Its treatment will depend on the physical demands of the patient, triggering event and injury mechanism of PD, number of dislocation episodes, patellofemoral joint morphology, and concomitant injuries.\u0000After primary PD, despite the risk of recurrence being 33-77%, first treatment option is mostly conservative, except if an osteochondral fragment needs to be refixed or removed. This practice has been questioned lately by the Patella Instability Severity Score that determines the risk for recurrent PD in function of age, bilaterality, and anatomical risk factors. Risk behavior in relation to sports activity seems to be an additional risk factor. \u0000The treatment of recurrent PD is surgical with only low recurrent rate (2-4%). Medial patellofemoral ligament (MPFL) reconstruction is the most widely used technique. The ideal candidates are subjects with painless intervals between PD and without major trochlear dysplasia (TD) or patella alta. However, postoperative pain and loss of flexion might be observed if the graft is overtensioned or its fixation point malpositioned. Trochleoplasty is the only technique that aims at improving containment with the risk to increase peak forces due to incongruence. Due to the demanding technique its superiority over isolated MPFL reconstruction has been described only in grotesque TD and PD in higher knee flexion (>60°). Tibial tubercle osteotomy can be distalized in case of patella alta or medialized in case of lateralized tibial tubercle. Corresponding exercise recommendations can be found in national and international guidelines, which show a relatively solid evidence base. However, there are large knowledge gaps with respect to training recommendations for ambitious amateur sportswomen and elite athletes. Therefore, among both athletes and their supporting staff there is a large degree of uncertainty, which sports activity and to what extent it can be maintained safely without risk to the mother and her unborn child. In addition to the consideration of contraindications, where sporting activities must be avoided, there are certain training-associated precautions (e.g. when training at high altitude or in hot conditions) that should be respected. A study by EY shows that more than 94% of female C-Suite executives today played competitive sport, and over 50% played at university level [1]. Furthermore, supporting girls and women in sport leads to stronger women, stronger communities, and significantly contributes to economic growth overall. {"title":"Helping athletes flourish using mindfulness and acceptance approaches – an introduction and mini review","authors":"","doi":"10.34045/sems/2021/18","DOIUrl":"https://doi.org/10.34045/sems/2021/18","url":null,"abstract":"Mindfulness and acceptance are core concepts of the third wave of cognitive and behavioral psychotherapies. Their aim is not to overpower thoughts and emotions by suppression and using control techniques but to change the relationships with them. This article focuses on mindfulness- and acceptance-based approaches in sports. It briefly outlines these approaches and concludes that they are characterized by promoting three processes, which are present-moment attention, metacognitive awareness, and acceptance. Subsequently, the paper describes one approach particularly suitable for sports in detail – Acceptance and Commitment Therapy – with a special focus on how athletes can promote psychological flexibility and thrive in their effort to perform. This paper further addresses the question of whether mindfulness- and acceptance-based approaches are effective in sports by summarizing research from the past five years in the form of a mini-review. Findings from 19 reviewed studies, including 15 controlled or randomized controlled studies, suggest that different athlete populations can benefit from mindfulness- and acceptance-based approaches in terms of athletic performance and well-being. Positive effects on performance and well-being could be mediated by other psychological processes, e.g., enhanced emotion regulation. {"title":"Psychological health in elite sports during COVID-19 \u0000Psychische Gesundheit im Leistungssport in Zeiten von COVID-19","authors":"","doi":"10.34045/sems/2020/8","DOIUrl":"https://doi.org/10.34045/sems/2020/8","url":null,"abstract":"Die Änderungen des öffentlichen Lebens, Isolation, Quarantäne und damit verbundene weitere Einschränkungen der gewohnten Routine sowie Ängste und Sorgen sind nur einige Beispiele für die psychischen Belastungen durch die COVID-19-Pandemie (1). Nicht nur die Normalbevölkerung, sondern auch bzw. vor allem Leistungssportler sind diesen Belastungen ausgesetzt, und Leistungssport ist in Zeiten von COVID-19 nur noch stark eingeschränkt denkbar. Von einem Tag auf den anderen änderten sich für Sportler über Jahre und Jahrzehnte etablierte Tagesstrukturen und Gewohnheiten. Training ist häufig nur noch in Isolation und Kleingruppen (z.B. mit maximal 5 Personen) fernab des gewohnten Umfeldes umsetzbar. Diese für Leistungssportler spezifischen Belastungen für die psychische Gesundheit durch die COVID-19-Pandemie gilt es ebenso zu beachten wie mögliche Auswirkungen auf die körperliche Gesundheit (2). Geschlossene Trainingseinrichtungen, abgesagte Trainingslager, Unsicherheiten und Unklarheiten bezüglich der Fortführung der Saison, abgesagte Wettkämpfe und verschobenen Grossanlässe wie die Olympischen Spiele und die Fussball-Europameisterschaft sind weitere Beispiele (2,3). Einige Sportler könnten später einmal glauben, dass die Verschiebung der Olympischen Spiele ihnen eine Teilnahme an einem einmaligen Lebensereignis oder gar eine sicher geglaubte Medaille verwehrt hat. Andere, die aktuell noch von einer schweren Verletzung genesen, oder die im nächsten Jahr ihren Leistungszenit erreichen, kommt die Verschiebung entgegen, und sie profitieren. Grossanlässe sind häufig Zeitpunkte für Transitionen aus dem Sport. Das Karriereende per se kann bereits eine Belastung für die psychische Gesundheit darstellen (4).
