Methods The authors choose 255 cases of hypothyroidism, 274 cases of subclinical hypothyroidism and 130 cases of the control group to test serum TC, TG, HDL, LDL, LPa, Hcy and ET. The correlation were analyzed. Results The level of TC, TG, LDL of hyperthyroidism group and subclinical hyperthyroidism group are higher than the control group, HDL is lower than the control group. The level of TC, TG, LDL of hyperthyroidism group is higher than subclinical hyperthyroidism group, HDL is lower than subclinical hyperthyroidism group. The level of Hcy of hyperthyroidism group and subclinical hyperthyroidism group are higher than the control group. The level of Hcy of hyperthyroidism group is higher than subclinical hyperthyroidism group. The level of ET of hyperthyroidism group is lower than the control group and subclinical hyperthyroidism group. There are few reports with atypical SJS induced by medications. This is a case report of atypical SJS induced by Lamotrigine (LTG). A 31-year-old Japanese woman, who was bipolar disorder and prescribed an initial dosage of LTG 25mg/day ten days ago, presented with high fever and throat pain for two days. At first she was suspected of pharyngitis and administered antibiotics. She was hospitalized on the first visit day, but she was getting worse and fell into shock over several days. Furthermore she presented with erosions and bleeding of her lips and oral mucosa on day6, and so she was diagnosed with SJS, but her skin lesions were mild even at the extreme of the clinical course. She gradually developed acute pulmonary failure, liver dysfunction, coma and Disseminated Intravascular Coagulation (DIC). But she was treated with intensive care consisting of steroid pulse followed by high dose corticosteroids, and so she gradually recovered from SJS and corticosteroids could be tapered. At last she was discharged on day50 and had no sequelae with SJS. It was extremely difficult to diagnose this case as atypical SJS because of the unremarkable skin lesions and similarity to pharyngitis. It was also characteristic that she was already in severe status when it satisfied the diagnostic criteria for SJS. But physicians should not miss from the perspective of the fatality and social problem. RT-PCR assay was used to detect the expression of MLH1 gene mRNA in 10 colon cancer cell strains. 62 fresh colorectal adenocarcinoma samples and paired distal normal mucosa were selected in our hospital. They comprised 37 males and 25 females aging 32 to 67 years old with the mean of 43.6. The cellular RNA was extracted, and immunohistochemistry assay was used to detect the expression of MLH1 protein in colorectal cancer, and MSP method was utilized to examine the methylation status of the first exon of MLH1 gene in the tissues. In the 10 cell strains, only the MLH1 mRNAs of SW480 and Caco2 were expressed, and no MLH1 mRNA expression was detected in the other eight strains. The expression level of MLH1 mRNA in the 62 cases of cancer tissue was higher than that in the paired normal mucosa, and MLH1 protein was mainly expressed in the cytoplasm. The expression level of MLH1 protein in adenocarcinoma tissues was significantly higher than that in the paired normal tissue (P<0.01), while the methylation level of cancer tissues was statistically significantly lower than that of the paired normal mucosa (P<0.05). Aberrant DNA methylation dominates the regulation of the MLH1 gene expression changes in colorectal cancer. Methods: to investigate and analyze the incidence of various diseases during the voyage from December 2017 to February 2018 of the crew of a survey ship, to understand the incidence of various diseases during the voyage and analyze the possible causes, and to provide corresponding treatment plans for the incidence of each stage during the voyage, so as to provide scientific basis for the medical support of the survey ship at sea. Results: the survey was conducted on 300 crew members of a survey ship, including 292 men and 8 women, aged 18 to 60, with an average age of 39. The top five diseases in the survey crew during the voyage were upper respiratory tract infection (35.33%), skin disease (22.33%), oral ulcer (14.67%), training injury (14.33%) and gastrointestinal diseases (13.67%). The incidence of upper sensations, skin diseases, oral ulcers and gastrointestinal diseases in the under 30s group was significantly higher than that in the other two age groups (p<0.05), while the incidence of training injuries was mainly concentrated in the 31 - 40s group (p < 0.05). Conclusion: during the voyage, diseases such as upper respiratory tract infections, skin diseases, oral ulcers, training injuries and gastrointestinal tract diseases are more common. {"title":"Statistical Modeling for Findings the Determinants of Neonates’ Very Low Birth Weight","authors":"S. The present study aims to identify the determinates of neonates’ very low birth weight which have significant effects on their birth weight using generalized additive probabilistic modeling. In this present study very low birth weight (BWT) is the response variable with heterogeneity and non-normality in nature which can be modeled through either by the Log-normal or by the gamma models. A well-known method is joint modeling of mean and variance (JGLM) to handle this heterogeneity and non-normality but this study introduced the most advanced regression techniques namely generalized additive model (GAM). Materials and Methods: The present article is based on the secondary data on 174 very low birth weighted neonates along with 26 explanatory factors/ variables. The very low birth weight of 174 neonates is heterogeneous, positive, and gamma distributed. Therefore, generalized additive model with gamma distribution and log link function has been introduced to analyze this very low birth weight. The Native American neonates has the smaller birth weight (BWT) than the other racial neonates namely black, white or oriental (P-value = 0.009). The BWT is smaller for those neonates who were outsiders from Duke (P-value = 0.05) and it also decreases to the female births (P- value=0.09). BWT is higher for the pneumothorax infants than non-pneumothorax with P-value <0.001. It is smaller for those infants for whom oxygen supply had been introduced in between 30 days of his/her birth (P-value=0.002). The neonates who were not been survived they also have the smaller birth weight than the alive neonates (P-value= 0.07). Besides these factors, lowest pH in first 4 days of neonates’ life (P-value=0.09), Apgar score at one minute (P-value=0.03) and the interaction effects of lowest pH and Apgar score (0.03) are the significant variables (continuous cofactors) for neonates’ very low birth weight. Hospital stays in number of days, platelet counts and gestational age in weeks are also highly significant factors and these are entered in the GAM model as a non –parametric smoothing term (P-value<0.001). The birth weight of each new born baby is identified as heterogeneous and gamma distributed (possible). Methods: Type 2 diabetes were established in male SD rats by feeding high lipid diet and injection of streptozotocin. after T2DM models established successfully, the rats were divided into model group, low-dose HJJF group (HJJF1), high-dose HJJF group (HJJF2), and Western medicine group (Gliquidone+Benazepril), and accepted corresponding drugs for 8 weeks respectively. Another normal group were used as control group, fed with normal diet, no drug intervention. The levels of fasting blood glucose (FBG), serum superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), IL-6 and TNF-α were tested. The ERS signaling molecules glucose-regulated protein 78 (GRP78) and c-Jun N-terminal kinases (JNK) mRNA transcription level in thoracic aorta vessel were determined by reverse-transcription polymerase chain reaction (RT-PCR). Results: Compared with model group, After drugs intervention, all administered groups could significantly decrease FPG (P<0.05), elevate SOD and GSH-Px (P<0.05); all could reduce levels of TC, TG, LDL-C, IL-6 and TNF-α (P<0.05), and raise HDL-C (P<0.05); all could depress the transcription levels of GRP78 and JNK mRNA (P<0.05). With increasing dose, the HJJF effect is more significant (P<0.05). The expression of NHX1 in podocytes of diabetic nephropathy patients was observed by immunofluorescence staining and laser confocal microscopy. The immortalized podocytes were cultured in vitro, and the podocytes were stimulated with high glucose for 48 h. RT-PCR, Western blot and immunization were used. Fluorescence detection of mRNA and protein expression of NHX1 in podocytes stimulated by high glucose; Western blot, immunofluorescence and scratch assay were used to detect the expression of NHX1 and the expression of podocin, the activity of podocytes and the nuclear access of TRPV5. The transcription of the target gene downstream of TRPV5 was detected by RT-PCR. NHX1 expression was significantly decreased in podocytes and high glucose-stimulated podocytes of diabetic nephropathy patients (P<0.05). After silencing NHX1, podocyte marker protein podocin expression was significantly decreased and podocyte activity was increased. The nuclear translocation of TRPV5 increased, and the transcription of the target gene downstream of TRPV5 increased (P < 0.05). In contrast, the expression of overexpressed NHX1 group was significantly increased, the activity of podocytes was decreased, the nuclear access of TRPV5 was decreased, and the transcription of the target gene downstream of TRPV5 was decreased (P<0.05). {"title":"Bilateral Semi-Skeletonized IMA; Less Thermal Injury, Easier to Harvest, Early Post Operative Comparison with Single IMA Patients After CABG","authors":"K. A. However, its use is not widespread enough among heart surgeons due to its technical difficulty and the longer period of time it requires. There are still many articles being published on the subject. In patients who receive BITA, harvesting is usually performed in a skeletonized fashion. This protects the patient from sternal complications because it disturbs the vascularity of the chest wall less compared to the pedicled technique. However, the risk for injury on IMA is high. This is where harvesting in semi-skeletonized fashion distinguishes itself. Not only does it disturb the vascularity of the chest wall less but it also has a lower risk for thermal injury. Furthermore, it provides some advantages in terms of time over skeletonized fashion. The study enrolled 24 patients who underwent an isolated CABG operation using cardiopulmonary bypass (CPB) by a single surgeon between March 2017 - December 2017. 12 patients (10 males, 2 females; mean age: 55.083) underwent CABG operation using semi-skeletonized BITA (Table 1). Another 12 patients (8 males, 4 females; mean age: 56.25) underwent CABG using LIMA and venous graft. Post-operative sternal wound complications and post-operative pain of patients were compared. In the patients of the BITA group, cross clamp time, operation time and mammary harvesting time caused a slight prolongation. None of patients had deep sternal infection or mediastinitis or mechanical sternal dehiscence. BITA harvesting patients had more postoperative pain. As a result using semi-skeletonized ITA is more beneficial for sternal wound recovery compared to IMA harvested with the pedicled technique because the former causes less trauma in the thorax and disturbs the bleeding of the chest wall less. EV71's pathogenesis has not understood thoroughly and the antiviral drug has been under development, which led to its emergence as a clinically fatal neurotropic virus. Early diagnosis is one of the main methods to reduce mortality. The aim of this study was to establish and evaluate a colloidal gold immunochromatography for qualitatively detecting the EV71-IgM in serum sample. Method: In this study, the different kinds of coating antigen were the focus of optimization. The pH value, particle size of the colloidal gold and other parameters and materials were also considered to achieve the good performance in detecting strips. The CA16-positive, EV71-positive and negative serum were used to compare and evaluate the accuracy and specificity of strips. Result: The accuracy of the strips coated by EV71-VP1-VP2-VP3 was higher than the EV71-VP1 strips 20%. The specificity of the EV71-VP1 strips was slightly higher than EV71-VP1-VP2-VP3 strips according to CA16 positive serum samples. Compared with commercial strips, this diagnostic method just needed 2 µL serum samples for detection, and the accuracy of the EV71-VP1-VP2-VP3 strips was significantly higher than the commercial strips. In this retrospective study, this study mainly focused on surgery for elderly patients with colorectal cancer. In this study, the characteristics of preoperative, surgical and postoperative variables were described and risk factors for postoperative complication were analyzed. Totally, 221 patients were included in this study. The average age of patients was 78.84 years old and the preoperative nutritional status was roughly normal. 125 (56.56%) patients had comorbid diseases and 146 (67.59%) patients had an ASA score of 3 or 4. Emergency surgery was performed in 12 (5.43%) patients. 125 patients received open surgery while 96 received laparoscopic surgery among which 7 (7.29%) converted to open surgery. 36 (16.29%) patients had to get enterostomy. The average operative time was 220.00 min and the median blood loss was 100 ml with 55 (24.89%) patients needing intraoperative transfusion. As for risk factors for postoperative complication, patients with preoperative comorbid disease, longer operative time, more blood loss and intraoperative transfusion seemed more likely to have complication, although without significant difference. Preoperative low level of album (< 30 g/L) and emergency surgery were independent risk factors for postoperative complication (p = 0.037 and 0.021, respectively). In conclusion, surgery for elder patients with colorectal cancer had its own characteristics, and was controllable in general.
