El Hafed","doi":"10.33552/ojor.2021.05.000601","DOIUrl":"https://doi.org/10.33552/ojor.2021.05.000601","url":null,"abstract":"Eagle’s syndrome, or stylohyoid complex syndrome, is a rare facial pain syndrome characterised by latero-cervical pain radiating to the face, pharyngeal pain increased by swallowing, linked to an abnormal enlargement of the styloid or calcification of the stylo-hyoid ligament. The incidence of the presentation is about 4 to 8 per 10,000 individuals. The establishment of the diagnosis is a challenge in itself as we need to differentiate it from the neuralgias of cranial nerve such as glossopharyngeal neuralgia and superior laryngeal neuralgias. The surgical management of the Eagle syndrome consists in the shortening of the elongated styloid process, with the surgical access by intra oral or cervical. We report a case of a 28-year- old men admitted to our otorhinolaryngology department with sharp neck pain and a sensation of having a foreign body in the throat. Physical examination found bilateral tonsilar hypertrophia, with palpation of a solid immobile structure. CT scan with 3D reconstruction revealed elongated styloid processes with ossification of the left stylo-hyoid ligament. {"title":"Permanent Childhood Hearing Loss in Infants with Hypoxic Ischaemic Encephalopathy: Incidence and Risk Factors","authors":"Alpana Kulkarni","doi":"10.33552/ojor.2021.04.000599","DOIUrl":"https://doi.org/10.33552/ojor.2021.04.000599","url":null,"abstract":"Objective: To study the incidence of permanent hearing loss in infants with hypoxic ischaemic encephalopathy (HIE) and the risk factors associated with hearing loss. Study design: A single centre retrospective cohort study was completed in all term and near term infants with HIE over 9 years (2010-18). Newborn screening with one/two stage protocol was carried out initially (automated otoacoustic emissions only or with automated auditory brainstem testing if baby in NICU for more than 48 hours) of all babies with HIE. Babies with Stage 2 and 3 HIE were cooled. Those who failed this protocol had diagnostic auditory brainstem testing (ABR) which identified the hearing loss as sensorineural or auditory neuropathy spectrum disorder. Aetiological investigations were done to rule out other causes of sensorineural hearing loss including MRI IAMS, cytomegalovirus and Connexin 26 testing. Results: Ninety-nine babies were eligible and 8 died. Ten out of 91 babies failed the newborn screen. Of the 10, 8 (8.8%) had permanent hearing loss. Six out of 8 babies had bilateral sensorineural hearing loss, 1 had unilateral sensorineural hearing loss and 1 had unilateral auditory neuropathy spectrum disorder. Permanent hearing loss was associated with low Apgar score at 1 and 5 min (CI 0.5, 3.9, p=0.01), persistent pulmonary hypertension (p=0.0002, CI-0.5,-0.2) and loop diuretics (CI-0.4,-0.1, p=0.0002). Permanent hearing loss was found in all grades of HIE. Conclusion: This study adds to the previous studies indicating that there is high incidence of permanent hearing loss seen in term and near term babies with HIE (cooled as well as not cooled). Further studies with larger samples are needed to study the risk factors in this cohort and compare the incidence of hearing loss in cooled versus not cooled babies. What’s new: This study confirms that permanent hearing loss of all degree and severity can occur in all grades of hypoxic ischaemic encephalopathy. It indicates that auditory neuropathy spectrum disorder can develop secondary to HIE. Although postoperative scars in a blepharoplasty are relatively rare, they can generally be improved by massage and late with the application of topical corticosteroids, with the understanding that their best use is during the first twelve months. An early intervention should be avoided as this can worsen the result and rarely improve the final result, when there is a healing problem, remembering that stress is a determining factor in the final result. The laxity of the canthus tendons should always be evaluated either by directly pulling the eyelid down, with an eyelid glide test, the impingement test, and a poor eyelid position prior to surgery should always be documented. There are several well-proven techniques of skin closure using a variety of materials like braided or monofilament material, absorbable and nonabsorbable conventional suture materials, metal clips and adhesive tapes. Conventional skin suturing techniques do have certain disadvantages. From the past three decades, many new biomaterials have been discovered in the medical field. Bio-adhesives are one among them. These materials when applied on the incised tissues possess the ability to bind them together. This group of adhesives are called Cyanoacrylates. The purpose of the study was to compare the efficacy and clinical outcome of the two techniques of wound closure by using conventional polypropylene suturing and cyanoacrylate glue. Objectives of the study: The objective of this study was to compare the pain, duration of the procedure, postoperative infection, wound dehiscence and esthetic outcome of cyanoacrylate with the polypropylene suturing in skin wounds. Materials and method: A total number of 50 subjects, with lacerations of head and neck region in the age group of 15-40 years were included in the study. Group I: Lacerations closed using polypropylene Group II: Lacerations closed using cyanoacrylates Variables like pain, esthetic outcome, time taken, wound dehiscence, infection was taken into account. Conclusion: In our study we noted that cyanoacrylates is an easily applied material, has good physical properties, has good wound healing characteristics, overcomes the disadvantage and discomfort of suture removal, infection, duration of closure and produces cosmesis comparable to that produced by sutures with excellent patient satisfaction. It can manifest itself with headache, nasal congestion, bad breath, and recurrent infections. Its diagnosis can be made by anterior rhinoscopy, endoscopy, and radiology. Its treatment is surgery. Case report: A 9-year-old male patient was admitted to our clinic with recurrent nasal infection. In the endoscopic examination, a hard, irregular mass was found in the right nasal passage. Opacity was observed in the right nasal floor on direct radiography. The mass, which was removed under general anesthesia, was sent to pathology. Pathology result was compatible with rhinolith. Discussion: Rhinolitis has been known for years. Rhinolitis may be asymptomatic in some patients.
