Campos","doi":"10.5935/2526-8732.20210020","DOIUrl":"https://doi.org/10.5935/2526-8732.20210020","url":null,"abstract":"Objective: To document the role of physiotherapy in the context of palliative care in cancer through an integrative literature review, as the prevalence and survival of cancer are increasing and physiotherapy can significantly contribute to the treatment of these patients. Methods: A search was made in the literature for scientific articles that substantiated the physiotherapist’s role in palliative care in oncology, and an assessment of the methodological quality of the selected clinical trials. Results: Studies demonstrate positive results, as they propose effective protocols that mostly include exercises in different modalities asso ciated or not with other resources such as PNF, TENS, manual therapy, ultrasound, ther-motherapy, acupuncture, and oxygen therapy, which help in reducing symptoms such as cancer fatigue, pain, and dyspnea, improving the patient’s quality of life. The average score for clinical trials was reasonably low. Conclusion: The results reaffirm the importance of physiotherapeutic interventions in cancer patients receiving palliative care. Métodos: Foi literatura de artigos científicos fisioterapeuta nos cuidados paliativos em oncologia e avaliação qualidade metodológica ensaios clínicos selecionados. Resultados: Os demonstram resultados positivos, pois propõem protocolos eficazes que, em sua maioria, incluem exercícios em diferentes modalidades associados ou não a outros recursos como FNP, TENS, terapia manual, ultrassom, termoterapia, acupuntura e oxige-noterapia, que auxiliam redução de sintomas como como câncer, fadiga, e dispneia, melhorando a qualidade de paciente. The purpose of this study was to evaluate epidemiological data and clinical outcomes of patients with lung cancer treated with non-curative intent assessed by geriatric assessment (GA) and restricted access to new therapies. Methods: We evaluated older adults with non-small cell lung cancer treated with palliative chemotherapy or first generation target therapy who underwent GA. Overall survival (OS) was calculated and we performed univariate and multivariate analysis to determine prognostic factors for OS. Results: Fifty-four patients aged ≥70 years had a median age of 76 years, 76% was classified as Katz A and 50% as Lawton <27. 68.5% were at risk of malnutrition or malnourished; 61.1% had ≥2 comorbidities. The mean OS was 17.1 months. In the univariate analysis, lower survival results were observed for patients with ECOG≥2 (HR=5.9; p<0.001), higher number of metastasis sites (HR=2.0; p=0.04) and liver metastasis (HR=12.6; p<0.001). In multivariate analysis, male gender, ECOG≥2, more than 2 metastatic sites and liver metastases were associated with higher risk of death. We did not observe differences in survival according to functional capacity. Conclusion: GA showed no prognostic relationship in our small population. Patients with ECOG≥2, more than 2 metastatic sites and hepatic metastasis presented higher risk of death. There are limited data on systemic treatment for metastatic disease. Objective: Our study evaluated the median progression-free survival of patients with metastatic tumours, comparing TC and AC status for different treatments. Methods: Retrospective series of patients with metastatic NETp treated from 2002 to 2019 in a large cancer centre were analysed. Our primary endpoint was progression-free survival according to morphological classification (TC vs. AC). All patients received at least one treatment modality (e.g., somatostatin analogue [SSA], chemotherapy [ChP], and everolimus [Eve]). Variables were analysed using the chi-square test, median progression-free survival (mPFS) rates (months), with comparisons evaluated by the log-rank test. Results: Twenty-seven patients were included: 44% with TC and 56% with AC. TC patients were on average 58-years-old, 83.3% were female, and 33.3% received more than one treatment. AC patients were on average 61-years-old, 66.7% were female, and 20% received more than one treatment. All patients were treated more frequently with SSA (TC: 75% vs. AC: 80%, p=0.756). Cisplatin and etoposide were the most frequent ChP regimen (TC: 75% vs. AC: 30%, p=0.248). Patients with TC and AC treated with SSA had higher mPFS in months (TC mPFS SSA: 14.5, Eve: 2.50, ChP: 4.0, SSA + Eve: 4.50; AC mPFS SSA: 7.50, Eve: 4.50, ChP: 7.50, SSA + Eve: 7.00). Conclusion: Although the statistical analyses did not show a significant difference between treatment, numerically, more patients with TC or AC experienced tumor control with SSAs, where the mPFS pairs showed a possible tendency to differentiate themselves from the other regimes (Eve and ChP).
