{"title":"Experience of endovenous high-frequency electric welding in the treatment of chronic diseases of the lower extremities veins.","authors":"Valentyn A Khodos, Hlib O Melnychuk","doi":"10.36740/Merkur202405101","DOIUrl":"https://doi.org/10.36740/Merkur202405101","url":null,"abstract":"<p><strong>Objective: </strong>Aim: The purpose was to improve the results of treatment of chronic diseases of the lower extremities veins by using endovenous high-frequency electric welding in automatic mode, and to evaluate the obtained results..</p><p><strong>Patients and methods: </strong>Materials and methods: The results of treatment of 146 patients with chronic diseases of the lower extremities veins in the period from 2018 to 2023 were analyzed. There were 87 women (59.59%) and 59 men (40.41%). Reflux in the great saphenous vein was observed in 124 (84.93%) patients, in the small saphenous vein - in 22 (15.07%). Elimination of reflux in the great and small saphenous veins was performed by the method of endovenous high-frequency electric welding using the device «High-frequency electrocoagulator EK-300M» (Ukraine) in automatic mode. The stability of occlusion in the great and small saphenous veins after endovenous high-frequency electric welding was assessed on the basis of ultrasound examination at 7 days, 1, 3, 6, 12 months and more.</p><p><strong>Results: </strong>Results: The postoperative pain along the treated vein was noted in 2 (1.36%) patients, discomfort - in 124 (84.93%), feeling of a pull on the inner surface of the thigh - in 65 (44.52%), paresthesias in the thigh or lower leg - in 6 (3.57%), inflammatory infiltration along the treated vein - in 1 (0.68%), recurrence of the disease - in 2 (1.19%). Сomplete ablation was achieved in 146 (100%) operated patients. Complete ablation of veins was achieved due to the fact that the parameters of endovenous high-frequency electric welding, which, unlike endovasal laser ablation and radiofrequency ablation, are generated automatically based on the principle of feedback between the device itself and a specific level of impedance of the venous wall, ensure high-quality thermal treatment of all layers of the vein in each individual segment. Aim: To analyze the complications during gynecologic surgery procedures and identify possible risk factors associated with this type of surgery in Ukraine.</p><p><strong>Patients and methods: </strong>Materials and Methods: We conducted a multicenter, retrospective cohort study in tertiary care hospitals from 12 regions of Ukraine from January, 2021, to December, 2023. The study included women who underwent gynecologic surgery at these hospitals. Postoperative complications are defined by Clavien-Dindo classification.</p><p><strong>Results: </strong>Results: A total of 13,937 women were included in our study. The overall prevalence of 30-day complications was 13.9%. The most frequently postoperative complication types were surgical site infections, hemorrhage, urinary tract infection, bowel injury, urinary tract injury, and bladder injury. Age 60 years or older (adjusted odds ratio (aOR 1.81), BMI ≥30 kg/m2 (aOR 1.78), diabetes mellitus (aOR 1.45), procedures for gynecologic cancer (aOR 2.58), prior pelvic surgery (aOR 1.61), emergency procedure (aOR 1.83), ASA-physical status class 5 vs. 1 or 2 (aOR 4.31), operative time greater than 3 hours vs. less than 1 hour (aOR 2.92), wound class 4 vs. 1 (aOR 4.28), and open abdominal approach for surgery vs. laparoscopic procedure (aOR 2.65) were significantly associated with postoperative complications.</p><p><strong>Conclusion: </strong>Conclusions: This study found a high prevalence of complications rate in gynecological surgery varied widely depending on the approach and type of procedure. Quality of life was assessed in 6 and 12 months after AVR with myocardial revascularization using Short Form 36 Survey (SF-36).</p><p><strong>Results: </strong>Results: We selected 130 patients who underwent AVR together with myocardial revascularization (first group (n=51) - a combination of AVR and percutaneous coronary interventions (PCI); second group (n=79) - a combination of AVR and coronary artery bypass grafting (CABG). The duration of cardiopulmonary bypass and aorta cross clamping in first group was recorded by 1.5 times and by 1.4 times significantly lower compared to the second group (p=0.0001). The pain index after 6 months according to the SF-36 questionnaire was found to be 13.5% (p=0.017) significantly higher in patients of the first group compared to the second group, while other indicators did not differ among groups.</p><p><strong>Conclusion: </strong>Conclusions: Patients with the hybrid approach of treatment were characterized by a similar level of postoperative complications with the surgical group. After the initial processing, 43 studies were selected for further analysis. They were structured and characterised according to the following criteria and features: types of research; time it was conducted; its authors; participants in sociological and experimental studies. The reflection of the dominant mental states (stress, anxiety, fear, depression, oppression, pessimism, confusion, apathy) in empirical, empirical-analytical, and experimental studies has been determined. The results of studies of mental states in the fields of medicine, psychology, etc.,have been characterised. Treatment is difficult due to the lack of causal therapy, guidelines and requires an individualized approach. The most common treatments are systemic retinoids, immunosuppressants, phototherapy and biological therapy. This article presents the case of a 73-year-old man suffering from type 1 pityriasis rubra pilaris. The patient was initially treated with acitretin, which was discontinued due to hypogammaglobulinemia. This rare side effect of acitretin has not been previously published. As a second-line treatment, the patient received methotrexate, but with no clinical improvement after 3 months and an increase in skin pruritus. In either method, the therapist is a covered therapeutic mirror for the patient/client. Music therapy, unlike psychotherapy, works almost exclusively on emotions and on the symbolic layer of expression of personality. In the case of music therapy, the verbal layer plays a less significant role than in the case of psychotherapy. The clinical effectiveness of music therapy has been studied much less than that of psychotherapeutic methods. Samples were collected in Al-Najaf city in Iraq, from Al-Forat Al-Awsat Oncology Center. The study was achieved at the Laboratories of the Faculty of Science in the University of Kufa. Fifty samples of breast cancer tissue, and twenty controls benign tissue were included in the study. The study has investigated relationship between expression of biomarker with grade, age of patient and tumor size.</p><p><strong>Results: </strong>Results: The study showed that the cytoplasmic expression of CD74 with more clear and intensive staining in the cytoplasm, and reported that CD74 positivity rate was 52%. A significant association between CD74 expression and grade and size of tumor, so CD74 can be considered as a biomarker for prediction of breast cancer in women. {"title":"Analysis of indicators of caries of permanent teeth in children according to the endemic features of the Poltava region, Ukraine.","authors":"Lyudmila I Amosova, Lyudmyla F Kaskova, Oksana S Pavlenkova, Olena V Khmil, Svitlana Ch Novikova, Larysa P Ulasevych","doi":"10.36740/Merkur202401117","DOIUrl":"10.36740/Merkur202401117","url":null,"abstract":"<p><strong>Objective: </strong>Aim: This research is focused at analyzing the indicators and substantiating the peculiarities of caries prevention in permanent teeth in schoolchildren of Poltava region, taking into account the endemic features of the Poltava region.</p><p><strong>Patients and methods: </strong>Materials and Methods: A comparative study was conducted among 608 pupils of secondary schools in Kremenchuk, who consumed drinking water with fluoride concentrations in the lower limits of the norm, and 1214 pupils of secondary schools in Poltava, who consumed drinking water with fluoride concentrations in the optimal upper limits of the norm.</p><p><strong>Results: </strong>Results: The rates of caries in permanent teeth in children living in a region with fluoride concentrations in drinking water in the optimal-upper normal range are several times lower than in children of the same age living in a region with fluoride concentrations in drinking water in the lower normal range, and a significant increase in the prevalence and intensity of caries is observed from 7 to 9 to 12 years of age, as teeth after eruption are most vulnerable to caries. In a region where the fluoride concentration in drinking water is in the optimal-upper range of the norm, children with early forms of fluorosis have the lowest prevalence and intensity of caries.</p><p><strong>Conclusion: </strong>Conclusions: Endemic features of the region directly affect the prevalence and intensity of the caries process. The structure of the carotid artery and its stiffness were assessed using the ultrasound method, and the level of abdominal fat was determined using dual-energy X-ray absorptiometry.</p><p><strong>Results: </strong>Results: Carotid Intima-Media Thickness in patients with essential hypertension reliably exceeded the corresponding indicator of the control group. We observed a significant increase in arterial stiffness indicators, which is explained by the increased stiffness of blood vessels in patients with obesity. They were divided into two groups. Group I received adrenaline alone in one side and a mixture of adrenaline and tranexamic acid in the other side. Group II received adrenaline alone in one side and tranexamic acid in the otherside. Parameters like surgery time, blood loss, and surgical field quality were studied.</p><p><strong>Results: </strong>Results: In Group I, the combination of adrenaline and tranexamic acid significantly reduced blood loss and enhanced surgical field quality compared to adrenaline alone. In Group II, adrenaline outperformed tranexamic acid in shortening surgery duration and improving surgical field quality. However, there was no significant difference in blood loss reduction between adrenaline and tranexamic acid.</p><p><strong>Conclusion: </strong>Conclusions: The study concluded that tranexamic acid is less effective than adrenaline when introduced as topical intranasal pledgets in both decreasing the time needed for the surgery and improving the subjective satisfaction of the surgeon while there is no significant difference regarding decreasing intraoperative blood loss.
