Acute cholecystitis is the second most common cause of complicated intra-abdominal infection. \u0000Aim. Analysis of the microflora in abscess due to perforation of the gallbladder and liver abscess, determination of its resistance to antibacterial drugs. \u0000Materials and methods. The microflora of 35 patients with perforated cholecystitis and liver abscess was studied. Material for microbiological research was taken during surgery. Sampling the abscess content , tissue samples of abscess walls, blood test for sterility. \u0000Results and discussion. Content of the subhepatic abscess of patients with perforated cholecystitis were examined, 23 strains of microorganisms were detected. Gram-positive coccal microflora was represented by the genera Streptococcus, Staphylococcus, Enterococcus and Micrococcus. Rod-shaped microorganisms in the vast majority of cases were represented by the family of facultative anaerobes Enterobacteriaceae (Escherichia coli, Klebsiella oxytoca). \u0000During studying the microflora of a liver abscess, 7 strains of bacteria and 1 strain of Candida fungi were found. \u0000Conclusions. The causative agents of acute biliary infection are polymicrobial flora. Enterococcus faecalis and Escherichia coli was dominated, in abscess due to perforation of the gallbladder. Enterococcus faecium and Escherichia coli was dominated, in liver absceses . \u0000The study of the sensitivity of selected strains of microorganisms to antimicrobial drugs revealed multiple resistance. Study of the effectiveness of the use of suture material with antioxidant properties for the prevention of suture failure in the small intestine (SmlI) under conditions of acute peritonitis (AP). \u0000Materials and methods. The research material was 70 white rats. AP was simulated in 63 animals. After 12 hours, a laparotomy was performed. In 56 animals, after washing the abdominal cavity, the SmlI was dissected and sutured. Kapron was used in the control (C), in experiment (E) - kapron impregnated with a 5 % solution of ethyl methylhydroxypyridine succinate. In 7 intact rats, 7 with AP models, and 1, 3, 5, and 7 days after SmlI suturing, sections of SmlI were taken for histological examination. The preparations were stained with hematoxylin-eosin, according to Mikel Calvo, the R/B ratio (R/BС) was determined. \u0000Results and their discussion. 12 hours after AP modeling, the R/BC indicators in the connective tissue fibers of the submucosal base of the TC increased significantly (p<0.05). 1 day after suturing, R/BC indicators increased significantly (p<0.05) in both C and E. After 3 days, R/BC indicators increased in C and significantly decreased in E (p<0.05) . In E, fibrin was found between the fibers of the ligatures. After 5 days, the indicators of R/BC in C and E significantly decreased (p<0.05), but in E they were significantly lower (p<0.01). In C, fibrin was found between the fibers of the ligatures, in E — granulation tissue. After 7 days, R/BC indicators in C and E decreased, but in E they were significantly lower (p<0.01). In C, unfilled areas remained in the granulation tissue between the fibers of the ligatures. \u0000Therefore, after suturing the wound of the small intestine in animals with AP models with kapron ligatures impregnated with a 5 % solution of ethyl methylhydroxypyridine succinate, according to histological studies, the reduction of the oxidation activity of the proteins of the intestinal tissues adjacent to the ligatures and the regeneration of the areas of the sutures are accelerated. \u0000Conclusions. 1. 12 hours after the simulation of acute peritonitis in rats, the R/B ratio increases statistically significantly (p<0.05) in the connective tissue fibers of the submucosal base of the small intestine. 2. 1 day after suturing the wound of the small intestine, regardless of the type of ligatures used, in animals with models of acute peritonitis, the indicators of the R/B ratio increase statistically significantly (р<0.05). 3. The results of minimally invasive puncture-drainage interventions in 56 patients with acute severe pancreatitis were analyzed. Indications for the implementation of puncture-drainage interventions were the presence of bulk fluid formations in the abdominal cavity and retroperitoneal space. These interventions were the first and only surgical intervention in 28 (50%) patients who recovered without any additional surgical procedures. According to the studied sources of literature of domestic and foreign authors, the mechanisms of matrix synthesis and the remodelling phase, the role of cytokines in the inflammatory response, as well as the participation of nitric oxide in the reaction with hydroxyl free radicals to form peroxide nitrate, a reactive free radical with the ability to cause tissue destruction and induce apoptosis, are revealed. The optimization of the method of examination of patients with suspected ischemia of the limbs when conducting a military medical commission during martial law. \u0000Materials and methods. 46 men (25-59 years old) with suspicion of chronic limb ischemia were examined. Examination of each patient was carried out according to both standard and optimized methods. The standard technique included a survey, physical examination, rheovasography, optimized - instead of the latter, it included photoplethysmography, perfusion index, tests with loading and cooling. \u0000Results and their discussion. Chilliness was noted by 14 people, intermittent gait – by 19, pain – by 38. In 7 examined, the pulse on the main arteries was preserved. According to the standard method, ischemia was diagnosed as the leading syndrome in 43 (93.5 %) patients, including compensated – in 18 (39.1 %), subcompensated – in 23 (50.0 %), decompensated – in 2 ( 4.3 %). Alternative pathology was diagnosed in 2 (4.3 %) patients. In 1 (2.2 %) patient, ischemia was assessed as a non-conducting syndrome. According to the optimized technique, ischemia was diagnosed as the leading syndrome in 36 (78.3 %)* patients, including compensated – in 19 (41.3 %), subcompensated – in 13 (28.3 %)*, decompensated – in 4 (8.7 %). Alternative pathology was diagnosed in 6 (12.8 %) patients. In 4 (8.7 %)* patients, ischemia was assessed as a non-conducting syndrome (* – differences are significant). Photoplethysmography, perfusion index, tests with loading and cooling made it possible to more objectively determine the degree of impaired blood flow and functions, which is the main criterion (according to Order No. 402) for determining the degree of suitability for military service. \u0000Conclusion. The relative technical simplicity of performing a transcutaneous puncture of the pancreatic cyst cavity under the control of modern medical imaging devices, as well as the simultaneous possibility of performing a minimally invasive medical and diagnostic manipulation regardless of the severity of the patient’s condition, attracts the attention of many researchers. \u0000The goal. The improvement of the results of treatment of cystic formations of the pancreas in patients with chronic pancreatitis with the help of minimally invasive interventions. \u0000Materials and methods. The retrospective results of treatment of 42 patients aged 18 to 72 years (average (36.7±8.4) years) with cystic formations of the pancreas in chronic pancreatitis for the period from 2015 to 2022, who were in the department of surgery of the liver and biliary tract of State University «Institute of General and Emergency Surgery named after V. T. Zaitsev National Academy of Sciences of Ukraine» are summarized. \u0000Results and their discussion. In the presence of small cystic lesions of the pancreas, not connected to the main pancreatic duct, the most appropriate method of intervention was their repeated therapeutic transcutaneous punctures under the control of ultrasonography. In patients with large cystic masses greater than 7 cm in diameter that are not connected to the main pancreatic duct, the operation of choice was long-term transcutaneous «pig tail» drainage under ultrasound guidance. Open surgical interventions (making of cystoenteroanastomoses) were performed as a second stage for patients in whom cystic formations connected with the main pancreatic duct. \u0000Conclusions. Treatment of patients with cystic lesions of the pancreas in chronic pancreatitis should be staged, and begin with transcutaneous fine-needle aspiration puncture of the cystic cavity. Peritoneal exudate in acute pancreatitis, which is found in 60–80 % of patients with acute destructive pancreatitis, is highly toxic and contributes to multiple organ failure. \u0000Materials and methods. The study is a retrospective analysis of the results of treatment in 69 patients with acute pancreatitis complicated by pancreatogenic peritonitis. Patients underwent clinical, laboratory and instrumental examination. For statistical processing, the normalized value of the Pearson coefficient (C’) was used. \u0000Research results and their discussion. Clinical and laboratory predictors of the development of acute destructive pancreatitis complicated by pancreatogenic peritonitis have been determined. They were given a score. \u0000Conclusions. Mine-explosive injury is a gunshot combined multiple injury that occurs in a person as a result of the impact of the impact factors of the explosion of engineering, mine ammunition and is accompanied by deep and widespread tissue damage and contusion. The purpose of the work is to analyze the methods of surgical treatment of patients with mine-explosive injuries in the conditions of martial law. Materials and methods. The results of surgical treatment of 64 patients with mine-explosive injuries were analyzed, 27 (42.2 %) were women, 37 (57.8 %) were men, who were being treated at Dergachiv Central Hospital. All victims underwent surgical interventions depending on the location of the injury, its nature, the severity of the patient, the development of traumatic or hemorrhagic shock. Results and their discussion. Given that the most frequent type of injuries were wounds of various localization, the most common type of surgical intervention was surgical treatment of wounds. The scope of the intervention depended on the depth and size of the wound, as well as the nature of tissue damage. Surgical manipulations on the chest were performed to eliminate pneumothorax (pneumohydrothorax), pleural puncture was performed or pleural drainage was established. If the Rouvilois-Gregoir test was positive, a thoracotomy was performed followed by suturing of the lung wound. Invasive diagnostic methods, laparocentesis and laparoscopy were performed for suspicion of damage to the organs of the abdominal cavity, with the help of which damage to the organs of the abdominal cavity was excluded. The use of laparocentesis in such situations, although its diagnostic value is lower than that of video laparoscopy, allows to determine the pathological content in the abdominal cavity, which is sufficient to detect life-threatening consequences of abdominal injuries, for example, such as intra-abdominal bleeding. Conclusions. Victims with explosive lesions require a large number of surgical interventions in various anatomical areas. The determination of the impact of the developed complex treatment of patients with peritonitis on the functional state of effector cells of antimicrobial resistance during the course of the disease. \u0000Materials and methods. The study included patients with toxic and terminal stages of peritonitis (124 patients), who were treated in 2022. They were divided into 3 groups. Group I (main) included 39 (31.4 %) patients whose complex treatment included cytochrome C. Group II (main) included 41 (33.1 %) patients whose complex treatment included cytochrome C and Tivorel. The comparison group consisted of 44 (35.5 %) patients in whom the indicated drugs were not used. \u0000Results. In patients of group I, the 7th day of the study was characterized by a significant decrease in the spontaneous activity of neutrophil granulocytes (NG) in the NBT-test relative to the initial values by 2.21 times (p<0.001). However, a 2.91 (p<0.001) times increase in the activity of induced NG was determined. An increase in the functional spontaneous activity of monocytes relative to the initial values was determined by 3.12 times (p<0.001). Indicators of functional activity in the induced reaction exceeded initial values by 4.96 times (p<0.001). In patients of the II group, on the 7th day of the study, a decrease in the functional activity of NG in the spontaneous NBT-test was found relative to the initial values by 2.94 times (p<0.001). A 4.52-fold increase in NG activity in the induced test relative to baseline values was determined (p<0.001). An increase in the functional activity of monocytes was established in the spontaneous and induced NBT-test. These indicators were 2.93 (p<0.001) times and 5.25 (p<0.001) times relative to the initial values, respectively. \u0000Conclusions.
