Prostatolithiasis is one of the most common diseases of the prostate in which, due to various etiological factors, stones are formed in the parenchyma of the prostate gland. Due to the lack of a single diagnostic algorithm, there is no accurate epidemiological information on the prevalence of prostate stones. According to various authors, this figure starts from 7.35 % and reaches 88.6 %. Depending on the localization, prostate stones are divided into endogenous, exogenous and mixed. Asymptomatic course, polyetiological pathogenesis, frequent recurrences of the inflammatory process in the prostate gland with the subsequent appearance of symptoms of the lower urinary tract, sclerosis of the prostate and bladder neck determine the extremely important role of this pathology. At the same time, the potential role of progesterone in the male endocrine, reproductive and genitourinary systems from the standpoint of the historically established interpretation of this sex steroid hormone only as a female steroid with predominantly reproductive and antiproliferative effects has remained in the shadow of research for many decades. However, over the past two decades, interesting data has begun to accumulate that progesterone is just as important for men as for women. Moreover, in both sexes progesterone performs not only classical (reproductive) effects, but also has a wide range of so-called non-classical (non-productive) effects, which make it possible to call it a kind of “gray cardinal” of steroidogenesis. In the review article, based on the available literature, general and particular issues of endocrinology of progesterone in the male body are considered. Spermogram remains one of the main laboratory methods of male infertility diagnosis, but normal spermogram do not guarantee the patient’s fertility. More over, it does not allow us to find out the cause of infertility in 10–30 % cases. The sperm DNA fragmentation (SDF) studying is a possible solution to this problem.Aim. To systematize the currently available information about of clinical significance, causes, methods of diagnosis and treatment of SDF to be used by an urologist in rutine practice.Materials and methods. The search of publications in databases was carried out eLIBRARY and PubMed using the keywords: male infertility, sperm DNA fragmentation, etiology, causes, varicocele, oxidative stress, diagnostics, treatment. 111 studies were selected, which are included in this review.Results. Alcohol, tabacco smoking, narcotic substances, psychoemotional stress, adverse environmental factors and occupational intoxications, reproductive system infections and inflammation, varicocele and older paternal age can lead to SDF increasing and unfavorable reproductive outcomes, for natural pregnancy and while use an assisted reproductive technologies. The data on the role of metabolic syndrome in the genesis of SDF are equivocal. The oxidative stress is the main mechanism of sperm DNA damage. The SDF level can be estimated using direct (Comet, TUNEL) and indirect (SDS, SCSA) methods. Treatment of SDF is aimed at eliminating the detrimental factors, combating oxidative stress and recombinant follicle-stimulating hormone use.Conclusion. The level of SDF is an important indicator for assessing male reproductive potential and predicting the success of various methods of overcoming infertility. Currently, the use of an operating microscope and microsurgical instruments to perform this operation has proven its effectiveness. The question of the expediency of using intraoperative dopplerography remains a subject of discussion.The analysis of 23 articles using the MEDLINE database (PubMed) was carried out. The data on the efficacy and safety of microsurgical subinguinal correction of varicocele with and without intraoperative dopplerography are compared. We report the case of a 64-year-old male presented with multiple skin-colored, partially hyperemic cystic located in the scrotum area. They were accompanied by an active inflammatory process in the form of pain, swelling, hyperemia. The lesion was present for about 1 month. The presence of a tumor thrombus in the cavity of the main vessel in most cases is accompanied by massive bleeding during thrombectomy. To reduce the volume of blood loss, it is possible to use cardiopulmonary bypassto reduce the potential risks associated with blood loss and unstable hemodynamics. Sometimes tumor thrombosis can be growth into the vein wall or lead to occlusion IVC, we consider that it’s indication for resection of the IVC. Thrombectomy with laparotomy access can be performed even with type IV tumor thrombosis, if the thrombus is not fixed to the wall of the suprarenal IVC and surgeon have enough view of suprarenal segment.This article presents a variant of surgical treatment of patients with right kidney cancer, type IV IVC thrombosis, occlusion infrarenal IVC and ileofemoral thrombosis. RT male carriers have an increased risk of reproductive disorders, but the causes of this still are not clear enough.Aim. To evaluate semen parameters in RT(13;14) male carriers.Materials and methods. 24 men with 45,XY,der(13;14)(q10;q10) karyotype and infertility (n = 23) or recurrent pregnancy loss in spouse (n = 1).Results. Oligoasthenoteratozoospermia was detected in 18 (75 %) patients, asthenoteratozoospermia in 3 (13 %) patients, and azoospermia in 2 (8 %) patients. Normozoospermia was detected in 1 (4 %) patient, whose wife had a history of recurrent pregnancy loss.Conclusion. Paraurethral cyst (PC) is often found in women aged 20–60 years. The prevalence of PC in the female population is 1–6 %. Since PC are often located in the distal urethra and lie close to the genitals of a woman, the symptoms associated with this disease cause women both physical and psychological suffering.Aim. To assess the quality of life of women with PC.Materials and methods. The study included female individuals (n = 106) aged 18–60 years, in whom PC were detected during examination of the perineal region. The quality of life of the patients was assessed on the basis of the health quality questionnaire – SF-36.Results. The average volume of the PC in the observed women was 3.2 ± 1.3 cm. In the observed women, the intensity of pain in the perineum was directly proportional to the duration of the disease: up to a year – 86.3 ± 4.1; from one to three years – 76.4 ± 8.3; more than three years – 64.4 ± 9.2 (p <0.05). In this regard, in this category of patients, indicators of physical activity, psychological health and role functioning due to emotional state were reduced. Repeatedor constant pain during urination and during sexual activity causes severe distress, which affects the quality of life of patients with cystic formations in the paraurethral region.Conclusion. To perform comparative analysis of sperm characteristics and DNA fragmentation in men vaccinated with GamCOVID-Vac (Sputnik V) and unvaccinated men.Materials and methods. Characteristics ofsemen analysis and DNA fragmentation index in ejaculate samples of 122 men, 28 of whom were vaccinated with Gam-COVID-Vac (Sputnik V), were analyzed. Study participants did not have history of coronavirus infection COVID-19. Vaccinated patients were divided into 2 groups: group 1 (n = 10) included patients for whom the time between full vaccination (2 doses of the vaccine) and ejaculate sampling was shorter than 75 days; and the group 2 (n = 18) included patients for whom the time between vaccination and ejaculate sampling was longer than 75 days. Unvaccinated men comprised the control group (group 3, n = 94). Standard semen analysis was performed in accordance with the accepted laboratory guidelines of the World Health Organization (2010). DNA fragmentation was evaluated using fluorescent labelling of single- and double-strand DNA breaks (TUNEL).Results. Totalsperm countin the group 1 was 265 ± 75 million, in the group 2 – 355 ± 67 million, in the group 3 – 304 ± 26 million. Progressive motile (PR)sperm in the group 1was 18.5 ± 1.9 %, in the group 2 – 23.2 ± 3.0 %, in the group 3 – 18.2 ± 1.1 %. Normal sperm morphology in the group 1 was 4.0 ± 0.5 %, in the group 2 – 5.8 ± 0.9 %, in the group 3 – 4.1 ± 0.3 %. The best ejaculate quality was observed in patients of the group 2, however, no statistically significant differences in semen analysis characteristics between the groups were found. Percentage of sperm with fragmented DNA (DNA fragmentation index, DFI) in the group 1 varied between 4.6 and 31.4 %, in the group 2 between 0.8 and 36.5 %, in the group 3 (control) between 1.3 and 39.8 %. Mean number of germ cells with DNA breaksin the group 1 (12.5 ± 2.4 %) was higher than in the group 2 (10.9 ± 2.0 %) and control group (10.5 ± 0.7 %), but with no statistical significance. Percentage of patients with DNA fragmentation index above the reference value (≤15 %) in the groups 1, 2 and 3 was 20, 22 and 18 % respectively. In 3 of 6 patients with increased DFI in the groups 1 and 2, this value was above the reference level even before vaccination.Conclusion.
