During the pretransplant cardiac workup, the diagnosis of cirrhotic cardiomyopathy would be easily missed without the aid of some supportive criteria because most patients have normal rest echocardiography. The period of highest hemodynamic instability during liver transplantation occurs after revascularization of the liver graft and is called postreperfusion syndrome. The aim of this study was to investigate if patients at risk for cirrhotic cardiomyopathy have a higher risk for postreperfusion syndrome during liver transplantation. The secondary purpose of the study was to assess if the supportive criteria used for cirrhotic cardiomyopathy evaluation correlate with the severity of the liver disease. Material and Methods: The study group included 52 patients undergoing orthotopic liver transplantation for cirrhosis. The variables recorded were: demographic data, MELD and MELD Na scores, the length of the QT interval corrected for heart rate, plasma levels of brain natriuretic peptide (BNP), cold and warm ischemia times, vasopressor use, transfusional requirements and the occurrence of postreperfusion syndrome during transplantation. Results: The patients were divided into 2 subgroups based on the occurrence of the postreperfusion syndrome. There was no statistically significant difference in BNP levels, length of QTc interval, MELD or MELD Na scores between the subgroups. BNP levels were correlated with QTc interval length (p=0.005), MELD and MELD Na scores (p=0.025 and p=0.001). The correlation between QTc interval length and the severity scores was not statistically significant. Conclusions: Our study showed that postreperfusion syndrome was not correlated with the severity of the liver disease or with the presence of risk factors indicating cirrhotic cardiomyopathy. Herlea","doi":"10.21614/JTMR-21-1-76","DOIUrl":"https://doi.org/10.21614/JTMR-21-1-76","url":null,"abstract":"The retroperitoneum can host a wide spectrum of pathologies, including a variety of rare benign tumors and malignant neoplasms that can be either primary or metastatic lesions, whose correct diagnosis can be a challenge for the medical team and whose approach is often complex. We present the case of a patient diagnosed in June 2011, as a result of non-specific abdominal symptoms, with a small retroperitoneal mass with cystic appearance, whose affiliation could not be established. The evolution of this tumor was stable during the 3.5 years of clinical, biological and imaging follow-up, until January 2015 when the patient experienced neurological symptoms of lumbar nerve root compression which could have been associated with the increase in size of the retroperitoneal tumor. The therapeutic management was surgical tumor resection, and histopathology was suggestive for a pseudo-cyst, whose etiology remained unspecified. The peculiarity of the case is represented by the long silent evolution of a retroperitoneal mass, in which the association of neurological symptoms have posed problems of differential diagnosis, and in which the tumors etiology and sudden growth in size haven’t been elucidated by the histopathological examination, where no cellular structures able to guide the diagnosis were detected. Given the ever-increasing number of antibiotic options available, as well as the growing problem of antibiotic resistance, there is a need for research on this important topic. Methods: We retrospectively reviewed the clinical data of 45 patients receiving laparoscopic right hemicolectomy for colon cancer to investigate the occurrence of perioperative infections following laparoscopic surgery. This study included 45 cases receiving laparoscopic right hemicolectomy from January 2013 to December 2014. A 48-hour treatment of antibiotic prophylaxis was used. results: The incidence of incisional surgical site infection (SSI), organ/space SSI, and remote infection was retrospectively investigated. Incisional SSI occurred in 1 (2.3%) patient. Organ/space SSI occurred in 1 (2.3%) patient. No remote infection occurred. conclusions: The incidence of incisional SSI and organ/space SSI was low using a 48-hour treatment of antibiotic prophylaxis after laparoscopic right hemicolectomy for colon cancer. Popescu","doi":"10.21614/JTMR-21-1-70","DOIUrl":"https://doi.org/10.21614/JTMR-21-1-70","url":null,"abstract":"The objective of this study was to determine whether the mitogen-activated protein kinase kinase kinase kinase 4 (MAP4K4) expression can differentiate pancreatic ductal adenocarcinoma (PDA) from chronic pancreatitis (CP) and to analyze its biological and clinical significance in PDA. We have previously found by gene array analysis that MAP4K4 is overexpressed in PDA tissues. Here, we measured MAP4K4 gene and protein expression in 58 PDA and 9 CP surgical specimens using RT-PCR and immunohistochemistry, and analyzed their association with clinical outcomes. MAP4K4 mRNA expression was significantly higher in PDA versus CP tissues (Mann Whitney test, p<0.003), and correlated with TNM stage (Kruskal-Wallis test, p£0.026). MAP4K4 protein expression was significantly correlated with tumor grade (Spearman’s Rho = 0.449, p£0.01), and tended to associate with shorter overall survival (p£0.076, log-rank test). In conclusion, our data indicate that the level of MAP4K4 expression directly correlates with PDA progression. A detailed occupational history, a comprehensive workplace assessment and a proof of the relation between asthma attack and the presence of a risk factor in the workplace environment are the key factors for diagnosis. Early diagnosis of occupational asthma has a major positive influence on improving both evolution and prognosis. However, due to the reluctance of the patients to discuss health issues with coworkers, managers, occupational and family doctors, and to the difficulties in demonstrating that an occupational risk factor is the trigger of the asthma attack, the diagnosis is often delayed. We are reporting for the first time in Romania an occupational asthma in a bank desk officer that has been diagnosed after several years of work related symptoms by using the peakflowmetry measurement, a recently accepted demonstration of the occupational etiology of asthma. A proper staging of the disease allows the selection of the appropriate treatment, including the surgical approach. The present review aims to discuss and illustrate the role of computed tomography (CT) and magnetic resonance (MRI) in the differential diagnosis of the retroperitoneal or mediastinal masses, prior discovered by ultrasound or X-ray examination. Furthermore, it is discussed the role of CT and MRI for staging the patients with a histological proof of neuroblastoma. CT scan is very useful to detect and characterize the thoracic and abdominal tumors and organ metastases, with a special quality in detecting structural microcalcifications. MRI is sensitive for cranio-spinal primary tumors, marrow-bone metastases and mediastinal or pelvic primary or secondary tumors. The objective of our paper is to review the published articles to confirm our belief that rosacea is associated to or can predispose to various diseases, mainly inflammatory diseases. Material and methods: Review all the papers published in the last two decades in PubMed, Google Academic and Medscape databases, linking rosacea and various disorders of the body systems. Results: We found that rosacea is associated to several illnesses of major systems (gastrointestinal, cardiovascular, respiratory) and that there are also links between rosacea and immune system disorders and psychiatric diseases. Rosacea clinical course can be influenced by various drugs prescribed for the previous mentioned diseases. Conclusions: Rosacea should no longer be considered a localized cutaneous condition but rather a complex pathology with systemic reverberation. Methods: The records of 184 patients who had undergone surgery for complicated liver hydatid cyst in our institution during 2005 and 2014 were reviewed retrospectively. Results: Among all complications, the most common were intrabiliary rupture (140 patients) and suppuration of the cysts (27 patients). Eleven cases had a combination of two complications. Other complications were rupture in the thorax (4 cases), rupture in the peritoneum (7 patients) and vascular erosions (6 patients). Surgery was based on the nature of complications, number and sizes of hydatid cysts and patients general condition. The surgical treatment of choice was partial (peri) cystectomies and drainage of the residual cavity with suture of the biliary fistula. In cases with complications such as suppuration, remaining bile fistulas, large cyst cavities or extrahepatic location, external drainage of the common bile duct (CBD)was mandatory. Post-operative complications occurred in 45 patients (24.4 %). Postoperative mortality was 1 % (two patients). Conclusion: Appropriate investigation and well planned surgical techniques may improve the outcome of complicated liver echinococcosis. These data suggest that cyst diameter is an independent factor that is associated with a high risk of biliary-cyst communication in clinically asymptomatic patients.
