder joint,\u0000elderly patients with osteoporosis. All patients included in the\u0000study were followed up after 6, 12 and 24 months. Rowe score\u0000was used to assess the stability of the shoulders and goniometry\u0000to assess the range of motion of the glenohumeral joints.</p><p><strong>Results: </strong>The results confirm that the remplissage technique\u0000with Bankart repair takes the upper hand over the conservative\u0000management and does not produce any severe adverse effect\u0000on postoperative shoulder range of motion. A slight restriction\u0000(≈10º) observed in external rotation did not prevent 69% of patients\u0000from resuming their preinjury sports activities. At the last\u0000follow-up, 90% of patients had a stable shoulder. Conservative\u0000management was associated with high rate of recurrence limiting\u0000the daily activity of our patients and interfering with their\u0000return to sports activities. Except from the recurrence of glenohumeral\u0000instability, no patient had a complication following arthroscopic\u0000Hill-Sachs remplissage.</p><p><strong>Conclusion: </strong>Conservative\u0000management after anterior shoulder dislocation including immobilization\u0000in neutral position leads to significantly higher and unacceptable\u0000high failure rate compared with early arthroscopic\u0000remplissage with Bankart repair. The slight restriction in external\u0000rotation post remplissage does not significantly affect the quality\u0000of life and return to sports activities and further supports the\u0000use of this safe, relatively short procedure, in the management\u0000of glenohumeral instability with concurrent Hill-Sachs lesions.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais. This deformity is located in a pure transverse plane\u0000at Lisfranc’s joint. Isolated metatarsus adductus is corrected\u0000spontaneously for the majority of newborns. In rare uncorrected\u0000cases, it could result in Z-shaped foot with a functional\u0000hindfoot valgus to equilibrate the resistant metatarsus\u0000adductus. As well, in residual clubfoot, recurrent metatarsus\u0000adductus varus is observed, usually in children over three\u0000years.\u0000In flexible metatarsus adductus the treatment is conservative.\u0000The surgery is proposed in toddlers and after failure of\u0000conservative treatment. Procedures carried out on metatarsals\u0000gave good results on short term, but showed a high rate\u0000of recurrence and growth disturbance. Osteotomies proximal\u0000to the Lisfranc’s joint: calcaneo-cuboid fusion, anterior resection\u0000of calcaneus, and opening wedge osteotomy of medial\u0000cuneiform, gave permanent correction but they act only on\u0000one of the sides of deformity. Therefore, the theory of elongated\u0000lateral column associated with a shortened medial\u0000column is crucial in dealing with this deformity: combining\u0000opening wedge osteotomy of cuneiform with closing wedge\u0000osteotomy of cuboid described by Jawish et al. in children\u0000over 4 years allows – in all causes of metatarsus adductus\u0000stiffness – a lateral shifting of forefoot. Concerning the associated\u0000heel’s valgus, it is corrected in Z-shaped foot after the\u0000associated\u0000heel’s valgus, it is corrected in Z-shaped foot after the\u0000double osteotomy cuneiform/cuboid. Computed tomography revealed a complete\u0000bilateral facet fracture-dislocation at the C4C5 level, with a\u0000unilateral facet fracture-dislocation on the left side at the\u0000C3C4 level; there was also a sagittal fracture of the fifth cervical\u0000vertebra extending through the middle of its body with\u0000a second fracture through the posterior arch. The patient\u0000was taken urgently to the operating room and closed reduction\u0000maneuvers were performed under general anesthesia,\u0000followed by an anterior prevascular approach for C3 to C5\u0000fusion with two iliac crest grafts and a plate. Patient’s muscle\u0000strength was 3/5 on all four limbs on discharge and between\u00004/5 and 5/5 at one year. He stopped self-catheterization nine\u0000months after the accident. At two years follow-up, X-rays and\u0000CT scan showed a stable construct with satisfactory fusion.\u0000This is the first paper, in the literature, to describe double\u0000level contiguous cervical dislocation with a sagittal split fracture,\u0000and managed via an anterior only approach.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais.
