Younis","doi":"10.31482/mmsl.2022.001","DOIUrl":"https://doi.org/10.31482/mmsl.2022.001","url":null,"abstract":"Introduction: “Ankylosing spondylitis (AS)” is an inflammatory disorder that affects the axial skeleton, peripheral joints, as well as entheses, resulting in significant disability. “Tumor necrosis factor-α (TNF-α)” inhibitors are regarded to be a helpful treatment for patients with active “AS”. This study aimed to investigate the effectiveness and response rate of “etanercept” in a group of patients with “ankylosing spondylitis” in Mosul, Iraq. Methods: A prospective, “open-labeled”, non-randomized 12 weeks study was undertaken on 43 participants with “ankylosing spondylitis” in the “Rheumatology unit” of “Ibn Sina Teaching Hospital”, and the diagnosis was made using the “modified New York criteria”. Participants were assessed at the outset of the study, week 4, and week 12 after receiving etanercept 50mg subcutaneously once weekly. The “Bath Ankylosing Spondylitis Disease Activity Index (BASDAI)” was utilized to assess disease activity, while the “Bath Ankylosing Spondylitis Function Index” was utilized to assess functional status (BASFI) at baseline, 4 weeks, and 12 weeks. BASDAI 50 was used to assess the response rate. Results: Mean patients’ age was “36.6±8.47” years; men accounted for “90.7 %” of the cases, with the mean disease length being “9.6±5.90” years. A marked decrease in BASFI and BASDAI was found four and twelve weeks after commencing treatment compared to baseline (p=0.000). “BASDAI 50 %” response was fulfilled by 42.5 % of the participants after 4 weeks and by 65%% after 12 weeks of therapy with “etanercept”. There was a marked fall in the mean ESR and CRP after four and twelve weeks of “etanercept” therapy. The study assesses the phytochemical, antioxidant (using a DPPH radical scavenging assay) and antimicrobial activities and identifies minimum inhibitor concentrations of Citrus sinensis (orange), Citrus Limonum (lemon), and Citrus reticulata (tangerine) extracts and their silver nanoparticles. Fourier Transform Infrared (FTIR) spectroscopy and scanning electron microscopy (SEM) was used to analyze the produced AgNPs. The synthesized AgNPs have a size of less than 100 nm according to SEM examination. Their DPPH radical scavenging activity and reducing power increased in a dose-dependent way that was more than that of their aqueous and alcoholic extracts. In comparison to Staphylococcus aureus and Candida albicans, silver nanoparticles were found to be more efficient towards Escherichia coli. Their activities were increased with increasing dosage. Whereas, no inhibition zones were conducted with the examined plain citrus peel extracts. Materials and Methods: This study included samples of local and imported nuts and dried fruits. Results: The study showed different genera of contaminant fungi such as Alternaria, Penicillium, and Aspergillus species which are very common fungi associated with samples. The most fungal contamination was found in pistachio and black raisins, followed by balahsisi, taffy raisins, yellow raisins, dried apricot and peanut with shell. A total of nine species of Aspergillus were isolated on (potato dextrose agar, malt extract agar and dichloran rose bengal agar) media A. niger, A. flavus, A. fumigatus, A. tamarii, A. aculeatus, A. parasiticus, A. oryzae, A. japonicus, A. ochraceus were detected from all types of nuts and dried fruits samples. The most frequent species were A. niger followed by A. flavus and A. fumigates with percentage frequencies of 45.2%, 20.3% and 13.9% respectively. The ability of toxicogenic fungi to produce AflatoxinB1 by ELISA test of Sunlong corporation in nuts and dried fruits samples ranged from 94.8 to 136.4 μg/kg, with the highest levels of ability to produce AflatoxinB1 in nuts samples (peanut with shell, sunflower seeds and walnut) at levels 135.8, 129.0, and 128.9 μg/kg respectively, while in dried fruits samples the highest ability to produce AflatoxinB1in dried apricot, taffy raisins and yellow raisins was at levels136.4,123.1 and 118.0 μg/kg, respectively. Studenti, kteří jsou vystaveni vysoké míře stresu a zároveň nedokáží využít vhodné strategie zvládání těchto zátěžových situací, bývají ohroženi rozvojem syndromu vyhoření již během studia. Předmětem této práce bylo prozkoumat blíže otázku stresu, a to v souvislosti se syndromem vyhoření, s důrazem na rozvoj tohoto jevu ve skupině pregraduálních studentů Ústavu sociální práce v Hradci Králové. Stejně jako posouzení souvislostí využití strategie zvládání stresu versus riziko vývoje syndromu vyhoření u studentů. Vzhledem k počtu respondentů není záměrem této studie zobecnění nalezených výsledků, ale snaha o hlubší pohled do této problematiky a aplikace získaných poznatků do výuky studentů oboru sociální práce. Pro vyhodnocení strategií zvládání stresu byl využit dotazník SVF 78, syndrom vyhoření byl posouzen dotazníkem Burnout Measure. Nejčastěji používanou strategií zvládání stresu byla perseverace. Řadíme ji do skupiny negativních strategií, jelikož přímo zhoršuje a prodlužuje stresovou reakci organismu na prožívající zátěž. V otázce vztahu mezi strategiemi zvládání a syndromem vyhoření se ukázala pozitivní korelace u negativních strategií zvládání stresu, jako jsou úniková tendence, perseverace, rezignace nebo sebeobviňování. Pozitivní korelace v těchto parametrech znamená, že studenti, kteří využívají tyto strategie zvládání stresu, mají větší míru ohrožení syndromem vyhoření. Negativní korelace byla nalezena u pozitivních strategií, mezi které patří odklon, náhradní uspokojení, kontrola situace, pozitivní sebeuspokojení. Classical cancer therapies include chemotherapy, radiation therapy, and immunotherapy. A typical treatment is chemotherapy, which delivers cytotoxic medications to patients to suppress the uncontrolled growth of cancerous cells. Conventional oral medication has a number of drawbacks, including a lack of selectivity, cytotoxicity, and multi-drug resistance, all of which offer significant obstacles to effective cancer treatment. Multidrug resistance (MDR) remains a major challenge for effective cancer chemotherapeutic interventions. The advent of nanotechnology approach has developed the field of tumor diagnosis and treatment. Cancer nanotechnology enables direct access to tumor cells, resulting in enhanced drug localization and cellular uptake. Since the early 1990’s, several solid lipid nanoparticle (SLN) or SLN-based systems for the delivery of cytotoxic drugs have been manufactured and tested with success. High shear homogenization, microemulsionbased SLN, Supercritical fluid technology, spray drying, and solvent emulsification/evaporation methods can all be used to successfully formulate SLN.There is great potential to enhance cancer chemotherapy by incorporating it into a solid lipid nanoparticle (SLN) drug delivery system. Improving tumor diffusivity, improvement of body distribution, and inhibiting MDR are the main attributes. Decreased 25OHD concentrations have been linked to insulin resistance, ovulatory and period abnormalities, lower pregnancy outcomes, and other indications of PCOS, according to epidemiological studies. There might be some, but inadequate, research that vitamin D supplements can help women with PCOS with menstruation disruption and insulin sensitivity. Vitamin D inadequacy may bestow to the exacerbation of PCOS, and vitamin D intake may be helpful in the treatment of this condition. However, the present evidence is insufficient, and further experimental investigations are in need to establish vitamin D supplementation's potential advantages in this population. Many investigators claimed a pertinency between vitamin D concentrations and PCOS, nevertheless, these researches bear rattle factors of conjoined with other disorders or ailments, such as obesity, diabetes, and hirsutism. In the view of this, the present review detailed the role of vitamin D inadequacy and related biomolecule abnormalities, including sex hormones, and insulin and we listed their psychological and cardiovascular encounter.
