{"title":"The relationship between workplace violence, anxiety, and depression in nurses during the COVID-19 pandemic.","authors":"Ana Paula Nogueira de Magalhães, Diego de Oliveira Souza, Sónia Mafalda Pereira Ribeiro","doi":"10.47626/1679-4435-2024-1337","DOIUrl":"10.47626/1679-4435-2024-1337","url":null,"abstract":"<p><strong>Introduction: </strong>Workplace violence severely affects nurses, and the COVID-19 pandemic has intensified this issue, resulting in physical and mental illness, which seriously affects their work and lives.</p><p><strong>Objectives: </strong>To analyze the relationship between workplace violence, anxiety, and depression among front line nurses in the fight against COVID-19.</p><p><strong>Methods: </strong>This mixed-methods study simultaneously collected qualitative and quantitative data in three municipalities in Alagoas, Brazil, between January and June 2022. The qualitative data were derived from interviews with six nurses, using the thematic oral history technique. The quantitative data were derived from a questionnaire administered to 131 clinical and practical nurses.</p><p><strong>Results: </strong>The oral histories indicated anxiety-related feelings, such as fear, nervousness, worry, and panic, which made the workers vulnerable to violence. According to the quantitative data, 53.4% (n = 70) of nurses experienced violence, primarily psychological (n = 69; 52.7%). Anxiety and depression symptoms were present in 21.4% (n = 28) and 30.5% (n = 40) of the nurses, respectively, and violence was significantly associated with both anxiety (p = 0.003) and depression (p = 0.004).</p><p><strong>Conclusions: </strong>The results show that nurses suffer a high prevalence of violence. {"title":"Intra- and extra-work psychosocial factors in physicians performing mandatory social service in Nariño, Colombia.","authors":"Angie Ximena Ortiz-Chamorro","doi":"10.47626/1679-4435-2025-1429","DOIUrl":"10.47626/1679-4435-2025-1429","url":null,"abstract":"<p><strong>Introduction: </strong>Psychosocial risk among mandatory social service physicians is a relevant concern due to work demands and adverse conditions.</p><p><strong>Objectives: </strong>To identify intra- and extra-work psychosocial factors affecting physicians who completed their mandatory social service in different regions of the department of Nariño, Colombia, during the COVID-19 pandemic.</p><p><strong>Methods: </strong>This was an observational, descriptive, cross-sectional study based on primary data sources. A total of 104 mandatory social service physicians from Nariño, southwestern Colombia, were surveyed using the Intra- and Extra-Work Psychosocial Risk Questionnaires from the psychosocial risk batery developed by the Ministry of Health and Social Protection of Colombia. Absolute and relative frequencies were calculated for each variable of interest. Total risk scores, as well as intra- and extra-work risk dimensions, were analyzed and compared with previous studies.</p><p><strong>Results: </strong>Overall, 79.8% of physicians were classified as being at very high psychosocial risk, while 78.8% were at very high risk for intra-work factors, with the most affected dimensions being \"leadership,\" \"job demands,\" and \"rewards\". Additionally, 39.4% were at very high risk for extra-work factors, with \"influence of the extra-work environment on work\" being the most affected dimension. The \"family relationships\" dimension showed a low risk in 49% of participants.</p><p><strong>Conclusions: </strong>The high prevalence of psychosocial risk, especially in emotional demands and leadership, highlights the urgent need for implementation of intervention strategies within an epidemiological surveillance system. A structured method to assist in this process could have significant practical applicability.</p><p><strong>Objectives: </strong>To develop an analytical method to support better decision-making regarding the establishment of a causal relationship between health status and occupational activities.</p><p><strong>Methods: </strong>The Design Science Research methodology was employed. In the knowledge exploration phase, the existence of analytical methods for determining a causal relationship between illness and work was investigated, along with multicriteria decision analysis techniques that could support the development of a new method. The ELECTRE TRI classification technique was selected as the basis for the analytical framework. To assess operability of the classification method, 27 simulated problem situations were analyzed. The results were found to be easy to analyze, classified as Certain, Extremely Likely, Very Likely, Likely, Plausible, Slightly Plausible, Slightly Likely, Unlikely, and Impossible. These changes have contributed to greater job insecurity and an increased prevalence of work-related illnesses, particularly mental health disorders and musculoskeletal conditions, which are among the leading causes of occupational absenteeism. The World Health Organization recognizes psychosocial factors at work as key determinants of occupational stress and occupational health outcomes. Inadequate management of these factors is associated with presenteeism, absenteeism, and adverse mental and physical health effects. In Brazil, although progress has been made, such as the incorporation of psychosocial factors into Labor Regulatory Standard-1, the issue remains insufficiently addressed. Assessments that rely exclusively on quantitative methodologies often fail to capture the subjective dimensions of workers' experiences. This opinion article proposes a mixed-methods approach, integrating surveys and qualitative interviews, to comprehensively assess psychosocial factors in the workplace. A holistic evaluation, incorporating both objective and subjective aspects, is essential for an accurate understanding of these factors. Furthermore, the findings should inform the development of management strategies aimed at mitigating or eliminating workplace stressors that contribute to work-related illnesses. Informal workers and the unemployed - already vulnerable before the pandemic - had greater difficulty adopting social distancing. This study analyzes occupational data from patients at a referral hospital for COVID-19 in the city of Rio de Janeiro, Brazil, considering the scarcity of information on workers' health during the pandemic.</p><p><strong>Objectives: </strong>This research sought (i) to investigate the relationships between the type of employment relationship and deaths and (ii) to describe the most frequent occupations according to the type of employment relationship and the percentage of deaths.</p><p><strong>Methods: </strong>One thousand four hundred and four medical records of hospitalizations that occurred between August 2021 and November 2021 were analyzed.</p><p><strong>Results: </strong>The most represented professions were vendors/sellers, masons/bricklayers, doormen, security guards, drivers, and rideshare drivers. Compared to formal workers, the relative risk of death was 77% higher among homemakers, after adjusting for age. The relative risk of death was 11 and 29% higher among informal workers and the unemployed, respectively, compared to formal workers, but this difference was not statistically significant. Methods: Forty-seven nurses working at Hospital Universitário do Oeste do Paraná, from the three shifts, were analyzed. Burnout syndrome was evaluated using the Maslach Burnout Inventory questionnaire, and sleep quality was evaluated using the Pittsburgh Sleep Quality Index. Statistical analysis was performed with the Mann-Whitney Test and the Spearman's correlation coefficient.</p><p><strong>Results: </strong>Only three nurses (6.00%) met burnout criteria, but 59.57% had high scores for emotional exhaustion. In relation to sleep quality, eight (17.00%) presented good sleep quality. The correlation between sleep quality and burnout syndrome was significant in the emotional exhaustion dimension (p = 0.0244), being strongly linked to poor sleep quality. There were no differences between working the day and night shifts and the quality of sleep. This logic is now revered as a new ethos, that of competitiveness between workers, which inevitably leads to illness through the daily overtaxing of human limits. Herein, we discuss burnout syndrome from a sociogenic perspective, i.e., as a severe consequence of contemporary work processes on worker health. The World Health Organization's recognition of burnout syndrome as a work-related mental illness opened debate with global repercussions. As information about the syndrome has disseminated, certain aspects have been trivialized, with analysis tending to focus on personal weakness, which individualizes the treatment and prevention of symptoms. This type of analysis, although important for highlighting the debate, overlooks the socio-historical and psychosocial aspects of burnout. These factors are also associated with musculoskeletal disorders and mental and behavioral issues among workers. Objectives: To describe and analyze the relationships between management characteristics and psychosocial factors at work, as perceived by industrial workers, and their impact on increased work-related stress.</p><p><strong>Methods: </strong>This is a case study with a qualitative approach, using in-depth interviews and participant observation during fieldwork.</p><p><strong>Results: </strong>The 5 companies studied predominantly employed a Taylorist production model. Rigid hierarchical management was observed, as well as encouragement of competitiveness among workers, poorly trained leadership, and insufficient investment in training and Occupational Health and Safety. Additionally, there was a disregard for psychosocial factors that can trigger stress and illness.</p><p><strong>Conclusions: </strong>Our results demonstrated that, irrespective of the specific industrial activity, the primary psychosocial factors at work were overwork, lack of autonomy and control over tasks, lack of social support, lack of career planning and recognition, poor communication, and psychological abuse, including moral harassment. This study aimed to assess the psychosocial factors related to the work of nursing staff in intensive care units, as reported in the literature. An integrative literature review was conducted in December 2023 using the Base de Dados em Enfermagem, Virtual Health Library, Literatura Latino-Americana em Ciências da Saúde, PubMed, Index Psicologia - Periódicos, BMC Medical Ethics, Web of Science, Cumulative Index to Nursing and Allied Health Literature, and Scopus. The guiding question was: \"How do psychosocial factors contribute to the illness of nursing staff in intensive care units?\" A total of 21 articles published between 2000 and 2021 were included, with a predominance of Brazilian studies using qualitative methodologies. The findings revealed a phenomenon previously overlooked or not recognized as a significant issue, despite the historical presence of psychosocial factors in the nursing profession. The reviewed studies indicate psychosocial risks directly impact the health of nursing staff. There is a need for further research, particularly intervention studies, longitudinal studies, and causal relationship analyses. Data collection instruments were administered via Google Forms from August to September 2023.</p><p><strong>Results: </strong>Our analysis highlighted medium psychosocial risks arising from the Division of Tasks, Lack of Recognition, and Psychological, Social, and Physical Harms factors. The factors that require immediate attention are Social Division of Labor and Mental Exhaustion. There is a clear need for interventions focused on enhancing communication and relationships between team members and managers, preventing prejudice, discrimination, and abuse in the workplace, improving work organization and overall working conditions, and prioritizing the enhancement of free/leisure time, sleep, and food intake.</p><p><strong>Conclusions: </strong>It is essential to develop initiatives focused on health care, prevention, and health promotion by identifying and analyzing potential risks and harms affecting the health of health care workers.
