In this article we describe\u0000a new method of sternal fixation using nonabsorbable\u0000sutures which are placed in a transverse and crossed\u0000fashion anterior to the sternum. This method provides stable\u0000sternal fixation and spares the patient a second operation to\u0000remove the steel bar. The absence of metallic implants allows\u0000clearer view of the thoracic structures in future X-rays, CT\u0000scans and MRI, and is likely to be more acceptable to patients\u0000than the implantation of a metallic strut in their chest. From a\u0000geometrical point of view, the global tilt is the sum of the\u0000C7 vertical tilt and the pelvic tilt. The aim of this study is to\u0000evaluate the global tilt by analyzing its correlation with spinal\u0000malalignment.</p><p><strong>Methods: </strong>A cohort of patients who underwent\u0000a lumbar pedicle subtraction osteotomy (PSO) for major\u0000sagittal malalignment was realized. All patients had preoperative\u0000and postoperative full spine EOS radiographies to\u0000measure spinopelvic parameters. The lack of lordosis was\u0000calculated after prediction of theoretical lumbar lordosis.\u0000Correlation analysis between different spinopelvic parameters,\u0000including the global tilt, was performed for preoperative\u0000and postoperative values.</p><p><strong>Results: </strong>Thirty-one consecutive\u0000patients were included. All parameters were correlated with\u0000spinal malalignment but the global tilt was the most correlated\u0000parameter in preoperative (r = 0.71) and in postoperative\u0000(r = 0.78). When spinal and pelvic parameters were analyzed\u0000separately, 19% of patients presented mismatches\u0000between spine and pelvis.</p><p><strong>Conclusion: </strong>This study highlights\u0000the interest of a global parameter evaluating the spinal\u0000balance and the pelvic balance together. The global tilt\u0000appeared to be the most correlated parameter in this study\u0000with spinal malalignment and could be used for the interpretation\u0000of clinical series in spine surgery.</p>","PeriodicalId":430072,"journal":{"name":"Le Journal médical libanais.
