The reallocation process is applied to non-administered parenteral chemotherapy preparations at the centralized chemotherapy production unit of the National Institute of Oncology in Rabat. The quantities of drugs and the costs saved by reallocation by doses rounding on the one hand and by including the readjustment of doses in the reallocation process on the other hand were evaluated. The impact of dose readjustment in optimizing reallocation was determined.</p></div><div><h3>Results</h3><p>One hundred and ten record sheets were exploited; the reallocation rate was 86% when the dose readjustment was included in the process and 45% when it was not included. The amount of drugs and cost saved varied from 46,033<!--> <!-->mg to 90,200<!--> <!-->mg and from 14,147 USD to 25,358 USD, respectively, when dose readjustment is included in the reallocation process.</p></div><div><h3>Conclusion</h3><p>Dose readjustment is an opportunity to improve non-administered parenteral anti-neoplastic reallocation. Its implementation is relatively easy. However, it is still a subject of debate within the profession.</p></div><div><h3>Objectif</h3><p>Montrer la place et l’intérêt de l’ajustement des doses dans le processus de réattribution des préparations de chimiothérapie non administrée.</p></div><div><h3>Matériels et méthode</h3><p>Il s’agit d’une étude rétrospective d’une durée de 6 mois. La procédure de réattribution des préparations non administrées a été appliquée aux préparations retournées à l’UCPC et enregistrée par des fiches d’enregistrement. Les quantités de médicaments et les coûts économisés par la réattribution par arrondissement de doses, d’une part, et d’autre part, en incluant le réajustement de doses dans la procédure de réattribution ont été évalués. L’impact du réajustement de la dose dans l’optimisation de la réattribution a été déterminé.</p></div><div><h3>Résultats</h3><p>Cent dix fiches d’enregistrement ont été exploitées, le taux de réattribution était de 86 % lorsque le réajustement de dose était inclus dans la procédure et de 45 % lorsque n’était pas incluse. Quant à la quantité de médicaments et au coût économisés, ils évoluent respectivement de 46,033<!--> <!-->mg à 90,200<!--> <!-->mg et de 14,147 USD à 25,358 USD lorsque le réajustement de dose est inclus dans le processus de réattribution.</p></div><div><h3>Conclusion</h3><p>Le réajustement de la dose est une opportunité d’améliorer la réattribution des antinéoplasiques parentéraux non administrés. Sa mise en place est relativement facile. Bags of matrix admixtures were prepared for validation; electrolytes were added at different concentrations. We studied the linearity, accuracy and precision. The validated method was used to assess the conformity of electrolytes concentrations within the parenteral nutrition bags. The acceptance limit for conformity assessment was set at<!--> <!-->±<!--> <!-->20% of the target concentration.</p></div><div><h3>Results</h3><p>Correlation coefficients of the regression lines were higher than 0.999. The method showed good repeatability and intermediate precision (RSD<!--> <!-->&lt;<!--> <!-->5%). For accuracy evaluation, constant biases have been observed. Therefore, correction factors equal to 0.89 for Sodium and 1.06 for Potassium were applied. Tests on 270 bags showed 80.74% conformity. Following the extension of the mixing time from 15 to 30<!--> <!-->seconds, 69 new bags were controlled and conformity reached 98.55%.</p></div><div><h3>Discussion</h3><p>Non-conformity is increased by increasing the total volume and exceeding the theoretical capacity of the bag. The extension of the mixing time significantly increases the conformity rate.</p></div><div><h3>Conclusion</h3><p>A laboratory analyzer provides a valid and precise method for the determination of electrolytes in parenteral nutrition bags. Le plan blanc est déclenché dans notre établissement. La Pharmacie usage intérieur (PUI) est un des services prioritaires mobilisé automatiquement. L’objectif est de présenter notre retour d’expérience afin d’en dégager les points forts, les points sensibles et de mettre en place des actions d’amélioration.</p></div><div><h3>Matériels et méthode</h3><p>La méthodologie CREX (comité de retour d’expérience) a permis de recueillir les informations sur les évènements selon leur chronologie, de les analyser par rapport aux référentiels internes disponibles. Une réunion de partage associant tous les acteurs présents lors du plan blanc a permis de réaliser ce CREX.</p></div><div><h3>Résultats</h3><p>La PUI a su répondre aux besoins des services accueillant les victimes en appliquant correctement les procédures prédéfinies. La coordination, la communication et l’organisation sont des atouts majeurs pour la réussite de la gestion de notre réponse au plan blanc.</p></div><div><h3>Discussion/Conclusion</h3><p>La formation associant théorie et exercices pratiques a été proposée comme axe d’amélioration pour former davantage de personnel à la gestion du plan blanc, mais également pour maintenir opérationnel le personnel déjà formé.</p></div><div><h3>Introduction</h3><p>On December 11, 2018, a terrorist attack took place at various locations in the Strasbourg Eurometropolis. The emergency action plan called “plan blanc” is launched in our hospital. The hospital pharmacy is one of the priority services requisitioned automatically. The objective, here, is to present our feedback in order to identify its strengths, sensitive points and to implement improvement actions.</p></div><div><h3>Materials and method</h3><p>The CREX (feedback committee) methodology made it possible to collect information on events according to their chronology, to analyse them in relation to the available internal standards. A sharing meeting associating all the players present during the “plan blanc” allowed to carry out this CREX.</p></div><div><h3>Results</h3><p>The hospital pharmacy was able to respond to the needs of services receiving victims by correctly applying predefined procedures. The clinical pharmacy department of the Rabat Ibn Sina hospital has proposed a series of actions that aim to standardize practices, train teams and raise their awareness on environmental hygiene and personal protective equipment. The objective of the current article is to highlight the role of clinical pharmacists as well as the strategies and means implemented to reinforce protection measures and infection control within hospitals in the context of the SARS-COV2 pandemic. When the pandemic broke out, working strategy was declined in 7 steps. First of all, it became necessary to set up an expert made up of clinical pharmacists and their collaborators. The work started by collecting all the documents containing relevant data relating to the coronavirus. The next step was to draft and validate bio-cleaning protocols and the equipment information sheets. Training and simulations were organized with the different care units. During this health crisis, the clinical pharmacy department effectively adapted to the new situation and reinvented itself by putting personal protection and environmental hygiene as absolute priorities in addition to its routine missions.</p></div><div><p>La situation pandémique exceptionnelle actuelle nécessite de la part des professionnels de santé une réorganisation et une adaptation des pratiques. Le service de pharmacie clinique de l’hôpital Ibn Sina de Rabat a proposé une série d’actions qui visent à standardiser les pratiques, former les équipes et les sensibiliser à l’hygiène du milieu et aux équipements de protection individuelle. L’objectif du présent article est de mettre en évidence le rôle des pharmaciens cliniciens ainsi que les stratégies et moyens mis en œuvre pour renforcer les mesures de protection et la lutte contre les infections dans les hôpitaux dans le contexte de la pandémie du SRAS-COV2. Le travail a commencé par la collecte de tous les documents contenant des données pertinentes relatives au coronavirus. L’étape suivante a consisté à rédiger et à valider les protocoles de bio-nettoyage et les fiches d’information sur les équipements. Des formations et des simulations ont été organisées avec les différentes unités de soins. {"title":"Qualité des prescriptions hospitalières de pansements exécutées en ville : quel bilan et comment les améliorer ?","authors":"Marie Hentz ,&nbsp;Sandra Wisniewski ,&nbsp;Martial Lombard ,&nbsp;Aurélie Bros ,&nbsp;Marion Fourtage ,&nbsp;Bénédicte Gourieux","doi":"10.1016/j.phclin.2020.10.006","DOIUrl":"10.1016/j.phclin.2020.10.006","url":null,"abstract":"<div><h3>Objectif</h3><p>L’objectif de notre travail était d’analyser la qualité des prescriptions hospitalières des pansements réalisées dans notre CHU en ciblant certains pansements prescrits et dispensés en ville (PHEV).</p></div><div><h3>Méthode</h3><p>Grâce à la collaboration avec la direction régional du service médical Alsace-Moselle (DRSM), 256 ordonnances anonymisées du 1<sup>er</sup> semestre 2018 ont été analysées, en lien avec des codes LPPR identifiés.</p></div><div><h3>Résultats/discussion</h3><p>Ces ordonnances concernaient 200 patients et 368 références de pansements, soit en moyenne 1,4 types de pansements/ordonnance. Après analyse des prescriptions dispensées en officine de ville, plusieurs anomalies ont été identifiées : près de 50 % des ordonnances présentaient au moins une anomalie d’ordre réglementaire, quasi 12 % au moins une sur la quantité de pansements à dispenser, plus de 40 % au moins une dans la taille du pansement prescrit et près de 30 % une dans le libellé du pansement. Ainsi, seulement 41,80 % des ordonnances (<em>n</em> <!-->=<!--> <!-->107/256) étaient dispensées sans ambiguïté. A contrario, près de 2/3 des ordonnances posaient problème à la dispensation du fait d’ordonnances incomplètes. De telles dispensations pouvaient avoir des impacts tant pour le patient (non continuité des soins) que pour les pharmaciens d’officine (pertes de temps voire financières).</p></div><div><h3>Conclusion</h3><p>Nous travaillons sur une ordonnance type pour améliorer le taux de dispensation univoque en ville.</p></div><div><h3>Objective</h3><p>The objective of our work was to analyze the quality of hospital prescriptions of dressings made in our Hospital University by targeting certain dressings prescribed and dispensed in the city (PHEV).</p></div><div><h3>Method</h3><p>Thanks to the collaboration with the regional direction of the Alsace-Moselle Medical Service (DRSM), 256 anonymized prescriptions from the 1st half of 2018 were analyzed, in relation to identified LPPR codes.</p></div><div><h3>Results/discussion</h3><p>These prescriptions concerned 200 patients and 368 references of dressings, i.e. an average of 1.4 types of dressing/order. After analysis of the prescriptions dispensed in city pharmacies, several anomalies were identified: almost 50% of the prescriptions had at least one regulatory anomaly, almost 12% had at least one on the quantity of dressings to be dispensed, more than 40% had at least one in the size of the dressing prescribed and almost 30% had at least one in the wording of the dressing. Thus, only 41.80% of prescriptions (<em>n</em> <!-->=<!--> <!-->107/256) were dispensed unambiguously. On the other hand, nearly 2/3 of the prescriptions were problematic to dispense because of incomplete prescriptions. Entre juin 1990 et juin 2020, 123 numéros totalisant 347 articles et quelques 1500 pages ont été publiés. La majorité des informations contenues dans le BIP portent sur le médicament (80 % des articles) et son bon usage (38 %). Avec la mise en place de la Dispensation avec Délivrance Nominative (DDN) et la diffusion des campagnes relatives au bon usage des antibiotiques, les thèmes des articles ont évolué au fil des années. Ainsi, le BIP reflète les changements au sein de l’établissement et atteste du rôle central du pharmacien en matière de bon usage du médicament.</p></div><div><p>Since June 1990, the hospital pharmacy of Saint Dié des Vosges publishes its newsletter called BIP. Between June 1990 and June 2020, 123 editions totaling 347 articles and more than 1500 pages have been published. Most of articles from BIP are about medication (80%) and its proper use (38%). With the implementation of the Unit Dose Distribution (UDD) and the dissemination of campaigns relating to antibiotics, themes of articles have changed over the years.
