In addition, these pathogens pose a difficult challenge in the prevention of its cross–transmission and contamination. Clostridium difficile, Methicillin Resistant Staphylococcus aureus (mRSA) and Pseudomonas aeruginosa have become important problems in the medical field in the past decade because of their resistance to commonly used disinfectants. It was initially described in 1958 by Jaffe, who separated it as a distinct entity from other intraosseous fibrous tumors. This tumor constitutes <1% of bone tumors and about 0.3% of all benign osseous tumors and it usually involves the tibia, scapula and femur [1]. The mandible represents the fourth most commonly affected site. It has a predilection to affect the posterior part of the mandible, most often in the ramusangle region. Clinical symptomatology is not specific. The imagery makes it possible to appreciate the extensive potential of the tumor .The histological appearance of the desmoplastic fibroma is identical to that of the extra-osseous desmoid, although the fibroma is infiltrative; there are no mitoses or nuclear atypia. Enormous datasets are now available describing patient behaviors, signs and symptoms of diseases, human– derived information, insurance claims data and pharmacy refill records, providers notes, and imaging. Regular database management systems we have seen in the past are not able to handle these huge datasets. To manage the substantial amounts of data collected about any one given patient, providers have begun to utilize Big Data Analytics as the primary method for handling this copious amount of data.1 Table 1 shows the characteristics of big data in the healthcare area. Data is collected not only from the EMR, but also frequently the patient uses sensors, social media, and smartphones to generate data. For the healthcare provider, it can be overwhelming to mine the data for any specific piece of information that may be critical to the patient’s care. Big Data however, can handle voluminous amounts of heterogenous data; structured, semi–structured, and unstructured. Much of healthcare data is now either unstructured such as medical notes, nursing notes, or other provider–written information, or semi– structured data. Metformin is still the preferred first-line agent for most TI diabetes patients; though most patients require multi-agent therapies to maintain stable blood glucose levels in the long term. Traditional Chinese Medicine (TCM) has recently become attractive to western healthcare systems because of its low costs as well as its comprehensive, holistic approach to preventing and managing diabetes symptoms. Their future depends on their offspring. Family plays an important role in the experiences of the infertile couple. Children are not only sources of economic support, especially for the woman, but inheritor of all the possession, titles, propagation of family lineage and what most people have spent their entire lives to achieve. It is acknowledged that in most resource poor countries, notwithstanding overpopulation, involuntary childlessness is a significant social and economic burden that deserves attention. Certain Africa adages, such as “omo l’asho” (one’s child is one’s cloth, indicating a woman without a child is naked), “omo l’ere aiye” (one’s child is one’s life’s sole profit), and “eni omo sin lo bi’mo” (one who is buried by her child or children is one who actually gave birth), reflect value of fertility and why involuntary infertility is an anathema in most African societies. Furthermore, some African communities never refer to a married woman by her name but by the name of her first child such as “Iya Joshua” meaning Joshua’s mother or “Iya Ibeji” meaning the mother of twins. Specifically, children often refer to their mother as “Maami” or “Mami” meaning my mother. In Yoruba ethnic group, it is often said that “Iya ni wura”, that is mother or motherhood is gold. We present the case of a 62 years old patient, suffering from chronic HBV and HDV hepatitis, which was found, in 2011 with a 7 cm HCC located in the sixth segment of the liver and resected. Within less than a year, tumor relapse was detected (multiple nodules in the remaining sixth segment). Systemic chemotherapy (Sorafenib) started being administered and two TACE procedures were performed at one year and seven months difference. Despite that, the CT and MRI showed the evolution of the disease. The therapy was stopped and the patient was listed for liver transplantation and the procedure was performed. The thirteen months post transplantation assessment showed no sign of tumor relapse. It is a rare condition that causes significant morbidity and mortality and often occurs in the elderly with significant comorbidities . Individual diagnostic and treatment strategies are required for optimal management and outcome. We describe a case of Bouveret’s Syndrome in an elderly patient who was managed successfully through surgery. Lupescu","doi":"10.21614/JTMR-22-2-119","DOIUrl":"https://doi.org/10.21614/JTMR-22-2-119","url":null,"abstract":"Purpose: The aim of this study is to describe the role of the percutaneous biliary drainage (PTBD) in the managament of patients with hepatic cholangiocarcinoma (CC) and also to assess outcomes and complications after PTBD procedures. Methods and Materials: 225 consecutive patients with biliary obstruction managed by PTBD were included. All cases were performed by four consultant interventional radiologist between January 2014 and March 2016. Demografic data, patients history, the radiological data base, laboratory test from and following intervention, procedural records, clinical and imaging follow-up were reviewed. Pre procedure imaging with abdominal ultrasound and computed tomography was performed in all patients. Complications were grouped into immediate (occurring <24 hours) early ( 24 hours to 30 days) and late (>30 days). 30-day mortality was calculated. Results: In total there were 111 male (49,3%) and 114 female (51,7 %). The mean age was 61,5 years and ranged from 88-27 years. The approach for PTBD was left sided in 21 cases (9,3%), and right sided in 204 cases (90,7%). There were 6 (<3%) technical failures giving a technical success rate of (97,3 %). These included 2 cases of bleeding and 4 cases were we were unable to cross the obstruction. It is characterized by raised intracranial pressure (ICP) in the absence of brain parenchymal lesion, vascular malformations, hydrocephalus, or central nervous system (CNS) infection. The diagnosis is usually confirmed by a high opening pressure of cerebrospinal fluid (CSF) with exclusion of secondary causes of intracranial hypertension. Objectives: The purpose of this review is to summarize relevant articles on the diagnostic tools used in evaluation and management of pediatric IIH. A summary of demographics, clinical presentation, diagnosis, neuroimaging, as well as existing evidence of treatment strategies is presented. Method: We conducted a bibliographic search in PubMed using the terms idiopathic intracranial hypertension (IIH), Pseudotumor Cerebri syndrome (PTCS). The review of the literature revealed a lack of published consistent data on the diagnosis criteria and management of idiopathic intracranial hypertension in children. Conclusion: This article provides a review of IIH in children and revised diagnostic criteria based on recent evidence and published opinion. Relative to the adult population, the demographic features and clinical presentation of IIH as well as the diagnosis and treatment guidelines for children are quite different.
