However, these environments face ethical-organizational challenges that significantly affect the physical and mental health of employees. To this end, this review analyzed the literature on ethical-organizational challenges faced by these institutions and their relationship with employee health and illness. A systematic review of studies published between 2010 and 2024 was conducted, including studies that specifically addressed the relationship between ethical-organizational challenges and health in the context of federal institutions of higher education. The main challenges included work overload, pressure for academic productivity, moral harassment, lack of institutional support, and lack of transparency in management. These conditions increase stress, burnout, and mental disorders among employees. However, organizational practices that promote ethics, equity, and professional appreciation contribute to greater engagement and better occupational health conditions. This report describes the case of a 48-year-old immunocompromised physician identified by Occupational Health Services during contact tracing following high-risk exposure to a tuberculosis case. The patient presented with fever, night sweats, and productive cough, and was referred to a Pulmonary Diagnostic Center for further evaluation. After an appropriate investigation, a diagnosis of lymph node tuberculosis was established. Antitubercular therapy was initiated without complications, and the patient returned to work following clinical and radiological improvement. An occupational health assessment was conducted, and a formal report of occupational disease was filed. The sequelae often restrict performance of daily activities, requiring the help of caregivers.</p><p><strong>Objectives: </strong>To assess the mental health and work overload of caregivers of stroke survivors based on ergonomics activity analysis.</p><p><strong>Methods: </strong>This mixed methods study included 76 informal caregivers, 11 of whom also participated in a qualitative study phase. The Self-Reported Questionnaire-20 was used to investigate mental distress, while the Zarit Burden Interview scale was used to assess work overload. The qualitative phase included an interview.</p><p><strong>Results: </strong>The prevalences of mental distress and work overload were 44.7% and 64.5%, respectively, and they were strongly correlated (r = 0.707, p < 0.001). Caregiver distress was associated with comorbidities (p = 0.003) and illiterate patients (p = 0.04). No variables were associated with work overload. Qualitative ergonomics analysis resulted in two categories: (1) description of the work routine and (2) the physical, mental, and emotional exhaustion of caregivers. {"title":"Risk factors for cardiovascular morbidity among municipal public servants.","authors":"Giovanna Vallim Jorgetto, Fátima Livorato, Sandra Soares Mendes, Edna de Fátima Medeiros Neves, Jacqueline Bentitte Candido","doi":"10.47626/1679-4435-2025-1407","DOIUrl":"10.47626/1679-4435-2025-1407","url":null,"abstract":"<p><strong>Introduction: </strong>Cardiovascular diseases are the leading cause of global mortality and significantly affect municipal public servants, particularly due to occupational stress and long working hours.</p><p><strong>Objectives: </strong>To assess cardiovascular risk factors diseases among public servants in a medium-sized municipality located in the eastern region of the state of São Paulo, Brazil.</p><p><strong>Methods: </strong>This was a cross-sectional study conducted in partnership with the Municipal Government of São João da Boa Vista, state of São Paulo, and the Centro Universitário das Faculdades Associadas de Ensino. The sample included 191 employees from general and specialized service sectors, with data collected in August 2022. The variables analyzed included demographic (age, sex, and educational attainment) and clinical data (physical inactivity, smoking, alcohol consumption, body mass index, waist circumference, and blood pressure). Statistical analyses were performed using tests with a significance level of 5% and 95%CI.</p><p><strong>Results: </strong>A higher prevalence of cardiovascular risk factors was observed among women, with statistically significant association for increased waist circumference (p < 0.001) and physical inactivity (p = 0.034). The main identified factors included obesity (40.8%), physical inactivity (68.0%), smoking (14.1%), and alcohol consumption (40.8%).</p><p><strong>Conclusions: </strong>Public servants showed multiple modifiable risk factors for cardiovascular diseases, notably obesity and physical inactivity. In-depth interviews were conducted with nine nursing professionals. The analysis was carried out in parallel with the collection of information throughout the research with the support of the qualitative data research software Atlas.ti.</p><p><strong>Results: </strong>The two central categories of this research were working conditions and quality of care. A dialogic relationship was observed between the two central categories of this research (working conditions and quality of care), in which good working conditions configured good quality of care. For the analysis of the basic causes of the occupational accidents reported, personal and work-related factors were identified, consolidating the data through Pareto diagrams. Subsequently, actions were taken on the source, the environment, and the worker.</p><p><strong>Experience report: </strong>In personal factors, 80% of the causes were due to insufficient practices and lack of attention. In the work-related factors, 45.2% of the accidents corresponded to unspecified work factors, i.e., there was not enough information to identify them. This involved analyzing the company's induction and training practices. {"title":"Factors related to the prevalence of pregnancy complications among subsistence workers in Medellín - Colombia.","authors":"María Osley Garzón Duque, Paula Andrea Uribe Cardenas, Fabio León Rodríguez-Ospina, Manuela Jiménez Cifuentes, Valentina Zapata Paz","doi":"10.47626/1679-4435-2023-1143","DOIUrl":"10.47626/1679-4435-2023-1143","url":null,"abstract":"<p><strong>Introduction: </strong>For the informal female workers on the streets and sidewalks of big cities, called \"venteras,\" experiencing a pregnancy in a healthy environment is an ideal difficult to achieve.</p><p><strong>Objectives: </strong>To determine the prevalence of pregnancy complications and their relationship with health, working, and non-working conditions among the \"venteras\" in downtown Medellín.</p><p><strong>Methods: </strong>This cross-sectional study utilized primary data collected through a survey administered to 291 workers. An assisted survey was conducted after obtaining informed consent from the participants. The variables included self-reported pregnancy complications, sociodemographic data, work-related conditions, home responsibilities, environmental factors, support and solidarity, and comorbidities. Descriptive and bivariate analyses were performed using chi-square tests and prevalence ratios (PR). A multivariate analysis was conducted using binomial regression. Statistical tests were carried out with 95% confidence intervals (95% CI) and a 5% margin of error.</p><p><strong>Results: </strong>The workers' mean age was 48 years, with a mean education of < 6 years; 39% had more than 2 dependents. Additionally, 53.6% reported feeling discriminated against by authorities, and 23.7% by their peers; 21.6% reported severe family dysfunction, and 15.5% showed moderate-to-severe depressive symptoms. The prevalence of complications during pregnancy was 36.9%. {"title":"Hormonal responses to a stress load and state anxiety, mood, tiredness, and recovery in Portuguese police cadets.","authors":"Maria Rosário Abrantes, Raquel Barreto Madeira, Luís Fernandes Monteiro, Catarina N Matias, Luís Miguel Massuça","doi":"10.47626/1679-4435-2023-1161","DOIUrl":"10.47626/1679-4435-2023-1161","url":null,"abstract":"<p><strong>Introduction: </strong>Police officers are repeatedly exposed to conditions and incidents unique to their work.</p><p><strong>Objectives: </strong>To describe the hormonal response to a stress load (circuit fitness test) in police cadets, and to verify the association of hormone responses (cortisol and testosterone) with state anxiety, mood, tiredness, and recovery.</p><p><strong>Methods: </strong>This is an analytical, observational, cross-sectional study with a quantitative approach. A total of 31 police cadets (all male; age 21.0 ± 4.4 years) were evaluated on February 4, 2022, at Instituto Superior de Ciências Policiais e Segurança Interna, Lisbon, Portugal, in four dimensions: morphological, fitness for police work (simulated circuit), hormonal (cortisol and testosterone responses), and psychological (anxiety, mood, stress, and recovery).</p><p><strong>Results: </strong>We observed differences in cortisol concentration upon awakening and the reference value; a decrease in cortisol and testosterone concentrations until the time of fitness testing; and increases in cortisol and testosterone concentrations in response to completing the fitness test. Regarding the hormone response in anticipation of the fitness test, there is a direct correlation of cortisol with hostility and social stress and an inverse correlation of testosterone with conflicts/pressure, sleep quality, emotional exhaustion, and the sum of the recovery and stress scales. Regarding the hormone response to actually performing the task, a positive correlation was found between cortisol and personal acceptance and self-regulation.</p><p><strong>Conclusions: </strong>These findings suggest that the police cadets exhibited a healthy response to the proposed stress situation. {"title":"Musculoskeletal pain associated with common mental disorders among bus terminal workers in Brazil.","authors":"Sabrina Gabriele Maia Oliveira Rocha, Hermano Alexandre Lima Rocha, Sâmia Graciele Maia Oliveira Giacomini, Anamerinda de Oliveira Diaz, Luciano Lima Correia","doi":"10.47626/1679-4435-2024-1301","DOIUrl":"10.47626/1679-4435-2024-1301","url":null,"abstract":"<p><strong>Introduction: </strong>Common mental disorders affect almost one billion people worldwide and are among the main causes of work absenteeism, representing a high cost for society.</p><p><strong>Objectives: </strong>We aimed to identify factors associated with symptoms of common mental disorders among workers in urban bus terminals in Fortaleza, Brazil.</p><p><strong>Methods: </strong>This analytical cross-sectional study was based on data from periodical and pre-admission medical examinations performed in 2022. We investigated mental disorder symptoms with the Depression Anxiety & Stress Scales 21, correlating associated factors through multivariate analysis.</p><p><strong>Results: </strong>The prevalence of stress and anxiety was about 11%, while that of depression was 8.3%. Self-reported leg pain was associated with depression, anxiety, and stress. Smoking was associated with depression (adjusted odds ratio 4.96; p = 0.002) and stress (adjusted odds ratio 4.08; p = 0.003).
