The main known factors of repeated implantation failures (RIF) in patients undergoing ART are unhealthy lifestyle, genetic factor, uterus pathology, endometriosis and adenomyosis, thrombophilia, and also male factor and embryonic factors. RIF factors and ways to overcome them receive much attention from researchers. However, although many possible RIF causes have been identified, much remains unknown regarding the etiology and management of this condition. In clinical practice, studies and interventions are often used without clear scientific rationale or convincing evidence of their effectiveness due to the lack of standardization of diagnostic and treatment methods. {"title":"Application of anterior bilateral sacrospinous fixation in the treatment of apical genital prolapse","authors":"A. A. Popov, S. A. A single-center open prospective comparative clinical study in parallel groups in parallel groups was carried out at the Clinic of Academician Krasnopolsky Moscow Regional Research Institute of Obstetrics and Gynecology. There were examined and treated 155 patients with stage II–IV symptomatic genital prolapse according to the Pelvic Organ Prolapse Quantification System (POP-Q) who underwent various variants of SSF. The patients were stratified into 3 groups: group 1 (LPSSF) – 34 patients with symptomatic apical genital prolapse who underwent SSF through the posterior vaginal wall using LPSSF ligatures (ligature posterior sacrospinous fixation); group 2 (LASSF) – 42 patients with symptomatic apical or anterior-apical genital prolapse, who underwent ligature SSF using the anterior approach LASSF (ligature anterior sacrospinous fixation); group 3 (MESH TASSF) – 79 patients with symptomatic apical or anterior-apical genital prolapse, who were treated by applying the anterior bilateral SSF method using synthetic tape MESH TASSF (tape anterior sacrospinous fixation). The frequency of intra- and postoperative complications, patient complaints, and relapse rates were assessed post-surgery. Functional outcomes were assessed using the PFDI-20 questionnaire (Pelvic Floor Distress Inventory-20) at 3, 6 and 12 months after treatment. Patients in group 3 were treated by applying a method we developed for correction of apical genital prolapse and concomitant cystocele reconstruction.Results. The method presented here was characterized by a lower blood volume loss compared to posterior SSF, minimized intra- and postoperative complications: decreased incidence of hematomas, buttock pain, rate of urinary disorders (stress incontinence, mixed forms of urinary incontinence, urgent urinary incontinence, urinary difficulty, bladder hypotension). Applying MESH TASSF fixation allowed to decrease duration of hospital treatment and alleviate pain severity in the postoperative period, and additionally contributed to improved patients' quality of life sustained for as long as 12 months post-surgery.Conclusion. The treatment results confirmed that the MESH bilateral SSF method using an anterior approach is clinically effective, relatively safe, and contributes to a markedly reduced rate of disease relapses. Artificial intelligence (AI) is a technology that simulates human brain data processing, its intellectual behavior and critical thinking. Sophisticated AI models can potentially improve patient management by speeding up processes and increasing their accuracy and efficiency at a lower cost of human resources. Compared to other specialties, use of AI in gynecology remains in its infancy. It is important to understand that the available methods for clinical imaging have certain limitations, namely clinician’s workload and data variably interpreted by different doctors. AI, in turn, has the potential to overcome these limitations while increasing diagnostic accuracy.Aim: to structure and analyze current published data on AI use in gynecology.Materials and Methods. A search for primary sources was carried out in the electronic databases PubMed, eLibrary and Google Scholar. The search queries included the following keywords \"artificial intelligence\", \"gynecology\", \"endometrial cancer\", \"endometriosis\", \"ovarian cancer\", \"diagnostics\", \"oncogynecology\" retrieved from February 2014 to February 2024. Articles were assessed according to PRISMA guidelines. After identification, before the screening stage, duplicates were excluded. At the screening stage, the titles and annotations of the identified articles were analyzed for eligibility to the review topic as well as for available full-text versions; abstracts and letters to the editorial board in scientific journals were excluded at this stage. 685 full-text articles were evaluated for eligibility, the inclusion criteria were as follows: publication in Russian or English; the study describes use of AI technologies in diagnostics or treatment of gynecological diseases. All disagreements between authors were resolved by consensus. Ultimately, 80 primary sources were included in this review.Results. AI-based systems have succeeded in image analyzing and interpreting and over the past decade have become powerful tools that have revolutionized the field of gynecological imaging. In the studies analyzed, AI was able to provide faster and more accurate forecasts and diagnostics, increasing the overall effectiveness of gynecological care. It is important to note that AI cannot fully replace doctors, but it can perfectly integrate into clinical practice, helping in the decision-making process and reducing errors in differential diagnosis and variability of interaction between different specialists. In the field of oncogynecology, undoubtedly one of the most promising aspects is the possibility of better and especially early diagnostics and, ultimately, improved patient survival.Conclusion. A great success has been achieved so far, and AI use is expected to extend in the next few years. Vulvovaginal candidiasis is an extremely common pathology of the female genital organs, leading to a long-term recurrent course and multiple complications. Although currently it is widely known about developing antibiotic resistance of bacterial pathogens, it is necessary to remember about similar phenomenon observed in other groups of infectious agents. In this regard, fungal infection also requires development of new therapeutic techniques and medicinal antifungal drugs, such as ibrexafungerp.Aim: to analyze available publications revealing the mechanism of action, efficacy, antifungal spectrum and results of clinical trials for a new oral antifungal drug ibrexafungerp.Materials and Methods. A search for publications in the electronic databases PubMed, eLibrary and ClinicalTrials.gov, published over the last 25 years was conducted using the following keywords in Russian and English: “candidiasis”, “vulvovaginal candidiasis”, “antifungal drugs”, “ibrexafungerp”, “clinical trials”, “mechanism of action”. Articles were assessed according to PRISMA guidelines. The titles and abstracts of identified publications were independently reviewed to retrieve relevant full text studies. After the selection procedure, 46 articles were included in the review.Results. This review provides information on the creation of the drug ibrexafungerp, its mechanism of action, the activity against a relatively wide range of pathogens, as well as the results from 13 ongoing and completed clinical trials in patients with fungal infection.Conclusion. More than 13 annual million deaths are caused by environmental pollutants worldwide. Urbanization, population growth, industrialization and globalization affect our lives both positively and negatively. Women can become lead exposed through occupational and environmental sources. Once lead enters the body, it is mainly deposited in diverse organs: brain, kidneys, liver and bones. The body stores lead mainly in the bones, where it accumulates over time that may be further released into the bloodstream during pregnancy, thus posing a threat to growing fetus.Aim: to examine a lead impact on newborn hematological parameters during perinatal period.Materials and Methods. A retrospective cohort study with 306 pregnant women and paired newborns was carried out. Peripheral blood lead level (BLL) in pregnant and postpartum women was analyzed by using the atomic-absorption spectrophotometry method. Blood specimens were collected for analysis in the third trimester of pregnancy.Results. We have detected a statistically significant decrease in the mean corpuscular hemoglobin concentration (MCHC) in babies born to mothers with BLL > 0.24 µmol/L vs. BLL < 0.24 µmol/L. This difference may indicate a decline in hemoglobin fetal production caused by lead intoxication.Conclusion. The prospective observational comparative study in parallel groups enrolled 43 pregnant women divided into 2 groups: main group – 23 pregnant women with diagnosed PE and control group – 20 apparently healthy women with uncomplicated pregnancy course, delivery and the postpartum period. To examine PE phenotypic features, the main group of pregnant women with PE was subsequently divided into 2 subgroups according to the date of pathology onset: early (n = 10) and late (n = 13) PE. Using the whole-genome next-generation sequencing (NGS), a comparative analysis of altered 18 microRNA level in placental tissue was carried out.Results. Pregnant women with early PE compared to the control group were characterized by significantly low expression level for hsa-miR-656-3p (p < 0.001), hsa-miR-323a-5p (p = 0.017), hsa-miR-519c-3p (p = 0.019), hsa-let-7i-5p (p = 0.019), hsa-miR-433-3p (p = 0.019), hsa-let-7g-5p (p = 0.030), hsa-miR-214-5p (p = 0.030), hsa-miR-27a-5p (p = 0.031), hsa-miR-339-5p (p = 0.041), hsa-miR-524-5p (p = 0.045), hsa-miR-1283 (p = 0.049) and high expression for hsa-miR-151a-5p (p = 0.007), hsa-miR-4521 (p = 0.018), hsa-miR-30d-5p (p = 0.026), hsa-miR-548l (p = 0.027), hsa-miR-133b (p = 0.034), hsa-miR-424-5p (p = 0.042), hsa-miR-211-5p (p = 0.049). Patients with late PE had significantly decreased expression for hsa-miR-656-3p (p = 0.050) and hsa-miR-574-3p (p = 0.017) as well as a significantly higher for hsa-miR-211-5p (p = 0.001) compared to the control group. Subgroup of women with early vs. late onset PE was characterized by significantly decreased expression level for hsa-miR-323-5p (p = 0.007) and overexpressed hsa-miR-30d-5p (p = 0.002), hsa-miR-5481 (p = 0.027).Conclusion. Obesity is a worldwide problem at epidemic scale. Bariatric surgery (BS) is the most effective method to treat morbid obesity characterized by long-term and positive effects on concomitant diseases. However, the effect of BS on female reproductive health has not been sufficiently studied to date, which makes relevant to conduct further investigations in this field.Aim: to summarize research data assessing BS effect on female reproductive health.Materials and Methods. There was conducted a search for publications in the electronic databases PubMed, eLibrary and Google Scholar to identify the following keywords and their combinations in Russian and English: \"bariatric surgery\", \"reproductive health\", \"sex hormones\", \"pregnancy\", \"menstrual cycle\", \"fetal\", \"neonatal\". The evaluation of the articles was carried out in accordance with the PRISMA recommendations. Based on search data, 868 publications from PubMed, 83 publications from eLibrary and 74 publications from Google Scholar were retrieved. After the selection procedure, 79 articles were included in the review.Results. To date, the data regarding the long-term BS effects on improved obesity-related endocrine disorders remain insufficient. It is better to recommend long-acting reversible contraceptives such as intrauterine device, to post-BS patients. Women with BS history should have a continuous comprehensive nutritional assessment as well as blood macronutrients and trace elements monitoring before, during and after pregnancy. Recognizing potential complications associated with BS in the anamnesis and timely seeking specialized medical aid or timely transfer of a patient to a specialized medical facility may improve pregnancy outcomes for a mother and paired newborn.Conclusion. While making decision to perform BS, it is necessary to fully understand its impact on female reproductive health, assess patients' reproductive intentions and fulfill health education obligations. All patients of childbearing age who are candidates for BS should participate in a joint decision-making dialogue on BS-related risks and benefits for reproductive health, which should be supported by extensive multidisciplinary work involving obstetricians, gynecologists, endocrinologists, psychologists and representatives of other medical specialties. Initially discovered as a mechanism to protect host neutrophils from pathogens and prevent spread of infection outside inflammatory site, neutrophil extracellular traps (NETs) have been implicated in progression of other diseases associated with sterile inflammation such as autoimmune diseases, diabetes, and cancer. NETs components (myeloperoxidase, citrullinated histones, cell-free DNA) exhibit manifold effects on tumor cells, thereby emphasizing a need to be aware of the features of biological functions related to their constituents and their place in carcinogenesis to identify major molecular targets for targeted therapy of gynecologic cancers in the future.Aim: to determine an impact of NETs on tumor progression/metastasis and thrombosis risk in gynecologic cancer.Materials and Methods. A single-center interventional study was conducted: 70 women with uterine, ovarian and cervical cancer were examined; 60 age-matched apparently healthy women without thrombotic complications were selected as controls. All study participants were examined for myeloperoxidase (MРO), citrullinated histone (сitH3), proinflammatory cytokine interleukin-1β (IL-1β), and neutrophil/lymphocyte ratio (NLR).Results. Laboratory biomarkers such as MPO (p < 0.001), IL-1β (p < 0.001) and NLR (p = 0.003) were significantly more often elevated in patients with oncological pathology compared to group of healthy women. 32 (45.7 %) of the 70 women with cancer of the reproductive system had metastases. Metastases-related analysis in patients showed significant differences in MPO level (p = 0.002), but not in level of citH3, IL-1β and NLR (p = 0.441, p = 0.159, and p = 0.739, respectively). Elevated citH3 vs. MPO, IL-1β and NLR level was significantly more often associated with developing thrombosis in study patients (p < 0.001).Conclusion.
