It is generally associated with subclinical hypothyroidism. Patients with this pathology are mostly asymptomatic. It is seen in childhood by an anterior cervical mass which confuses it with a cyst of the thyroglossal tract. The diagnosis is made by ultrasound imaging, scintigraphy and histological examination. Management is essentially hormonal. It may occur as a result of surgical procedure, deep neck space infections and malignancies of the upper aerodigestive tract. It may also result from perforation by an upper aerodigestive foreign body. We present a 26 year old housewife and mother of 2, presented to our facility with complaints of anterior neck opening discharging pus and food particles following traditional tonsillotomy. Examination revealed chronically ill-looking woman, weak, lying on couch, wasted, dehydrated. Poor oro-dental hygiene, enlarged and inflamed left tonsil, shattered right tonsil with a cavity at the inferior pole exuding pus. Neck examination revealed dyschromic anterior neck skin, features of dermatitis, a supra-sternal wound measuring 2x2cm exuding pus. An assessment of Pharyngo-cutaneous fistula from traditional tonsillotomy was made. Patient was resuscitated, placed on intravenous antibiotics, nasogastric intubation for feeding and daily wound dressing. Full blood count, electrolytes and urea as well as barium fistulography was done. Packed cell volume of 32.1%, White blood cell count of 11.9x109/L, Neutrophilia of 81.6%, platelets count of 141x109/L. Electrolytes, Urea and Creatinine revealed hyponatremia of 133mMol/L, hypokalaemia of 2.8 mMol/L, other parameters were normal. Retroviral screening was non-reactive. Barium fistulography revealed numerous fistulous tracts from the oropharynx, the largest being 13x1.5cm. The airways appear intact. Fistula became closed three weeks after commencement of conservative management. El Hafed","doi":"10.33552/ojor.2021.05.000601","DOIUrl":"https://doi.org/10.33552/ojor.2021.05.000601","url":null,"abstract":"Eagle’s syndrome, or stylohyoid complex syndrome, is a rare facial pain syndrome characterised by latero-cervical pain radiating to the face, pharyngeal pain increased by swallowing, linked to an abnormal enlargement of the styloid or calcification of the stylo-hyoid ligament. The incidence of the presentation is about 4 to 8 per 10,000 individuals. The establishment of the diagnosis is a challenge in itself as we need to differentiate it from the neuralgias of cranial nerve such as glossopharyngeal neuralgia and superior laryngeal neuralgias. The surgical management of the Eagle syndrome consists in the shortening of the elongated styloid process, with the surgical access by intra oral or cervical. We report a case of a 28-year- old men admitted to our otorhinolaryngology department with sharp neck pain and a sensation of having a foreign body in the throat. Physical examination found bilateral tonsilar hypertrophia, with palpation of a solid immobile structure. CT scan with 3D reconstruction revealed elongated styloid processes with ossification of the left stylo-hyoid ligament.
