Methods: Thirty cases (mean age 9.87±3.92 years, 13 males and 17 females) with acute lymphoblastic leukemia receiving anthracycline therapy were enrolled in the study, along with 30 agematched controls. Their cardiac functions were evaluated echocardiographically with two dimensional, M-mode, PW Doppler and PW tissue Doppler methods. Results: Left ventricular cardiac functions were significantly reduced in the study group. The fractional shortening (FS) was 36.09±3.17 in the study group as opposed to 39.65±3.21 in controls (p=0.03). Diastolic functions (E/A ratio) weren’t significantly altered in the study group. Right ventricular (RV) flow E/A was 1.36±0.29 and 1.34±0.69, respectively (p>0.05). Left ventricular (LV) flow E/A was 1.53±0.31 and 1.58±0.35, respectively (p>0.05). Myocardial performance index (MPI) was calculated in the study subjects using both PW Doppler and PW tissue Doppler methods, and compared to controls. RV flow MPI was 0.39±0.02, and 0.33±0.03, respectively (p<0.01). LV flow MPI was 0.41±0.08 and 0.34±0.04, respectively (p<0.01). RV tissue MPI was 0.44±0.04, and 0.39±0.05, respectively (p<0.01). LV tissue MPI was 0.42±0.02 and 0.36±0.01, respectively (p<0.01). Conclusion: The results of our study show that MPI and systolic functional changes are better indicators of anthracycline cardiotoxicity than diastolic functions. We suggest that RV and LV tissue MPI be primarily used in the search for anthracycline cardiotoxicity.","PeriodicalId":428200,"journal":{"name":"Journal of Dr. Behcet Uz Children's Hospital","volume":"45 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126335210","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In this study, we aim to determine the etiological factors related to types of headaches in pediatric age groups and the classification on the ground of the diagnostic criteria of the International Classification of Headache Disorders, 2nd Edition (ICHD-II, 2004) in a pediatric population with complaints of headache. Methods: The clinical and laboratory findings of 80 patients with headache were evaluated retrospectively. Results: There were 36 (45%) males and 44 (55%) females in the study. The mean age of the patients was 10.38±2.57 years (range 5-15). Of the eighty patients 28 had migraine (n=28; 35%), 19 had sinusitis (23.8%), 21 had tension-type (26.2%) and 3 had primary stabbing headaches (3.7%). Migraine and tension-type headaches were more frequently seen in girls. In about half of the patients (n=36; 45%) a psychiatric pathology was detected. The most frequent accompanying psyhiatric finding was anxiety (30%). Conclusion: We conclude that following a sound anamnesis, physical and neurological examination of the children with recurrent headaches, neurophysiological and neuroradiological methods should be resorted. On the ground of the these findings headaches should be classified as primary or secondary headaches. Psychiatric evaluation regarding comorbid diseases should be recommended for the children with primary headaches.","PeriodicalId":428200,"journal":{"name":"Journal of Dr. Behcet Uz Children's Hospital","volume":"61 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115800873","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Splenectomy may be performed for the treatment of many hematologic diseases. The aim of this study is to discuss recorded data of thalassemia major patients who underwent splenectomy. Methods: The records of 782 patients with thalassemia major were reviewed retrospectively. Of these patients, 35 splenectomized patients with available data were included in the study. Recorded data of patients were evaluated as for age at diagnosis, gender, age at splenectomy, surgical indications, chelation therapy, annual transfusion requirements, and postoperative clinical response. Statistical analyses were performed with statistical package of SPSS version 13.0. Results: Twenty-two (63%) male and 13 (37%) female patients were included in the study. Mean age at diagnosis was 8±2 months and splenectomy was 8.5 years. The main indication for splenectomy was increased transfusion requirement (57.14%). The annual transfusion requirement reduced from preoperative 235±30 mL/kg to 115±15 mL/kg postoperatively. Mean serum ferritin levels were 1745±761 ng/mL, and 1985±570 ng/mL before, and after splenectomy. None of the patients developed any complications such as sepsis or coagulation disorder. Conclusion: These results suggest that splenectomy is beneficial to improve the quality of the lives of patients and significantly reducing blood consumption. Inadequate iron chelation therapy may be associated with progression to splenectomy.","PeriodicalId":428200,"journal":{"name":"Journal of Dr. Behcet Uz Children's Hospital","volume":"21 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-12-05","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"121038255","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Oral health problems are more prevalent in socioeconomically disadvantaged groups of the population. The aim of this study is to evaluate oral health status regarding prevalence of dental carries and DMF-T values and the associated socioeconomic factors among 6. and 7. grade students of Kemalpaşa district in Izmir City. Methods: This cross-sectional study has been performed among 6. and 7. grade students. A total of 832 students were evaluated (624 in urban primary schools, 208 in rural primary schools). Dental carries and DMF-T score were the dependent variables. Age, sex, school type, wealth status, mother’s and father’s education, health insurance, father’s occupation were the independent variables. The univariate analysis were performed with chi-square test, and the multivariate analysis using a logistic regression model. Results: More than half (60.3%) of the students had at least one dental caries in their permanent teeth.The mean DMF-T score was 1.55 in the study population. The risk of dental carries in the permanent teeth increased 2-fold (1.10-3.66) among students of the rural schools, 1.64 times (1.062.54) in low socioeconomic status, while this risk was 1.95 times (1.10-3.48) higher among students of poorly educated mothers. The risk of having a DMF-T score above zero increased 2.06 times (1.12-3.81) for students who had educated mothers. Conclusion: Oral health status of the students shows the need for treatment and prevention is an important issue. Lower socioeconomic status of the family, and educational status of the mother, unemployed father and attending an urban primary school are socioeconomic variables associated with oral health problems. Oral health services which should deal with preventive and therapeutic measures must aim to cover all children and health care services in consideration of risk groups should be given at schools and primary health care centers.","PeriodicalId":428200,"journal":{"name":"Journal of Dr. Behcet Uz Children's Hospital","volume":"18 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2013-07-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"127701018","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
