They safely substitute for traditional chemotherapy in a signiﬁcant and growing number of malignancies. In this article, we review the United States Food and Drug Administration (FDA) - approved targeted and immunotherapies, currently used in oncology and compare their safety and efficacy in young versus geriatric and frail sub-population. The results suggest an overall comparable, if not superior efficacy in several tumor types, with acceptable toxicities across the board compared to cytotoxic chemotherapy and a favorable analysis in the comparison to the results observed in the younger population. The very decision to place elderly patients on exclusive palliative care can no longer be supported based on age or frailty alone. The historical concept of medical oncologists leaning for palliative treatments for these patients needs to be revisited. ABSTRACT A imunoterapia direcionada revolucionou o tratamento do câncer. Eles substituem com segurança a quimioterapia tradicional em um número signiﬁcativo e crescente de doenças malignas. Neste artigo, revisamos a Food and Drug Administration (FDA) - imunoterapias direcionadas aprovadas atualmente usadas em oncologia, e comparamos sua segurança e eﬁcácia na subpopulação jovem versus geriátrica e frágil. Com resultados sugerindo uma eﬁcácia geral comparável, se não superior, em vários tipos de tumor com toxicidades aceitáveis em toda a linha em comparação com a quimioterapia citotóxica, e uma análise favorável na comparação com os resultados observados na população mais jovem. A própria decisão de colocar pacientes idosos em cuidados paliativos exclusivos não pode mais ser sustentada apenas com base na idade ou fragilidade. Methods: Retrospective study included 91 patients with a conﬁrmed diagnosis of HNSCC. A total of 7 genes were analyzed: XRCC1, HOGG1, CYP1A1, GSTM1, GSTT1, GSTP1 and NAT2. Results: Regarding OS, the largest mean diﬀerences were observed comparing GSTT1 rs71748309 null and GSTT1 rs71748309 non-null genotypes (p=0.050). In the gene-gene interaction analysis, the higher diﬀerence to OS was observed to the combined genotypes of the GSTM1 rs4025935 and GSTT1 rs71748309 (p=0.286). Regarding DFS, the largest mean diﬀerences were observed comparing GSTT1 rs71748309 null and GSTT1 rs71748309 non-null genotypes (p=0.060) and to the combined genotypes of the GSTM1 rs4025935 and GSTT1 rs71748309 (p=0.313). Conclusion: None of the polymorphisms evaluated in xenobiotic metabolism or DNA repair genes were signiﬁcantly associated with HNSCC survival in our population. Methods: 126 patients with cancer were in a prospective cohort study, conducted between July/2012 and january/2013, they were interviewed before treatment and after completion of the 1st, 2nd and 3rd cycles, when clinical, demographic and laboratory data were collected. Results: 39,7% had toxicity grade ≥ 3. The latter led to more hospitalization, suspension, delayed treatment and death (p<0,05). Performance status 2 in cycles 1 (p<0,001) and 2 (p=0,025) were risk factors to the toxicity grade ≥ 3. When studied variables prior to first chemotherapy cycle, the body surface area < 1,69 m2 was associated with the occurrence of toxicity grade ≥ 3 (p=0,023) and with anemia (p=0,044) and thrombocytopenia (p=0,006) of any grade. Creatinine clearance < 50mL/min was associated with anemia (p=0,032), BMI < 18,5kg/m2 with thrombocytopenia (p=0,012), lymphocytes < 1500/mm3 to leukopenia (p=0,017), neutrophils < 3100/mm3 to neutropenia (p=0,002) and leukopenia (p<0,001), all of any toxicity grade. Conclusion: Approximately 40% of patients had toxicity grade ≥ 3, motivating more hospitalization, suspension, delayed treatment and death. Performance status 2 and body surface area < 1,69 m2 were related toxicity grade ≥ 3.
