{"title":"[Real-world evidence on pharmacological appropriateness in type 2 diabetes mellitus and cardiovascular disease].","authors":"Carolina Pintos, Mariana Andrea Burgos, Natalia Inés Pasik, Sofía Piccioli, María Florencia Grande Ratti, María Paula Russo","doi":"10.31053/1853.0605.v80.n4.42272","DOIUrl":"10.31053/1853.0605.v80.n4.42272","url":null,"abstract":"<p><strong>Objective: </strong>To estimate the proportion of individuals with established Type 2 Diabetes Mellitus (T2DM) and Cardiovascular Disease (CVD) who are receiving pharmacological anti-diabetic treatment with evidence of cardiovascular benefit at a hospital in Argentina.</p><p><strong>Materials and methods: </strong>Cross-sectional study conducted at the Italian Hospital of Buenos Aires. A consecutive sample of adult patients affiliated with the institutional prepaid health plan active in March 2020, diagnosed with T2DM and established CVD, was included. Data were collected from the Electronic Health Record. The proportion of pharmacological adequacy (combined use of metformin plus sodium-glucose co-transporter 2 inhibitors and/or glucagon-like peptide 1 receptor agonists) was reported along with its respective 95% confidence interval (CI).</p><p><strong>Results: </strong>A total of 1539 patients were included, with a mean age of 76.2 years; 65.3% were male, and 81.6% were overweight or obese. Hemoglobin A1c levels were recorded in the past year for 74.9% of patients, with an average value of 6.9% (SD 1.2). The most prescribed drugs were metformin (61.3%), insulin (26.7%), and gliptins (11%). Out of the total included patients, 82 exhibited pharmacotherapeutic adequacy for diabetes treatment, with a prevalence of 5.3% (95% CI 4.2-6.5).</p><p><strong>Conclusions: </strong>The prevalence of prescribing anti-diabetic drugs with evidence of cardiovascular benefit was 5.3% (95% CI 4.2-6.5). {"title":"[Recommendations for psoriasis treatment in Pediatrics].","authors":"Paula Carolina Luna, María Eugenia Abad, Margarita Larralde, Paula Boggio, Bruno Ferrari, María Fernanda Maccario, Carla Castro, Silvia Moreno, Débora Kaplan, Cristina Echeverría","doi":"10.31053/1853.0605.v80.n4.42874","DOIUrl":"10.31053/1853.0605.v80.n4.42874","url":null,"abstract":"<p><strong>Introduction: </strong>in one third of patients with psoriasis, symptoms start during childhood and adolescence, with a strong emotional and psychosocial impact.</p><p><strong>Objective: </strong>to develop a guideline for the systemic treatment of psoriasis in pediatric patients by means of recommendations based on the best available evidence.</p><p><strong>Materials and methods: </strong>Sources: articles indexed in PubMed, Epistemonikos, Google Scholar, Cochrane Library and Scielo, published between January 2010 and May 2022, in English, Spanish and Portuguese. Study selection: evidence-based clinical practice guidelines, systematic reviews, meta-analyses, randomized controlled studies, observational studies (case-control, cohort studies, real-life registries) and evaluations of biosimilar drugs in patients up to and including 17 years of age were considered. The keywords \"psoriasis\" and \"treatment\" were used in all three languages. Data extraction: the literature was evaluated using Grading of Recommendations Assessment, Development and Evaluation (GRADE) recommendations. Data synthesis: evidence tables were developed and analyzed by the expert committee. The questions for the development of recommendations were based on the PICO system (population, intervention, comparison, outcome).</p><p><strong>Results: </strong>A total of 8 recommendations and 7 points of good practice were developed. Percutaneous biopsy has become a transcendental tool for its diagnosis. Traditionally, computed tomography is used for these procedures because of its ability to clearly demonstrate bone and aerated lung. However, in selected cases it can be performed with ultrasound.</p><p><strong>Methods: </strong>Retrospective study conducted between January 2020 and December 2021, during the SARS-CoV-2 pandemic. All patients had pleural-based lung lesions or pleural lesions, some with a known history of cancer.</p><p><strong>Results: </strong>Thirty-six procedures were performed, in 32 (88.9%) the sample obtained presented diagnostic yield and the most used additional test was Immunohistochemistry in 23 (63.9%). Complications were reported in 5 patients (13.9%): 2 with mild pneumothorax, 2 with hemothorax (1 mild and 1 moderate) and 1 patient reported pain.</p><p><strong>Conclusion: </strong>Ultrasound is a valid method to be used as a guide for biopsies of pleural and peripheral pulmonary lesions. The objective of this study is to evaluate the oncological and perioperative results in patients with high-risk endometrial cancer (HREC) limited to the uterus, treated at the Hospital Italiano de Buenos Aires, between January 2010-2018.</p><p><strong>Methods: </strong>Retrospective cohort study that evaluated perioperative results, disease-free survival at 2, 4 years in patients with HREC.</p><p><strong>Results: </strong>Of a total of 123 patients, 74 met the inclusion criteria. Serous tumors were the most frequent histological type, n=38 (51%), while dedifferentiated tumors were the least frequent, n=2 (3%). Of all the patients included, 56 (76%) received at least one adjuvant treatment. Taxol platinum-based chemotherapy was implemented in 28 patients (38%), while 24 (33%) received a combination of chemotherapy and radiotherapy. The median follow-up time was 2.9 years. Disease-free survival in patients with stage IA at 2 and 4 years was 71% (95% CI 55-82) and 63% (CI 46-76), respectively, while those with stage IB were 53 (95% CI 33-70) and 38 (95% CI 19-58). Regarding the surgical approach, no significant differences were found in disease-free or overall survival when comparing the laparoscopic with the laparotomy approach (p=0.06).</p><p><strong>Conclusion: </strong>Only the FIGO stage showed an increased probability of death or relapse regardless of the type of adjuvant treatment and the type of surgery approach. The Buenos Aires Metropolitan Area (AMBA) region has the highest urban density in Argentina and has been the epicenter of COVID-19 infections and deaths. The objectives of this study are to characterize the habits of screen use, sedentary lifestyle and physical activity and describe their associations, in children under six years of age from AMBA, Argentina.</p><p><strong>Methods: </strong>analytical and cross-sectional study, in which data from the online survey (Google Forms®) were used. Through a non-probabilistic convenience sampling, fathers, mothers, legal guardians, and caregivers of children under six years of age were invited to participate. The survey was available during four months of the quarantine period and was made up of 31 questions about screen use, physical activity, and sedentary lifestyle of children.</p><p><strong>Results: </strong>256 surveys were answered, the majority (79.6%) were female and 50.8% in the age range of 31-40 years.</p><p><strong>Conclusion: </strong>The television is the most used and children begin to use touch screens from one year. The play time of adults with children and the time that they usually carry out activities such as reading a book or drawing is one to two hours per day. There was an association between owning the screen and using it for more hours. The mean age of presentation was 35 years. Seven of the patients were men and the remaining three were women. Most of them presented risky sexual intercourse as an epidemiological history. Pseudopustules were observed in 70% of the patients and all had genital, gluteal or perianal lesions. The complications observed were: local edema, proctitis, conjunctivitis and pharyngitis.</p><p><strong>Conclusion: </strong>we present 3 female patients out of a total of 24 women reported in the country, which represent only 2% of mpox infections in Argentina. In most cases we observe pseudopustules, an elementary lesion recently described for this entity. The diagnosis of adnexal torsion is challenging since both symptoms and physical examination are nonspecific. In most cases, the patient presents abdominal pain, followed by nausea and vomiting.
