Method: Retrospective analytical study of the radiological findings on X-ray and CT on a series of patients who attended the emergency department on suspicion of COVID-19 at university hospitals dependent of our institution in the period between March 1 and May 31, 2020. Results: During the study period, 127 cases of COVID-19 were detected in our institution. X-rays were performed in 57 (45%) of them, 46 (80%) of which did not show pathological findings. In total, 8 (14%)manifested as patchy ground glass opacities, and 3 (5%), in association with consolidations. The distribution was predominantly bilateral and in ground glass opacities. Two patients (18%) showed no pathological findings. As atypical findings, pleural effusion was evident in two (18%) patients, and pneumothorax, in one (9%). The most frequent location is on liver’s tissue, manifesting itself as an incidental finding when it is asymptomatic or with symptoms, as biliary colic. The Pan American Health Organization and World Health Organization (WHO) support Gharbi and WHO’s classification, allowing to estimate the stage of the hydatid cyst. Method: From a high complexity cardiovascular institute, 39 patients were selected and chest X-rays were perfor-med from August 1, 2018 to February 1, 2019. Selection criteria involved those who underwent computed tomography angio-graphy of thoracic aorta for clinical suspicion of AD. results: Within the sample, 72% were men (mean 63 years old), with hypertension as a most common risk factor. 43% of the patient had AD, 76% were type B. The mean maximum mediastinal width showed a difference of 1.5 cm between patients with AD and those without it. In contrast, the difference was 2 cm for the left mediastinal width. Maximum mediastinal width and left mediastinal width were statistically significant signs. Left mediastinal width presented good diagnostic capacity (area under the curve: 0.84). Cut-off point of 5.39 cm for the left mediastinal width was calculated (S: 82%; E: 77%). Finally, widening of the aortic knob, ascending/descending aorta or asymmetry between both showed to be statistically significant signs. Conclusions: Radiological signs for the diagnosis of AD was vali-dated. Pardo","doi":"10.1055/s-0040-1721347","DOIUrl":"https://doi.org/10.1055/s-0040-1721347","url":null,"abstract":"\u0000 Objetivo Evaluar retrospectivamente la capacidad diagnóstica de la elastografía cuantitativa para determinar la posibilidad de malignidad o benignidad de los nódulos tiroideos benignos y malignos, y determinar su utilidad para así decidir qué nódulos deberán ser sometidos a punción aspirativa con aguja fina (PAAF).\u0000 Pacientes y métodos Se analizaron 203 nódulos tiroideos de 195 pacientes remitidos por el Servicio de Endocrinología para estudio citológico durante el año 2018. A todos ellos se les realizó ecografía convencional, elastografía cuantitativa y PAAF. Se realizó un análisis estadístico mediante regresión logística, que pone en relación la probabilidad de que un nódulo sea sospecho de malignidad y el valor de la elasticidad medido en kilopascales (kPa) y el ratio elastográfico.\u0000 Resultados Existe una relación significativa y positiva entre el resultado citológico de Bethesda V/VI y los kPas/ratio elastográfico. {"title":"La elastosonografía cuantitativa: una herramienta para evaluar el riesgo de malignidad del nódulo tiroideo","authors":"S. M. Batallés","doi":"10.1055/s-0041-1740535","DOIUrl":"https://doi.org/10.1055/s-0041-1740535","url":null,"abstract":"La elastografía cuantitativa (o “shear wave elastography” [SWE]) representa un importante avance en la evaluación ecográfica de la elasticidad del nódulo tiroideo. Tal como expresan Uliaque Franco y col.1 en “Elastografía cuantitativa en la evaluación de nódulos tiroideos”, publicado en el presente número, con esta técnica es posible medir la elasticidad de los tejidos independientemente de la presión aplicadaporel operador, lo cual disminuye lavariabilidad intra e interoperador respecto a la elastografía cualitativa. La técnica utiliza ondas de corte inducidas por fuerza de radiación acústica centrada en un haz ultrasónico. La compresión provocada en el tejido es altamente localizada, ya que las ondas de corte inducidas son atenuadas por una región limitada cercana al centro del haz del ultrasonido. La onda final que recibe el transductor es la propagación de las ondas de corte, las cuales tienen relación con la dureza del tejido. Para medir la velocidad de las ondas de corte, el transductor debe mensurar la respuesta de al menos dos puntos y, mediante algoritmos, obtener la velocidad, teniendo el desplazamiento y el tiempo. Al reconstruir la onda de propagación de velocidad del tejido en la región desplazada y utilizando algoritmos de correlación cruzada, se obtienen imágenes en tiempo real, en escala de colores y la elasticidad de la región de interés en unidades de presión (Kilopascales [kPa]) o en unidades de velocidad (m/seg, cm/seg).2 Numerosos trabajos científicos han avalado la utilidad de la técnica para diferenciar los nódulos más duros de los más elásticos, considerando que la rigidez aumenta la sospecha de malignidad de los nódulos en cuestión.3–6 En el análisis de la bibliografía se hallan rangos variables de sensibilidad y especificidad del método que oscilan entre 57 y 86% y entre 85 y 93,9%, respectivamente (►Tabla 1).7 Uliaque Franco y col.1 expresan en su investigación que gran parte de la literatura sugiere que la elastografía muestra su mayor utilidad en el diagnóstico de nódulos benignos y hacen referencia a trabajos como los de Kagoya y col,8 Luo y col.4 y Nell y col,9 que llegan a esa conclusión. Si consideramos la fortaleza de la técnica en detectar los verdaderos nódulos benignos, estaría cumpliendo con la premisa de que la elastografía sirve para evitar biopsias innecesarias. Ahora bien, corresponde analizar qué conducta tomamos si nos encontramos frente a un nódulo con variables ecográficas sugestivas de malignidad (microcalcificaciones, nódulo más alto que ancho, contornos imprecisos etc.) que semuestra elástico o con bajos valores de presión. De ninguna manera dejamos de indicar la PAAF.
