Isokinetic parameters, including peak torque (PT), are common, but non-conventional parameters like speed and acceleration are understudied. Investigating these parameters enhances insight into musculoskeletal proficiency in injured players. The present case study explores conventional and untraditional isokinetic parameters in an OP soccer player.</div></div><div><h3>Material and methods</h3><div>A 20-year-old male soccer player with a 10-month history of groin pain was assessed using isokinetic dynamometry. Isokinetic testing at 60°/s and 180°/s assessed hip and trunk muscles. Conventional and non-traditional isokinetic parameters were analyzed for all selected muscles in both contraction modes.</div></div><div><h3>Results</h3><div>The change for each parameter (given as a percent) was computed at various speeds and contraction modes for all tested muscles.</div></div><div><h3>Conclusion</h3><div>The current case study presents a comprehensive analysis to identify the gold standard isokinetic parameters impacting the muscle performance of a soccer player with OP.</div></div><div><h3>Hintergrund</h3><div>Osteitis pubis (OP) ist eine ernsthafte, schmerzhafte Erkrankung, die die Muskelleistung von Fußballspielern erheblich beeinträchtigt. Isokinetische Parameter wie das Spitzenmoment (PT) sind weit verbreitet, doch nicht-konventionelle Parameter wie Geschwindigkeit und Beschleunigung sind bislang wenig untersucht. Zeitkritisches und zielgerichtetes Handeln zur raschen Erkennung und prioritätsorientierten Behandlung akut lebensbedrohlicher Verletzungen ist neben fundierten theoretischen Kenntnissen Voraussetzung für ein bestmögliches Outcome des Unfallopfers. Um die vorliegenden Verletzungen adäquat behandeln zu können, müssen klare Vorgaben in der Behandlungsreihenfolge eingehalten werden. Die Algorithmen des etablierten xABCDE-Schemas ermöglichen neben einer strukturierten Erfassung der medizinischen Probleme und Verletzungen auch eine Priorisierung der erforderlichen therapeutischen Maßnahmen.</div><div>Bänder- und Sehnenrisse, Verstauchungen, Zerrungen und Knochenbrüche gehören zu den häufigsten Verletzungen in vielen Sportarten. Besorgniserregend ist aber, dass fast ein Drittel der verletzten Sportler*innen über 16 Jahre die in einer landesweiten Kohortenstudie evaluiert wurden, eine sehr schwere Verletzung oder ein Polytrauma erlitten. Schwere Kopf- und Nackenverletzungen, thorakoabdominale Verletzungen und Verletzungen im Beckenbereich werden meistens als Ursachen für traumatische Todesfälle bei Sportler*innen angegeben. Aktuelle sportartübergreifende epidemiologische Studien zeigen übereinstimmend, dass der Motorsport den höchsten Anteil sowohl an (1) allen, (2) schweren und (3) tödlichen Verletzungen aufweist.</div></div><div><div>The optimal care of severely injured athletes in the early phase of trauma poses a particular challenge for team and event physicians. Time-critical and targeted action for rapid recognition and priority-oriented treatment of acute life-threatening injuries is essential for achieving the best possible outcome for the trauma victim, alongside solid theoretical knowledge. Clear guidelines must be followed in the treatment sequence to address existing injuries adequately. The established xABCDE scheme’s algorithms facilitate a systematic recording of medical issues and injuries while prioritizing the necessary therapeutic interventions.</div><div>Torn ligaments and tendons, sprains, strains, and fractures are among the most common injuries in many sports. However, it is concerning that nearly one-third of athletes over the age of 16 who were evaluated in a nationwide cohort study experienced a severe injury or polytrauma. Severe head and neck injuries, thoracic, abdominal injuries, and pelvic injuries are the most common causes of traumatic deaths in athletes. This paper aims to provide a comprehensive overview of the anabolic signals that contribute to muscle hypertrophy and discuss their strategic application in prehabilitation and rehabilitation contexts.</div></div><div><h3>Materials and Methods</h3><div>In this narrative review, we examine the primary mechanisms driving muscle growth, including mechanical tension, metabolic stress, and muscle damage, and their molecular mediators such as mTOR signaling, satellite cell activation, and hormonal responses. The review explains how these signals can be manipulated through various training variables, including contraction type, exercise selection, volume, intensity, and frequency. We also discuss the role of nutrition, particularly protein intake and timing, in optimizing the hypertrophic response.</div></div><div><h3>Results</h3><div>The current data shows that, contrary to previous assumptions, high mechanical tension is not mandatory to stimulate muscle growth. Metabolic stress is another effective anabolic signal, which expands the range of training stimuli for pre- and rehabilitation training into an important area.</div></div><div><h3>Conclusion</h3><div>By integrating current scientific understanding of muscle hypertrophy mechanisms with practical clinical considerations, the results equip sports orthopedists and rehabilitation professionals with evidence-based strategies to enhance the effectiveness of their interventions and improve outcomes for athletes and non-athletes across various stages of their careers and recovery processes.</div></div><div><h3>Hintergrund</h3><div>Muskelhypertrophie, die Zunahme der Skelettmuskelmasse, ist eine wichtige adaptive Reaktion auf Widerstandstraining und ein Schlüsselfaktor nicht nur für die sportliche Leistung, sondern auch im klinischen Umfeld, z. B. in der Prä- und Rehabilitation. Usually, this injury affects boys between 13 and 15 years old, who may or may not have previously been diagnosed with Osgood-Schlatter disease. Conservative or surgical treatment can be performed for this injury, depending the findings on physical examination and classification of the fracture.
