The results of changing the routing, an interdisciplinary approach to treating patients with malignant neoplasms of the maxillofacial region in the Penza region are presented. The epidemiology, clinical manifestations, main diagnostic methods, routing, treatment and rehabilitation of patients with head and neck oncology are considered. The results of treatment of patients with oral cavity and lower lip cancer using photodynamic therapy (PDT), both alone and in combination with radiation and chemotherapy, are analyzed. The study showed that PDT of tumors of the lower lip and buccal mucosa in mono-mode made allowed achieving 100% efficiency. After the completed treatment, all participants in the experiment were provided with a complex of rehabilitation, including nutritional support, psychological assistance, exercise therapy, training in correct swallowing tactics, percutaneous endoscopic gastrostomy, and a course of oral cavity massage. Antitumor efficacy was evaluated in rats with M-1 sarcoma and PC-1 alveolar liver cancer and mice with B16 melanoma and Ehrlich’s carcinoma, which were transplanted subcutaneously into the thigh area of the animals. The aim of the study was to determine the optimal regimes of photodynamic therapy that would allow achieving the maximum antitumor effect up to 21 days after the photodynamic therapy. The therapy was carried out under the control of the accumulation of the photosensitizer in the tumor and surrounding tissues of the thigh by selecting the doses of the drug and the parameters of laser radiation (energy density and power density). The effectiveness of therapy was assessed by the inhibition of tumor growth, by the percentage of animals with complete tumor regression, by the absolute growth rate in animals with continued tumor growth compared to controls. The results of our studies have shown that the domestic photosensitizer liposomal borated chlorin e6 has high antitumor activity in vivo. The data of fluorescence spectroscopy for monitoring the level of PS fluorescence in order to determine the optimal time of its exposure are demonstrated. The study of the optimal exposure time involved 80 patients with different skin phototypes. The study of the effectiveness of PDT involved 42 patients aged 44–68 years with signs of chrono- and photoaging. The PDT procedure was performed by irradiating the entire surface treated with the PS with a power density of 100 mW/cm2, a light dose of 120–140 J/cm2, a laser radiation wavelength of 660 nm, and a light beam area of 400–800 cm2. It was found that exposure to PS for 10–20 min gives the highest fluorescence and does not depend on the skin phototype. The clinical effect of PDT was achieved in 85.7% of patients; there were no negative subjective sensations. Moisture metrics increased on the skin of the face by 53%, reaching the control values in young healthy volunteers, on the skin of the hands - by 64%. Elastometry indicators on the skin of the face and hands increased by 19% and 16%, respectively. Thus, the PDT procedure with PS based on chlorin e6 is an effective method for correcting involutional changes in the skin, leads to a pronounced clinical effect, improves the parameters of skin moisture measurement and elastometry, and passes without undesirable local reactions. The study included two groups of patients. Patients of the first group (31 patients) underwent septoplasty with standard management in the postoperative period. Patients of the second group (31 patients) also underwent septoplasty, and then added PBMT to the standard measures of the postoperative period at 3, 6 and 24 h after septoplasty (λ = 0.890 μm, P = 10 W, 2 min) and then intranasally 48 h after septoplasty (λ = 0.630 μm, P = 8 W, 2 min). In patients of both groups, heart rate variability and pain were assessed using a visual analog scale within 48 hours after septoplasty. In patients of the second group, after the use of PBMT, the indicators of heart rate variability had a significantly lower total power, compared with patients of the first group. So, after PBMT, the ultra-low-frequency component of the spectral analysis of heart rate variability in the first group was 18580 ± 2067 ms2, which is significantly higher than in the second group (8086 ± 3003 ms2) (p <0.001). The low-frequency component of heart rate variability was also significantly higher in the first group (1871 ± 405 ms2) compared to the second (1095 ± 190 ms2) (p <0.005), which indicates an increase in the tension of the sympathetic part of the autonomic nervous system in the group without the use of PBMT. In the first 3 hours after surgery, the severity of pain between the groups did not differ significantly (p = 0.07). In the period from 6 to 24 hours after surgery, patients who did not undergo PBMT experienced significantly higher pain than patients with PBMT (p <0.001). The aim of our study is to determine the effect of the diode laser on the pathogenic microflora of periodontal pockets and root canals in patients with endo-periodontal lesions (EPL). We carried out a comparative assessment of the bacterial contents of the root canals and periodontal pockets by polymerase chain reaction (PCR) before and after treatment with a diode laser (Doctor Smile Simpler, wavelength 980 nm) for 54 patients with EPL. The control group consisted of 56 patients who were treated according to the generally accepted method, including professional oral hygiene, endodontic treatment, and curettage of periodontal pockets. Evaluation criteria were a qualitative assessment of the content of periodontopathogens in the root canal and periodontal pocket before and after and treatment. As a result of our study, we found a statistically significant (p<0.01) reduction in the colonization of periodontal pockets and root canals in patients with EPL after laser decontamination. The high technological effectiveness of the methods used in combination with the low risk of complications at the stages of endodontic and periodontological treatment provides an antibacterial effect and reduces the duration of inflammation. One of the most actively developing applications of antitumor PDT is the treatment of maxillofacial tumors.The study assessed the organization of medical care for patients with malignancies of the maxillofacial region (MNMR) in the Penza region and the provision of oncologists, maxillofacial surgeons specializing in the treatment of tumor pathology. In the course of the study, the incidence of MNMR of the population of the Penza region and the gender and age structure were determined. This method was developed initially to treat tumor diseases, where it had shown its high efficiency and safety. Now photodynamic therapy is actively used in the treatment of cancers of the skin, bronchi, stomach, cervix, larynx, or other regions. However, numerous studies have been carried out for the entire existence of the method, demonstrating new possibilities of its application. This review highlights a number of studies in which the efficacy and safety of antimicrobial and antimycotic PDT were studied in vivo and in vitro. It has been proven to have a positive effect on the reparative processes in the wound. An experimental study was carried out to study the effectiveness of photodynamic therapy in the treatment of peritonitis in mice. While various types of semiconductor dosimeters are available, photodiode sensors are seen as a reliable and cost-effective immediate dosimeter. This study investigates the capabilities of a monolithic photodiode with an on-chip trans-impedance amplifier as a dosimeter in diagnostic radiology. A photodiode sensor covered with black insulation tape is irradiated with the diagnostic x-ray of potential in range between 40 to 90 kV with constant tube current-time product of 50 mAs at 60 cm source-to-detector distance (SDD). Exposures of different tube current at the range of 10 to 250 mA with a constant tube voltage of 70 kVp at the same setup are made. The photodiode sensor connected to the electrometer gives out readings in the millivolt (mV), and the output of the photodiode and semiconductor detector is recorded. The photodiode’s energy dependency, reproducibility, dose response, and distance dependency were evaluated as the capabilities of the photodiode to be used as a dosimeter. For energy dependency, it shows a linearity of 0.9458, while the response to increasing tube current with a constant tube voltage shows the R2 of 0.912. The photodiode shows good dependency on the tube voltage and tube current. Other than that, it also showed a linear coefficient of 0.5138 for distance dependence which is considered as a good linearity fit value for a photodiode as initial performance. However, its reproducibility is poor due to its large capacitance. This monolithic photodiode with an on-chip trans-impedance amplifier has demonstrated good results for energy dependency but poor results for reproducibility. Modern possibilities of repairing laryngeal vocal function, methods for extending the lifetime of voice prostheses are considered. The author’s method of prevention of infection of vocal prostheses using the antimicrobial photodynamic therapy (PDT) with chlorin-type photosensitizer Radagel is presented. Performing antimicrobial PDT of vocal prostheses increased the average operating time to 11.9 months compared to the control group (6.8 months), where a monthly dose of 150 mg of fluconazole was used for prevention. PDT is a method based on exposure to light after preliminary introduction of a photosensitizer into the body with the formation of singlet oxygen, which has a cytotoxic effect. The results of research on the use of PDT with various photosensitizers in the complex of therapeutic measures in patients with CIN, VIN, VaIN are presented. These data on the effectiveness and safety of PDT, ease of use allow this medical technology to be attributed to one of the most promising areas in the treatment of pathological intraepithelial changes of the cervix, vulva and vagina.
