Hwang","doi":"10.7580/KJLM.2016.40.4.125","DOIUrl":"https://doi.org/10.7580/KJLM.2016.40.4.125","url":null,"abstract":"아동 성폭력은 널리 알려진 사회적 이슈이며, 우리나라에서 도 사회적으로 큰 파장을 일으켰던 ‘지적장애 소녀 가족 내 성폭행 사건’을 비롯하여 소위 ‘조두순 사건’까지 가족 내 뿐 아니라 가족 외 아동 성폭력 사례가 증가되고 있다. 다른 국가들에서는 아동 성폭력범죄자의 교정과정에서 다 양한 정신의학적 평가 및 치료를 도입하였고, 우리나라에서 도 이에 대한 치료와 평가에 대한 여러 가지 연구가 시행되고 있으나, 이 중 왜곡된 성적 인지 및 욕구 충족이 목적인 소아 성애증을 가진 아동 성폭력범죄자들에 대한 연구는 국내에서 미흡한 실정이다[1,2]. 정신질환에 대한 진단에 주로 사용되는 ‘정신질환의 진단 및 통계 편람, 제 5판’에 따르면 소아성애증은 남성 인구에서 3%-5% 정도의 유병률을 보인다고 생각되고, 진단기준은 A, B, C를 모두 충족하여야 한다[3]. {"title":"Case Report of Decapitation in Hanging","authors":"Jung Sik Jang, Kyung-moo Yang","doi":"10.7580/KJLM.2016.40.4.138","DOIUrl":"https://doi.org/10.7580/KJLM.2016.40.4.138","url":null,"abstract":"변사자(남, 49세)는 모텔에서 약 2달간 장기 투숙 중인 자로, 모텔 외벽과 옆 건물인 주유소 담벼락 사이의 비가림 천막 위 에 머리부위가 없이 사망한 상태로 있는 것이 주유소 직원에 의해 발견되었으며, 이후 머리부위는 몸과 1.2 m 떨어진 위치 에서 발견되었다. 변사자가 투숙한 모텔 객실의 나일론 완강 기 줄(폭 약 1.0 cm)이 창문 밖으로 내려져 있었고, 모텔 외벽 과 주유소 담벼락에는 혈액이 묻어 있었다. 변사자가 발견된 지점에서 변사자가 거주한 모텔방까지 높이는 약 9.8 m, 완 강기에 고정된 부위(현수점)에서 목을 맨 매듭까지 줄의 길이 는 약 3.5 m이었다. 매듭은 움직매듭(running noose)이고, 매듭부위에서 머리카락을 포함한 인체조직이 끼여 있는 것 이 발견되었으며, 여기에서 시행한 디엔에이형 분석에서 변 사자와 일치하는 디엔에이형이 검출되었다. 주유소에 설치된 폐쇄회로텔레비전 영상에서 발견 전날 자정쯤 변사자가 모텔 비가림 천막위로 떨어지는 것이 확인되었다. Choi","doi":"10.7580/KJLM.2016.40.4.99","DOIUrl":"https://doi.org/10.7580/KJLM.2016.40.4.99","url":null,"abstract":"사후 검체에서 눈 유리체액 당(glucose) 수치는 고혈당증, 당 뇨병 케톤산증 등을 진단하는 데 있어 중요한 표지자이다. 눈 유리체액에서 당 농도는 사후 초기에 감소하는 경향을 보이 기 때문에 낮은 농도의 당 수치가 사망 당시의 실제 수치를 정확하게 반영한다고 보기는 어려워 당뇨관련 질환이 없다 고 단정하지는 못한다. 하지만 눈 유리체액에서 높은 농도의 당 수치는 고혈당증, 당뇨병 케톤산증 등을 고려해 볼 수 있 는 주요 검사소견이 되어 이러한 질환을 증명하는 데 의미가 있을 수 있다[1]. 기존의 변사자의 병력에 대한 정보가 없을 시에는 일반적으로 부검 시 육안소견만으로는 당뇨와 연관한 질환을 진단하는 데 쉽지 않으며, 추후 조직검사 등을 통해서 진단할 수 있으나 시간이 오래 걸리게 된다[2]. 그러나 눈 유 리체액의 당 검사는 상대적으로 빠른 시간 내에 고혈당증, 당 뇨병 케톤산증 등을 진단하는 데 유용한 검사이다. Autopsy and microscopic examination identified features of diffuse alveolar damage in both lungs including hyaline membranes and intra-alveolar exudate. Gomori's methenamine silver stain of pink frothy materials in these exudates revealed thin-walled and cup-shaped microorganisms and a diagnosis of Pneumocystis jirovecii pneumonia was made. There were small granulomas in the pulmonary interstitium and hepatic lobules representing an unusual inflammatory reaction against Pneumocystis jirovecii. Extrapulmonary involvement with pneumocystis infection is a rare event occurring in 1% to 2% of all pneumocystis cases. Screening and confirmatory tests for human immunodeficiency virus (HIV) detection were positive. There was no information available regarding the patient’s medical history or the possibility of HIV infection prior to the autopsy, because the patient was a foreign worker who arrived in Korea 2 months before her death. Medical examiners often perform autopsies with limited information regarding the deceased person, even when person is a Korean national.
