{"title":"Die Relevanz des angeborenen Immunsystems für die atopische Dermatitis","authors":"R. However, few reports have investigated these factors simultaneously. The purpose of this study was to analyze neonatal immune responses to microbial stimuli as well as intestinal colonization of beneficial bacteria, in relation to the development of AD in a birth cohort. Methods: Pregnant women were recruited, and their infants were followed up until 7 months of age. Levels of interleukin (IL)-10 released from cord-blood mononuclear cells (CBMCs) stimulated with heat-killed gram-positive bacteria (Bifidobacterium bifidum and Lactobacillus rhamnosus GG) and Lactobacillus-derived peptidoglycan were measured. Fecal Bifidobacterium counts at 4 days and 1 month were quantified using real-time polymerase chain reaction. The development of AD was determined by means of a questionnaire at 7 months of age. Results: The levels of released IL-10 were significantly lower in infants with AD (n = 17) than in infants without AD (n = 53) for all stimuli. In infants with fecal Bifidobacterium, the incidence of AD was inversely associated with the release of IL-10 from cord blood mononuclear cells. Wir wiederholen uns aber nicht – denn diesmal steht die 50. Tagung der Deutschen Dermatologischen Gesellschaft von Anfang Mai im Zentrum des DermaCampus und damit auch einige der dort sehr präsenten Themen: Digitalisierung und Weiterbildung eben. Zum einen fallen diese Stichworte schon im Interview mit Prof. Leena Bruckner-Tuderman und ihrem Nachfolger als Präsident der DDG, Prof. Tilo Biedermann. Prof. Bruckner-Tuderman konnten wir direkt in Berlin sprechen und Prof. Biedermann fasste uns im Nachgang zur Tagung seine Sicht auf die Themen der Dermatologie in den kommenden Jahren zusammen. Immer wieder fallen dort die Begriffe Digitalisierung, Telemedizin oder auch Machine-Learning. Doch selbst Machine-Learning Experte Prof. Florian Theis sieht darin ersteinmal Tools, die die Diagnostik unterstützen, wie er im Rahmen seines Vortrags bei der DDG Tagung erklärte. Gut ausgebildete Dermatologen und Dermatologinnen werden also nach wie vor gebraucht – einige herausragende dermatologische Leistungen wurde in Berlin ausgezeichnet. Theurich, M. von Bergwelt-Baildon","doi":"10.1159/000500890","DOIUrl":"https://doi.org/10.1159/000500890","url":null,"abstract":"Kutane T-Zell-Lymphome (CTCL) sinde seltene und biologisch heterogene maligne Erkrankungen; die häufigsten Unterarten sind die Mycosis fungoides und das Sézary-Syndrom. Nach den derzeitigen Therapien ist der Verlauf durch hohe Rezidivraten geprägt, das Überleben ist jedoch in niedrigen Tumorstadien in der Regel nicht signifikant verkürzt. Anders ist dies in höheren Tumorstadien bzw. aggressiveren Subtypen des CTCL; hier ist die Überlebenszeit signifikant reduziert. In jüngster Zeit sind bei verschiedenen Aspekten der Tumorbiologie Fortschritte erzielt worden: Die Genomsequenzierung hat neue Strategien der NF-κB- oder JAK-Inhibition hervorgebracht, in der Translationsforschung wurden neue Zielmoleküle wie CD30 oder CCR4 identifiziert, und nicht zuletzt hat die Forschung zur Beeinträchtigung der Immunüberwachung an Bedeutung gewonnen. Auf der Grundlage der zunehmenden Kenntnis der funktionellen Rollen der Tumormikroumgebung und nichtmaligner infiltrierender Zellen zielt die Forschung gegenwärtig darauf ab, neuartige Behandlungsstrategien bei CTCL zu entwickeln. {"title":"Hormonelle und gynäkologische Risikofaktoren bei frontal fibrosierender Alopezie","authors":"E. Makrantonaki","doi":"10.1159/000501220","DOIUrl":"https://doi.org/10.1159/000501220","url":null,"abstract":"Introduction: Frontal fibrosing alopecia (FFA) is a condition that likely involves an interplay of autoimmune, hormonal, and environmental factors in its pathogenesis. There is a lack of comparative studies demonstrating the presence of hormonal background differences in FFA patients compared to the general population. Materials and Methods: A single-center case-control study was designed, including 104 female FFA patients and 208 controls. Patients and controls were interviewed, and extensive data regarding their gynecological and hormonal background were recorded. Results: One hundred four cases and 208 age-matched controls were included in the study. A significant difference of 2 years in the age of menopause was detected with a consistent mean increase in fertile life for the control group of 1.7 years. After the multivariate analysis, we found previous intake of tamoxifen to be a risk factor for the development of FFA (OR 14.89). The only protective factor identified was the previous use of an intrauterine device (IUD) (OR 0.22). Conclusions: An earlier menopause and tamoxifen intake might promote or maintain FFA, while the use of an IUD might protect from developing FFA. Wegen der Pathomechanismen, die bei der Karzinogenese des kutanen Plattenepithelkarzinoms eine Rolle spielen, wurde postuliert, dass diese Tumoren möglicherweise durch eine PD-1/PD-L1-Blockade beeinflussbar sind. Der vorliegende Bericht beschreibt den Fall eines Patienten, der wegen eines kutanen Plattenepithelkarzinoms mit Lymphknotenbeteiligung und Lungenmetastasen, das gegenüber 2 Therapielinien mit platinbasierten Schemata und Salvage-Operation refraktär war, eine Behandlung mit Nivolumab erhielt. Der klinische Verlauf war durch ein atypisches Ansprechmuster mit initialem Rückgang der Weichteilläsionen/viszeralen Läsionen bei gleichzeitig neu auftretenden Knochenläsionen gekennzeichnet. In den anschließenden bildgebenden Untersuchungen zeigten sich insgesamt eine Verbesserung und eine anhaltende Krankheitskontrolle unter Fortführung der Therapie. {"title":"Depressive Psoriasis-Patienten können ein schlechteres Ansprechen auf Etanercept zeigen - eine weitere Expertenmeinung","authors":"U. Methods: A total of 85 psoriasis patients who received 6 months of etanercept treatment were consecutively enrolled in this prospective cohort study. The Psoriasis Area and Severity Index (PASI) score was evaluated at month 0 (M0), M1, M3, and M6, and the corresponding PASI 75/90 response at each visit was assessed. Also, anxiety and depression symptoms were assessed by the Hospital Anxiety and Depression Scale (HADS) at M0, M1, M3, and M6. Results: Depression symptoms were observed to correlate with female gender (p = 0.004), longer disease duration (p = 0.018), and higher PASI score (p < 0.001), and anxiety symptoms were seen to be associated with female gender (p = 0.017), larger psoriasis-affected body surface area (p = 0.049), and higher PASI score (p = 0.017) in psoriasis patients. After etanercept treatment, HADS-Depression (HADS-D) and HADSAnxiety (HADS-A) scores were both decreased at M1, M3, and M6 (all p < 0.001) compared with M0. Most importantly, baseline depressed patients presented with a lower PASI 75 response rate at M3 (p = 0.014) and M6 (p = 0.005), and a reduced PASI 90 response rate at M6 (p = 0.045) compared with baseline non-depressed patients. Furthermore, multivariate logistic regression analyses revealed that depression symptoms at baseline were an independent predictive factor for the lower possibility of both PASI 75 response (p = 0.048) and PASI 90 response (p = 0.048) achievements at M6 in psoriasis patients. However, no correlation of baseline anxiety symptoms with PASI 75/90 responses was observed. {"title":"Melanoma in situ/Lentigo maligna: Wo und bei wem sollte ich darauf achten?","authors":"J. Maschke","doi":"10.1159/000499003","DOIUrl":"https://doi.org/10.1159/000499003","url":null,"abstract":"Introduction: Lentigo maligna (LM) is a rare form of in situ melanoma, frequently seen as a large patch in elderly patients. The aim of this study was to assess clinical and dermoscopic features of LM. Material and Methods: A retrospective study of LM patients presenting to our center between July 2007 and July 2017 was performed. Demographic data, anatomical location, laterality, diameter, Clark level, Breslow stage, ‘ABCD' signs and dermoscopic features were registered. Facial versus extrafacial LM were compared. Results: We found 21 LM, of which 12 had an extrafacial location and 9 a facial location. Half of the extrafacial lesions were located on an upper limb. The median age at diagnosis was 63 years (ranging from 38 to 84 years). Most LM cases were female (16/21) with phototype II (13/21). More than half of the patients (11/21) had a history of a skin neoplasm or actinic keratosis. The median diameter found was 6 mm (interquartile range = 4.5 mm), ranging from 1 to 15 mm. Five lesions were invasive (median Breslow depth of 0.2 mm), and 4 of them were extrafacial. Discussion: In this study LM was more frequently found in an extrafacial location and as a small patch with a 6-mm diameter medium. The epidemiology of LM/LM melanoma might be changing. Full body examination and dermoscopy are of the utmost importance for the diagnosis.
