A female infant (aged 9 months and birth weight 2500 gm) was selected as participant. After physical examination, the state of the participant was good (fever:37.6ºC, heart rate: 120/min, respiratory rate: 40/min). Holding a good health certificate, a cystic image was identified at pulmonary artery in chest X-ray report. The histology findings were consistent with CPAM Type I. CPAM surgery, generally favorable, results in low postoperative morbidity and mortality. CPAM is a rare developmental malformation of lung that causes pulmonary compression and hypoplasia leading to respiratory distress but the postnatal management of asymptomatic CPAM remains controversial.

Its incidence in children is probably underestimated and cases reported in the literature, particularly in pediatrics, are rare. \u0000Presentation of Case: we report the case of an 11-year-old male child diagnosed with pulmonary embolism complicating infective endocarditis. A thoracic CT angiography confirmed a distal pulmonary embolus, aneurysms of the lobar and segmental branches of the pulmonary artery and nodules and excavated pulmonary masses in favor of septic emboli. The evolution was unfavorable despite anticoagulant treatment and adequate antibiotherapy. \u0000Discussion: an overview of septic embolism associated with infective endocarditis will be the main topic focusing on key diagnostic and therapeutic choices. An early pulmonary CT angiography in case of any thoracic symptoms that do not evolve favorably in a child is highly recommended. \u0000Conclusion: The identification of other pediatric cases will be useful in order to establish diagnostic and therapeutic guidelines specific to the pediatric population.

Ingestion is often accidental in children and a real public health problem in low and middle income countries, particularly in Africa. \u0000Aims: The main objective of this study was to describe the epidemiological, diagnostic, therapeutic and scalable aspects of accidental ingestions of caustics in the medical emergency department of the National Children's Hospital Center Albert Royer (NCHCAR) in Dakar. \u0000Methods: This is a descriptive and analytical study over a 12-month period, from March 2020 to February 2021. The study population was children aged 0 to 15 years who were seen in consultation or hospitalized following a caustic product ingestion. \u0000Results: During the study period, we included 31 children for accidental ingestion of caustics out of a total of 900 hospitalized children, i.e., a hospital frequency of 3.44% (31/900). The mean age of the children was 21.29±13.10 months and the extremes were 8 and 60 months. The age group [12-18 months] was the most represented (25.81%). Boys were predominant (51.61%) with a sex ratio of 1.07. Sodium hypochlorite (bleaching water) was the main product ingested (64.52%); caustic soda concerned 22.58% of children. More than half (77.45%) of the children were symptomatic with mainly digestive signs including oral ulcerations (41.93%) and vomiting (38.71%). The oesogastroduodenal fibroscopy performed in 19 of the 31 children (61.29%) was normal in 14 children (73.68%). Five children had endoscopic lesions of which 2 cases were stage I and 3 cases stage IIa of the Zargar classification. Initial management was exclusively medical with antisecretory drugs (70.97%), analgesics (38.71%) and corticosteroids (35.48%). The evolution was favorable for the majority of patients. One child presented a dysphagia to solids after 4 weeks of evolution treated by endoscopic dilatation. \u0000Conclusion: Accidental ingestions of caustics are frequent in emergency departments. They generally affect children under 24 months of age. The management is medical in the uncomplicated forms which are more and more predominant. Prevention is based on education and awareness of the dangers of caustic products.

Most patients require long-term, and sometimes life-long, therapy with antiepileptic drugs (AEDs). AEDs are associated with significant side effects including radiological evidence of rickets, decreased bone mineral density (BMD), altered bone turnover, and increased risk of fracture. \u0000Aim of the Study: To assess the bone mineral density in epileptic children getting long term anti-epileptic drugs. \u0000Materials and Methods: This study was conducted in the Saleh Child Development and Disability Management Center (SCDDMC), of the Institute of Child and Mother Health (ICMH) Matuail, Dhaka. A total of 31 childrens age ranged 5-15 years diagnosed as epilepsy on the basis of both clinical examination and investigation (EEG) receiving antiepileptic drugs (AEDS) for at least two years were recruited in this study. \u0000Results: Almost two third (61.3%) children age group were in 5 to 10 years and 12(38.7%) in 10 to 15 years. Around three fourth (74.2%) children were male and 8(25.8%) were female. More than two third (67.7%) children had adequate sun exposure and more than three fourth (77.4%) children got adequate calcium and vit.D rich food. Majority (83.9%) of the patients got phenobarbitone (PB) followed by 17(54.8%) got combination, 13(41.9%) got valproic acid (VPA), 11(35.5%) got Nitrazipum (NTZ). More than one third (38.7%) children got mono therapy, 19(61.3%) got polytherapy. Majority of the children (96.8%) had serum calcium normal. The mean serum calcium was 9.22±0.78 mg/dl. The mean BMD of spine was 0.66±0.14. The mean BMD of neck left femur was 0.66±0.15. The mean BMD of neck right femur 0.66±0.15. More than two third (67.7%) children had low (<-1SD) BMD Z score. Almost two third (60.0%) children had mono therapy showed BMD Z score normal (>-1SD) and 6(28.6%) had BMD Z score low (<-1SD). Almost three fourth (71.4%) children got poly therapy showed low BMD and 4(20.0%) got poly therapy showed normal BMD Z score. No significant association was found between BMD score and type and duration of therapy. \u0000Conclusion: AEDs decreased bone mineral density due to long term use as mono-therapy or polytherapy. The assessment of BMD among children with epilepsy receiving long term AEDs is essential.

Neurofibromatosis (NF) is a neurocutaneous syndrome characterized by the development of tumors of the central or peripheral nervous system including the brain, spinal cord, organs, skin, and bones. Prineurinoma is a relatively rare tumour that has been shown to be a proliferation of perineural cells with an incomplete basal lamina and no onion bulb appearance. Neuroma also known as traumatic neuroma or amputation neuroma or stump neuroma, is a focal non-neoplastic area of proliferative hyperplastic reaction secondary to peripheral nerve damage that commonly occurs after a focal trauma (acute or chronic) or surgery, such as amputation or partial transection; We report an extremely rare clinical case of a 5 year old child with neurfibromatosis congenita type 1 with asyntomatic medullary comrepssion and incidental discovery of a left intraductal C4 perineurinoma during the exploration of an occipital-parietal neuroma associated with a dehiscence of the lambda suture. Echocardiography is essential for the diagnosis of HCM, to assess the left ventricular function, and the degree of left ventricle obstruction, and to rule out structural cardiac malformations. \u0000Objective: To study the association of maternal third-trimester glycemic level with HCM in IDMs. \u0000Study Design: Observational cross-sectional study. \u0000Participants: 108 diabetic mothers and IDMs. \u0000Intervention: Third-trimester Glycosylated hemoglobin (HbA1c) levels were measured and Echocardiography was done for IDMs in their first week of life. Data were analyzed for correlations between various parameters. \u0000Outcomes: HCM was diagnosed when the absolute value of Inter Ventricular Septum (IVS) was ≥ 2SD from the mean (5mm) and Asymmetrical Septal Hypertrophy (ASH) was diagnosed when the IVS: LVPW ratio was ≥ 1.3. \u0000Results: 85% of the study population had gestational diabetes, and 55.6% of them were on diet control only. The mean HbA1c was 6.69%. The mean birth weight was 3.1 kg. The most common complication observed was hypoglycemia (19.4%). Asymptomatic HCM was seen in 52% and ASH in 25.9% of the study population. HCM was found to be significantly associated with maternal HbA1c levels, birth weight and hypoglycemia. \u0000Conclusion: HCM is a frequent finding in IDMs when routinely screened by echocardiography. A significant positive association of HCM with maternal glycemic control was seen in our study and hence strict glycemic control during pregnancy is of paramount significance.

To determine the role of birth weight in predicting changes in serum ferritin levels. \u0000Methodology: This was a prospective comparative cross sectional study which was carried out at the Neonatal Intensive Care Unit of the University of Nigeria Teaching Hospital (UNTH), Enugu, Nigeria between June and December 2014. The study included 140 newborns of all birth weights delivered at the UNTH. These were categorized into extremely low birth weight (ELBW), very low birth weight (VLBW), low birth weight (LBW) normal birth weight and macrosomic. Babies with C-reactive protein levels > 10mg/dl, who were intra-uterine growth restricted, and whose mothers had conditions associated with low iron stores were excluded from the study. Anthropometric measurements were done for all subjects. Serum ferritin was measured at birth and this was correlated with birth weight and gestational age. \u0000Results: Serum ferritin levels ranged from 20.6 to 296.4µg/l. Ferritin levels showed a steady increase in relation to birth weight. (F = 42.453, P < .001). There was a significant association between ferritin levels and categories of birth weight. ELBW babies were 98 times more likely to have low ferritin levels than babies with normal weight (P < .001, OR = 97.600, 95% C.I = 10.083 – 944.711). VLBW and LBW babies were more likely to have low ferritin levels than babies with normal weight (P < .001, OR = 0.070, 95% C.I = 0.017 – 0.291)(P = .006, OR = 0.201, 95% C.I = 0.064 – 0.635). Only birthweight was a significant positive predictor of low ferritin levels (P = .024). \u0000Conclusion: Serum ferritin in new-borns showed a significant association with birth weight. This review provides a systematic evaluation of the published medical literature on potential etiologies and associated pathogenesis that may lead to CAL in children, in order to broaden clinical diagnosis and avoid misdiagnosis and underdiagnosis. The main pathogenesis of CAL is an innate immunity imbalance due to exposure of genetically susceptible people to various infections and/or environmental factors. Kawasaki disease is not the only cause of CAL, and pediatricians need to better understand the immunological mechanisms of the CAL to suspect and diagnose. In addition, attach great importance to rheumatic immune diseases and cardiovascular diseases secondary to CAL.
