{"title":"Reporte del primer caso de Síndrome Inflamatorio Multisistémico Pediátrico en el Hospital de Especialidades José Carrasco Arteaga en el año 2020","authors":"Jorge Andrés Torres Jerves, Domenica Camila Carpio Terreros","doi":"10.14410/2022.14.2.cc.19","DOIUrl":"https://doi.org/10.14410/2022.14.2.cc.19","url":null,"abstract":"BACKGROUND: PIMS/MIS-C involves an innate immune response of the host against a trigger, in this case a virus, which causes hyperinflammation induced by a cytokine storm; this immune response is responsible for the systemic clinical manifestations and cardiac complications. The syndrome has been diagnosed usually between 2 and 6 weeks after infection by SARS-CoV-2; however, many patients have negative PCR (polymerase chain reaction) tests for the virus and positive immunoglobulin tests, suggesting that the syndrome appears in the early stages of convalescence from COVID-19. CASE REPORT: A 15-year-old female patient, previously healthy, with a history of close contact with a relative with COVID-19 confirmed by PCR 15 days before the onset of symptoms. He presented a 5-day history of fever, vomiting, and diarrheal stools; 48 hours later she presented with generalized asthenia, hyporexia and epistaxis. We evidenced elevation of immunoglobin G against SARS-COV-2 and negative PCR. In complementary tests, we evidenced elevation of inflammatory markers (CRP, procalcitonin, ferritin, D-dimer) and BNP; the echocardiogram revealed a condensation process in the pericardium, as well as coronary ectasia. Evolution: After confirming the diagnosis of PIMS/MIS-C, we started treatment with immunoglobulin, prednisone, and acetylsalicylic acid. On the fourth day of hospitalization, she presented diaphoresis and moderate pain in the epigastric region that did not subside; Abdominal CT revealed signs of mesenteric adenitis, which reconfirmed the proposed diagnosis. After completing 72 hours of treatment, and being evaluated again, she was discharged. We prescribed continuance of prednisone treatment and acetylsalicylic acid and 500 milligrams of amoxicillin orally every 8 hours. In addition, we recommended follow-up by the cardiology service one week after hospital discharge. The patient remains without complications after this episode. CONCLUSION: The diagnostic suspicion of PIMS/MIS-C must be present in all patients with characteristic symptoms, under the age of 20 years, with a history of contact with a positive COVID-19 patient and who in most cases will have a positive PCR test or positive serology. Given the multiple and variable clinical manifestations, it is important to take into account the differential diagnoses, especially Kawasaki Disease, whose clinical manifestations this syndrome mimics. SLE is one of the most well-known and studied autoimmune pathologies, its overlap with other autoimmune diseases is known, being one of the least frequent its association with Systemic Sclerosis (SS). Here we present a case of SLE-SS overlap, which was diagnosed many years after the onset of symptoms. CASE REPORT: A 45-year-old female patient, diagnosed 14 years ago with Sjogren's Syndrome and Raynaud's Syndrome, without current treatment. She came to receive medical attention due to asthenia, a feeling of shortness of breath, erythema in the toes, joint pain in the distal interphalangeal joints of both hands, and lesions in the fingernails. After assessing the clinical features and with the help of complementary tests, mixed connective tissue syndrome was ruled out. The patient met the diagnostic criteria for SLE (2019 ACR/EULAR criteria) and SS (2013 ACR/EULAR criteria). EVOLUTION: After confirming the diagnosis of SLE-ES Overlap Syndrome, specific treatment with immunosuppressive drugs was started, to which the patient responded favorably, with progressive improvement of her symptoms, as well as improvement in clinical laboratory parameters. CONCLUSION: Overlapping syndromes, such as ES-SLE, are rarely poorly diseases; the diagnostic suspicion should be based on symptoms compatible with both diseases simultaneously and should be confirmed by antibody determination and supported with other complementary tests. Although antiretroviral therapy has improved the survival of people with HIV, the adverse effects caused directly or indirectly by antiretroviral therapy have caused poor adherence to it, leading to therapeutic failure. The aim of this study is to determine the frequency of laboratory parameters alterations in HIV patients under treatment with antiretroviral drugs. METHODS: A descriptive, observational and cross-sectional study was carried out. The sample consisted of 233 patients diagnosed with HIV under antiretroviral treatment, treated at the Infectious Diseases Unit of Hospital de Especialidades José Carrasco Arteaga. The analyzed variables were: age, sex, treatment regimen; alterations were sought in the following variables: lymphocyte count, hemoglobin, blood triglycerides, blood glucose, neutrophil count, transaminases, bilirubin, creatinine, urea and lactic acid. RESULTS: The majority of patients were male (86.70%), mainly between 30 and 39 years of age (33.91%). The most used scheme was \"Tenofovir + Emtricitabine + Efavirenz\" (57.94%). The laboratory analytical alteration presented most frequently in all treatment schemes, except for Tenofovir+ Emtricitabine + Lopinavir+ Ritonavir, was hypertriglyceridemia (25.75%). 24.46% of the patients did not present any analytical alteration. CONCLUSION: The majority of HIV patients during 2018-2019, in HEJCA, were male, between 30 and 39 years of age. The most widely used treatment scheme was \"Tenofovir + Emtricitabine + Efavirenz\". Hypertriglyceridemia was the most frequent laboratory abnormality, followed by hyperglycemia and anemia. The aim of this study was to evaluate the knowledge, attitudes and practices about the use of physical restraints of caregivers in geriatric centers in the city of Cuenca, during 2018. METHODS: Descriptive, observational, cross-sectional study in which a survey was applied to 60 caregivers of older adults from 6 geriatric centers in the city of Cuenca-Ecuador. The questionnaire we used, prepared by Suen KPL, evaluated the knowledge, attitudes and practices of caregivers regarding the use of physical restraints. Data was tabulated with SPSS version 22.0 software, percentages and frequencies were calculated; we used Fisher`s exact test searching for statistical association between the relevant variables. RESULTS: 75% of the caregivers in the study population were in a range of 20 to 39 years of age. Of all the participants, 90% were nursing assistants. Regarding knowledge, 66.7% of the participants had intermediate knowledge; only 10% had high knowledge. 83.3% of the participants had negative attitudes regarding physical restraints. Most of the population (68.3%) had very satisfactory practices. CONCLUSION: The caregivers of older adults in the geriatric centers studied in the city of Cuenca are mostly nursing assistants. There is a knowledge deficit in 23.3% of all caregivers. Most of the caregivers presented negative attitudes regarding the use of physical restraints. In relation to the practices, in general, we found a very satisfactory level. In Ecuador, the information about the epidemiological characterization of acromegaly is limited. The aim of this study was to determine the characteristics of the patients with acromegaly treated at Hospital de Especialidades José Carrasco Arteaga from January 1st, 2014 to December 31st, 2019. METHODS: This is an observational, descriptive, cross-sectional study. Data was collected from 88 electronic medical records of patients with a diagnosis of acromegaly treated during the aforementioned period. Statistical analysis was performed with PSPP 1.4.1 software. The results of qualitative variables are displayed in tables with numerical and percentage frequencies and for quantitative variables we obtained the mean and standard deviations. RESULTS: The male sex represented 55.7% of the population; the mean age at diagnosis was 43.27 years. Pituitary adenoma was registered in 83% of the cases. The accompanying symptoms were headache, visual field defects, and arthralgia in 18.2%, 27.3%, and 29.5% of the cases, respectively. 48.9% of cases received surgical treatment. The frequency of hypopituitarism was 38.6%. The frequency of comorbidities was: hypercholesterolemia (36.4%), arterial hypertension (29.5%), hypertriglyceridemia (22.7%), type 2 diabetes mellitus (22.7%) and ventricular hypertrophy (11.4%). CONCLUSION: The average age of patients with acromegaly was 52.60 years, the average age of diagnosis was 43.27 years, and it was more frequent in males. In 83% of the patients it was associated with the presence of pituitary adenoma. This condition has been linked to the use of corticosteroids, drugs that are currently frequently used to treat various pathologies. The therapeutic use of steroids can trigger an increase intraocular pressure, depending on the drug, the time and the route of administration, and leads to glaucoma induced by corticosteroids. Steroid induced glaucoma is considered a public health problem, and it is a preventable condition. There are several aspects that are still little known about this pathology, such as its general incidence and its exact pathophysiology; likewise, there are currently new diagnostic methods and innovative treatments. The World Health Organization points out that 18% of newborns with congenital heart disease will not be diagnosed until adulthood. The aim of this study was to determine the incidence of congenital heart disease in adult population at Hospital de Especialidades José Carrasco Arteaga in the period 2015-March 2020. METHODS: This is a descriptive, observational, cross-sectional study. The universe are all the patients from 18 years of age or older treated in the outpatient clinic and hospitalization. To consider the incidence, we filtered the patients with an ICD- 10 diagnosis of the congenital heart diseases considered, it was verified that the initial diagnosis was made in the HEJCA, in adulthood, during the established period of the study and that the diagnosis has been confirmed by imaging studies. In addition to the incidence, other clinical variables were studied, such as manifestations, family history of congenital heart disease, other diagnosed congenital pathologies, mortality, need for surgery, hospitalization, and days of hospitalization. RESULTS: The incidence of congenital heart disease in adults was 227 in 219 960 patients treated at the HEJCA; 58.6% of the diagnosed patients were female. Most of the patients were diagnosed in the age range of 46 to 64 years. 84.1% were acyanotic heart disease. Atrial septal defect represented 41.8% of all diagnosed heart diseases. The most common manifestations were murmur (36.8%) and dyspnea (27.8%). Only 6.6% of the population did not present clinical manifestations. 29.1% of the patients underwent surgery. Mortality in these patients was 12.3%. CONCLUSION: The incidence of congenital heart disease diagnosed in adults in HEJCA from 2015 to March 2020 was 0.1%. There was a higher incidence in females. The mean age of diagnosis was 48.5 years. The most common heart disease was atrial septal defect, followed by congenital aortic stenosis. The most frequent combination of clinical manifestations was dyspnea and murmur. The aim of this study was to determine the perception of the undergraduate students at Universidad del Azuay to opt for a vaccine against SARS-CoV2. METHODS: Descriptive, observational, cross-sectional study. An electronic survey was applied, between the months of January and February 2021, to a sample of 1 497 students enrolled in the 2020-2021 school year who agreed to participate. The survey contains 11 questions on sociodemographic data and characteristics of the participants and 6 questions on the perception of different aspects of the vaccine. The Chi-Square test was applied between the variables, with a confidence level of 95%. RESULTS: The acceptance of the participants for a free vaccine against SARS-CoV2 was 76.4%, this percentage of acceptance varies to 58.1% if the vaccine had a cost of 50 US dollars. Acceptance of a physician-recommended immunization process was 78%. 75.3% of the students believed that it is important to vaccinate people with comorbidities. 58% of students thought that the vaccines would be effective and 50.6% of those surveyed said that the vaccines would meet quality standards. CONCLUSION: Most of the participants would accept a vaccine against SARS-CoV2; acceptance increases if there is a recommendation from medical personnel and decreases if the vaccine has a cost. It is a rare type of skin cancer that is associated with a poor prognosis and a high rate of recurrence. Despite significant advances, its pathogenesis is still poorly understood. Early diagnosis and proper management are essential to improve the survival rate. Although it affects both genders, it has male predominance. It is presented as hard, bright, reddish, and asymptomatic tumors with a certain predilection for head, neck and limbs. The treatment is surgical and in some cases, it requires adjuvant radiation therapy. An early diagnosis and appropriate handling are essential to improve the survival rate. METHODS: We conducted an observational, descriptive study ⸻case series⸻, which included 10 patients diagnosed with MCC, from the dermatology service of the Skin Cancer Center of Hospital Italiano de Buenos Aires, from January 2004 to August 2018. RESULTS: Over the course of 15 years, ten patients, 5 men and 5 women, were clinically and histopathologicallly diagnosed with Merkel Cell Carcinoma, with mean age of presentation of 83 years (range: 74-95 years). There was a high frequency (70%) of immunological alterations due to autoimmune diseases, renal-transplanted patients, and chronic lymphocytic leukemia. The therapeutic option was conventional surgery, with a margin of 3 cm, in six patients and Mohs micrographic surgery in four patients. Three patients developed local recurrence, who received adjuvant radiotherapy. CONCLUSION: The mean age of presentation was 83 years of age; there was no predominance according to sex. Most cases were localized in head and neck. Most of the patients had a concomitant immunological alteration. Most cases were resolved by conventional surgery. Despite the continuous improvement of surgical techniques and antibiotic prophylaxis, this complication represents 38% of all nosocomial infections; it increases hospital stay, increases morbidity, mortality, and raises costs per patient. The aim of this study is to determine the prevalence of surgical site infection in herniorrhaphy and hernioplasty, and its associated factors, in patients treated at “Hospital de Especialidades José Carrasco Arteaga”, in Cuenca, from 2016 to 2018. METHODS: Analytical, cross-sectional study, based on 203 patients who underwent surgical hernia repair, in the period from January 2016 to December 2018. Analysis was performed with Pearson's Chi-square non-parametric statistic. To determine the association of risk factors, the Odds Ratio measure was used, with a 95% CI and values> 1; statistical significance was established with the p value <0.05. RESULTS: The prevalence of surgical site infection was 8.9%, and it was found to be associated with the type of hernia (OR: 3.57 p= 0.046) and use of drains (OR: 3.2 p+ 0.026). No association could be determined with the factors: smoking, diabetes, hypertension, immunosuppression, ASA index, use of prosthetic material, surgical approach or surgical emergency.
