{"title":"Funcionalidad del miembro inferior con exposición ósea con el uso de terapia con presión negativa VAC® vs. colgajo de gemelo medial","authors":"Enrique Antonio Chau Ramos","doi":"10.24265/horizmed.2023.v23n2.04","DOIUrl":"https://doi.org/10.24265/horizmed.2023.v23n2.04","url":null,"abstract":"Objective: To compare two reconstructive surgery techniques for lower limb injury with exposed bone and demonstrate that the VAC® (vacuum-assisted closure) negative pressure wound therapy is an alternative for potential recovery showing no significant changes that could lead to functional compromise. Materials and methods: An analytical, prospective, quantitative and longitudinal study conducted with all the patients of Clínica Stella Maris with traumatic injuries of the lower limb and exposure of the middle third of the tibia treated with the VAC® system and the medial calf flap in 2019. Results: The measurement obtained with the functional ambulation categories (FAC) scale showed better results among the patients treated with the VAC® technique (since 50 % got grade V) than those who underwent the flap technique (50 % got grade IV), being the differences statistically significant (p < 0.05). It was observed that the time to closure was longer with the VAC® technique due to the progressive regeneration process consisting of the complete filling or coverage of the lesion area. On the other hand, the difference in the postoperative pain intensity between the two techniques was evident, being moderate to intense with the flap technique and mild, for the most part, with the VAC® technique. Conclusions: The VAC® suction system is effective for bone coverage in traumatic defects of the anterior middle third of the tibia. It is an alternative for potential recovery that does not change the anatomical structures because it provides better functional results and fewer complications. {"title":"Apendicolito libre postapendicectomía","authors":"Hubert James Mendoza Rojas","doi":"10.24265/horizmed.2023.v23n2.11","DOIUrl":"https://doi.org/10.24265/horizmed.2023.v23n2.11","url":null,"abstract":"Dropped appendicolith following an appendectomy is a rare complication which may occur as a consequence of appendicolith expulsion from the appendix due to a previous perforation or failure to identify the appendicolith. A fecalith generally becomes symptomatic over time, as a result of abscess formation, intestinal obstruction, fistula tract or inflammation of the appendiceal stump. Two cases of dropped appendicolith following an appendectomy are described hereinbelow. Case 1: A 23-year-old female patient, who underwent an appendectomy the previous day, was admitted to the emergency room due to hypogastric pain. A computed tomography (CT) scan revealed a dropped appendicolith in the right iliac fossa, and a diagnostic laparoscopy showed intra-abdominal abscesses and a dropped appendicolith in the appendicular stump. Case 2: A 77-year-old male patient, who underwent surgery for acute appendicitis caused by appendicoliths, showed abdominal distension on the sixth postoperative day. The CT scan revealed intestinal obstruction and a dropped appendicolith at bowel loops. In the surgical reintervention, erosion of the mesentery caused by the appendicolith was evidenced. Both patients progressed after the reoperations. The incidence of dropped appendicolith following an appendectomy is currently unknown since few data are available in the literature worldwide. Therefore, it is important to present these cases to expand the casuistry, learn more about dropped appendicoliths following an appendectomy, demonstrate their complications if they are not removed early and show how to prevent them. {"title":"Cáncer de mama y mutación del gen ataxia telangiectasia: reporte de caso","authors":"Óscar Alejandro Bonilla Sepúlveda","doi":"10.24265/horizmed.2023.v23n1.11","DOIUrl":"https://doi.org/10.24265/horizmed.2023.v23n1.11","url":null,"abstract":"Among women, breast cancer is the most common cancer in the world, accounting for 25.7 % of all cancers and thus making it a topic of interest in public health. Breast cancer is a complex, heterogeneous genetic disease and, in the vast majority of cases, of unknown etiology. Around 7 % of breast cancer cases in the general population present a susceptibility gene mutation of Mendelian inheritance. The ataxia telangiectasia mutated (ATM) gene mutation is found in less than 1 % of the general population, and association studies conducted with controls have shown that these alleles are characterized by a moderate risk (RR: 2 ‐ 4) for hereditary cancer. Mutations in ATM incompletely cosegregate the disease, with an estimated 15 % of mutation carriers in this gene developing cancer. The incomplete penetrance of ATM, as well as other moderate‐risk genes, supports that they follow a polygenic model of cancer susceptibility. We report the case of a 35‐year‐old woman diagnosed with metachronous bilateral breast carcinoma—T4b N1 M0 stage IIIB left breast and T1c N0 stage I right breast, intrinsic luminal B subtype, HER2 overexpression and pathogenic variant of ATM gene c.7913 G>A, p. Trp2638*—who was treated with neoadjuvant chemotherapy, followed by mastectomy, sentinel node biopsy and radiotherapy.
