Gastric adenocarcinoma rarely metastasizes to the bladder, and these metastases typically exhibit a CK7-positive, CK20-negative phenotype. To date, there have been no reports of gastric cancer metastasis to the bladder that displays a CK7-negative and CK20-positive profile. Miniature Endoscopic Combined Intrarenal Surgery (mini-ECIRS) with low-wattage Thulium Fiber Laser (TFL) enables simultaneous stone clearance and precise endopyelotomy. We report a case of a 51-year-old woman with recurrent UPJO and renal stones treated using mini-ECIRS and TFL-assisted retrograde endopyelotomy. Dual endoscopic access allowed effective stone fragmentation and accurate UPJ incision. Operative time was 75 min, with no complications. Percutaneous nerve evaluation (PNE) is used to assess patient suitability for permanent SNM through temporary lead placement. While rare, complications such as hematomas can occur. This case report describes an acute pre-sacral hematoma following PNE lead placement in the absence of typical risk factors. We present a case of CIS involving the entire urinary tract in a renal transplant patient who underwent a complete urinary tract extirpation (CUTE) for BCG-refractory CIS of the bladder. It has been hypothesized that THP contributes to the maintenance of urinary homeostasis. We report a case of a 71-year-old male with a history of renal cell carcinoma treated with partial nephrectomy in 2017. He represented 8 years later with an enlarging, <u>enhancing</u> renal mass concerning for recurrence. A radical nephrectomy was performed and revealed a benign specimen with a positive PAS stain, consistent with THP. Diagnosis was confirmed through histopathology, immunohistochemistry, and cytogenetics. The patient underwent cytoreductive nephrectomy followed by adjuvant chemotherapy. A systematic review on the management of urethral strictures in this patient population was conducted. Only one case of successful buccal mucosa graft urethroplasty in a 46,XX male was found. Upon admission, elevated urinary vanillylmandelic acid and homovanillic acid levels were noted. Imaging revealed an 8.2 cm left adrenal mass, initially suspected as pheochromocytoma. Laparoscopic resection was performed. Pathology confirmed adrenal cavernous hemangioma (CD31/CD34+). The preoperative misdiagnosis resulted from the tumor's clinical features (hypertension, elevated catecholamine metabolites) and imaging characteristics mimicking pheochromocytoma.</div></div><div><h3>Conclusion</h3><div>Adrenal cavernous hemangioma is susceptible to preoperative misdiagnosis. Urgent retroperitoneal biopsy confirmed seminoma and, after the patient declined neurosurgical intervention, systemic chemotherapy was promptly initiated prior to delayed orchidectomy. Post-chemotherapy orchidectomy histology revealed minimal viable tumour and serial imaging showed a residual retroperitoneal mass with low-grade uptake on FDG-PET. The mass was deemed unresectable, and surveillance was favoured over immediate salvage.
