It is a benign, inflammatory condition that most frequently affects the tongue's dorsum and may spread to its lateral edges. The loss of filiform papillae and thinning of the epithelium are represented by the erythematous patches. It can be asymptomatic or patient may complains of pain and burning sensation. Besides advances in rgical techniques and radiation therapy, tumour may recur locally or metastasize to distant sites with local recurrence being predominant form. Here we present a case report of primary spinal chordoma which recurred locally within a year of surgical excision. Chordoma of spine is a rare low grade malignant tumour with strong propensity for local recurrence. {"title":"Patterns of bone marrow infiltration in lymphomas- A retrospective descriptive analysis from a tertiary care centre in Southern India","authors":"G. Anandani, P. Basu","doi":"10.18231/j.jdpo.2022.053","DOIUrl":"https://doi.org/10.18231/j.jdpo.2022.053","url":null,"abstract":"Bone marrow biopsy (BMB) examination is the gold standard for staging of lymphoma which is interpreted along with other clinical, laboratory and radiological investigations. This study aimed to evaluate the various patterns of BMB infiltration by lymphomas which include Hodgkin and Non Hodgkin lymphoma and the usefulness of BMB compared to bone marrow aspiration (BMA). : In a period of three years, there were 212 cases which showed lymphoma infiltration in bone marrow. We assessed the peripheral blood smear (PBS), BMA and BMB slides and concordance between each was evaluated. BMB slides were assessed for the pattern of involvement. The most frequent subtype was chronic lymphocytic leukemia/small lymphocytic lymphoma (CLL/SLL) (21.70%) followed by Hodgkin lymphoma (HL)(12.73%)and follicular lymphoma (FL)(11.32%). Predominant pattern of infiltration showed by CLL/SLL cases was mixed pattern, by FL was paratrabecular pattern, mantle cell lymphoma(MCL) showed either diffuse or nodular, diffuse large B cell lymphoma (DLBCL) and Burkitt lymphoma(BL) showed predominantly diffuse pattern. Hairy cell leukemia (HCL) and peripheral T cell lymphoma (PTCL) showed an interstitial pattern. In HL, the pattern was usually focal nodular however diffuse involvement was seen in a few. Of the 212 cases, 50.94% cases had atypical lymphocytes or lymphocytosis in PBS and 81.14% cases showed infiltration in BMA. Atypical lymphoid cells may not be present at times in peripheral blood or bone marrow aspirate in spite of infiltration in the bone marrow. {"title":"Covid-19: Consumption coagulopathy with increased severity and mortality - A retrospective study","authors":"Kunal Gaur, S. Natarajan, M. Gupta","doi":"10.18231/j.jdpo.2022.039","DOIUrl":"https://doi.org/10.18231/j.jdpo.2022.039","url":null,"abstract":"To compare quantitative values of parameters of coagulation pathway like Prothrombin Time (PT), Activated Partial Thromboplastin Time (APTT), International Normalized Ratio (INR) and D-Dimer (DD) on date of admission versus date of patient's mortality. A total of 50 patients who were tested positive for COVID-19 in our hospital between April 21, 2021 and May 30, 2021, were taken into this study. The changes in PT, APTT, INR and DD were compared at day of admission and day of patient's mortality. The study involved 50 patients (36 male and 14 female). D-dimer at the day of admission (mean 1540.79 ng/ml; IQR 231-8776) was found with an elevation of 250% at the day of mortality (mean 5379.04ng/ml; IQR 434-10000). PT at the day of admission (mean 13.602; IQR 11.6-14.8) was found elevated (Normal range-10.5-13.5sec) in 19/50 patients which increased to 43/50 patients at the day of mortality, an increase of 126%. According to the study done, the number of patients with increased levels of PT, APTT, INR and DD were substantially greater at the time of mortality compared to the same patients at their time of admission, suggesting a dynamic coagulation process in COVID-19 patients. Prospective Observational Study.To evaluate and describe histopathologic findings in the placentas of women affected with COVID19. A total of 16 placentas from pregnant women who were infected with COVID-19 in any gestational age and delivering between March 2020 and February 2022 were examined and compared with normal third trimester placentas received in the same time period. Among the 16 placentas examined, most significant change as a sign of maternal malperfusion is increased intervillous fibrin deposition, which is seen in 81.25% cases, followed by infarction, calcification, increased syncytial knots, haemorrhage, calcification, vascular congestion and fibrin thrombi. Most significant foetal malperfusion sign is Chorangiosis which is seen in 37.5% cases, followed by thrombi in umbilical vessels. Also, 25% of the placentas examined showed Villitis, 18.75% showed Deciduitis, 6.25% showed Chorioamnionitis and 6.25% showed vasculitis. We present a rare case of subleukemic leukemia presenting as pancytopenia in a 55 years female, who presented with mild constitutional symptoms of generalized weakness, easy fatiguability and loss of appetite for the past 3 months. A 4 months old female infant presented to us for aspiration cytology with clinical suspicion of sarcoma due to complaint of ankle swelling and failure to thrive. Fine needle aspiration (FNA) smears were acellular with only calcific deposits. On clinical co-relation depressed nose and frontal bossing were noted. We suspected it to be a case of congenital skeletal dysplasia. Plain X-ray of the lesion was ordered and it revealed stippled calcification in place of ankle bones consistent with chondrodysplasia punctata. The present case is an index case describing the cytology of chondrodysplasia punctata. CDP is mainly a radiologically diagnosed lesion, however the knowledge of its cytological picture through this case will make pathologists alike aware if they encounter such case.
