This study in-vestigated the diagnostic characteristics of two geriatric screening tools, G8 （Geriatric 8） and fTRST （Flemish version of Triage Risk Screening Tool）, in older patients with head and neck cancer and the prognostic value of these screening tools for overall survival, treatment completion and complications. The subjects were 30 patients over 70 years old who were evaluated by G8 and fTRST between March 2017 and October 2019. A Cox regression analysis with G8 and fTRST as the independent variables was conducted for the OS analysis. Univariate analysis showed that only G8, in which we set the cut-off value to 14, was a statistically significant prognostic predictor （hazard ratio 9.64, p ＝ 0.036）. Pembrolizumab as first-line treatment for recurrent and metastatic head and neck carcinoma was reported in 2019 and it is predicted that the first-line treatment will change. Many immune-related adverse events were reported by immune checkpoint inhibitors. Therefore, we should start administering steroid to control inflam-mation and severe IrAE. In addition, some patients should be put on steroid for a long time because of anti-inflammation. Furthermore, when cisplatin, which is one of the key drugs for head and neck cancer, is adminis-tered, steroid can prevent delayed nausea. An extremely rare location of distant metastasis is the pancreas, as reported in only 16 published cases. In this article we report a case with pancreatic metastases from thyroid papillary carcinoma. A 71-year-old woman underwent total thyroidectomy for thyroid papillary carcinoma with lung metastases. While the patient was undergoing TSH suppression therapy, the thyroid cancer spread to the pancreas. She began receiving Lenvatinib, a molecular-targeted drug, which was very effective, and the lung and pancreatic metastases diminished during 15 months. In rare cases like pancreatic metastasis from differentiated thyroid cancer, the standard of care is unclear.
