HCC ranks fifth from the top among the most prevailing cancers in men globally. It stands second among the major causes of mortality associated with cancer in the world. The majority of the cases of hepatocellular carcinoma (HCC) are due to cirrhosis developed after infection with the chronic hepatitis B virus (HBV) or hepatitis C virus (HCV).\u0000\u0000Objective:\u0000To assess the burden of hepatocellular carcinoma and to study the pattern of hepatitis B and C in HCC patients who visited oncology outpatient department (opd) services in a tertiary care hospital in Karachi. \u0000\u0000Materials and Methods:\u0000This is a retrospective study. The total number of patients visiting the Oncology OutPatient Department from July 2019 to June 2021 was recorded. Patients who had HCC of these were included in the study. The age, viral markers, and TNM staging of the patients were recorded. Both male and female patients belonging to any age group were included in the study.\u0000\u0000Results:\u0000During two years, a total of 2,102 patients visited oncology outpatient services for different cancers. Out of these, 50 patients had a diagnosis of HCC and were included in the study. The mean age of the patients was 59.76±10.16 years. 70% of the patients were male and 30% were female. 60% of the patients were positive for Hepatitis C, while 36% of the patients had all viral markers negative. \u0000\u0000Conclusion\u0000HCC is a fairly common malignancy but screening programs across the world are hampered by low rates of utilization. Infection with hepatitis B and C is frequently associated with the development of HCC. {"title":"Experience of Endoscopic Ultrasound Fine Needle Aspiration in Tertiary Care Hospital","authors":"","doi":"10.37184/lnjcc.2789-0112.3.15","DOIUrl":"https://doi.org/10.37184/lnjcc.2789-0112.3.15","url":null,"abstract":"OBJECTIVE:\u0000The objective of the study is to determine the spectrum of endoscopic ultrasound fine needle aspiration ( EUS FNA) in the diagnosis of the pancreas, GIT, hepatopancreatobiliary tract, mediastinal lymph node and hepatic lymh node by using rapid on-site evaluation (ROSE).\u0000METHODOLOGY:\u0000A retrospective study was conducted at the department of Histopathology and endoscopy suit (Liaquat National Hospital and Medical College) during the period of March 2019 to March 2022. A total of 55 patients undergoing EUS FNA were included in our study. 22 gauge needle was used for performing EUS FNA and stained with PAP alcohol fixed slides while stained with Diff –Quick air dried slides, as well as sample, also needed for cell block preparation.\u0000RESULTS: \u0000The mean age of the patient was 56.1. A total of 55 patients were included among them 33 patients were males and 22 females. The most common site was the pancreas, 38 cases with diagnosed as 68.4% were adenocarcinoma, mediastinal lymh node 9 cases 6.4%, gastric 4 cases 7.3%, rectal 1 case 1.8%, duodenal 1 case 1.8%, hilar mass 1 case 1.8% and hepatic lymh node 1 case 1.8%. \u0000CONCLUSION: \u0000EUS FNA is a valuable, safe procedure that can be done under conscious sedation but rare complications included bleeding and pancreatitis. It provides at least a cost-effective mode of provisional diagnosis for the diagnosis and cost-effective testing of the pancreas, GIT, post mediastinum or related organs. Close interaction with cytopathologists is essential to improve diagnostic yields. It has been shown that disease relapse occurs due to the existence of cancer stem cells (CSCs), residing within the tumor. \u0000Objectives\u0000To develop a cancer stem cell line model, exploiting the property of stem cells to resist treatment with anti-cancer therapeutic agents and to develop novel therapeutic interventions targeting the elimination of CSCs.\u0000Methods\u0000Single-cell clones (SCC) from MCF-7 cells were established in 96-well plates using a culture medium containing 5% FBS. Enrichment of cancer stem cells was carried out from established single cell clones which were initially cultured in a serum-deprived nutrient medium for six weeks followed by Doxorubicin treatment. Doxorubicin-resistant clones were established and evaluated for their growth and sphere-forming abilities. Further, these clones were characterized based on the presence of stem-cell markers using a semi-quantitative reverse transcription-polymerase chain reaction and results were compared with the parental MCF-7 cell line.\u0000Results \u0000In a complete medium, these spheres differentiated and started growing as a monolayer with differential expression levels of genes involved in stemness and cellular differentiation. When these spheres were sub-cultured again in stem-cell medium and detached to give rise to single cells, these clones retained sphere-forming ability. Doxorubicin-resistant clones showed a tendency to grow in spheres in a stem-cell medium with serum-deprived culture conditions.\u0000Conclusion\u0000Our results provide undeviating evidence of the successful establishment of clones of MCF-7 cell line exhibiting stem cell-like properties. The complex nature of chemotherapeutic agents results in multiple physiological and psychological issues and if the patient and their caregivers (family) are unaware of these effects, poses additional stress which may result in patient dissatisfaction. This pilot project aimed to provide patients'/families' awareness regarding the process of treatment and subsequently to improve their satisfaction level.\u0000Methods: This academic project is a mandatory assignment of clinical practicum course (for the degree of Masters in Nursing Sciences (Aga Khan University School of Nursing & Midwifery). An interventional design was employed for this study. A total of 30 cancer patients, that is 15 in the intervention group and 15 as controls were recruited. The project was initiated as an academic requirement (started in Jan 2015) and last for four months (Apr 2015). Patients planned for their first chemotherapy cycle were selected in both groups. A self-developed questionnaire was used to assess the patient satisfaction level regarding the chemotherapy process before and after the standardized and video-based educational intervention. An independent T-test was used to analyze the data. \u0000Results: The overall mean score of patient satisfaction level after the intervention was 18.80; whereas, it was 11.93 for another group. The patient's mean satisfaction level was higher for the intervention group. \u0000Conclusion: This study indicates video-based intervention for pre-chemotherapy teaching as an effective educational mode to assist patients throughout the treatment process. Thus, the symptomatology related to pituitary lesions could be secondary to mass effect or functional secondary to the hypo or hypersecretion of hormonal substances. \u0000\u0000Objective:\u0000The study was conducted to identify the relationship between lesion volume and the duration of symptoms.\u0000\u0000Methods: \u0000The study included 70 patients whose data were collected in a retrospective manner. Data was collected via outpatient and in-patient admission record systems, of the patients with pituitary adenoma presenting to the neurosurgical unit of Liaquat National Hospital, Karachi, between 2012-20. The volume of the pituitary adenoma was calculated based on the formula of volume ½ (length x breadth height). All adenomas were divided into three categories, based on volume. \u0000\u0000Results:\u0000A total of 70 patients of either gender the age of 17 to 74 were included. Among 70 patients, 2(2.9%) of patients were found with volume <0.52cm3, 42(60%) with 0.52-8.16 cm3 and 26(37.1%) with >8.16 cm3. No significant association was found between the symptom duration and volume of the adenoma.\u0000\u0000Conclusion:\u0000The relationship of volume with the duration of symptoms in pituitary patients was found insignificant. This could be attributed to multiple factors. As we do not have our data thus we need to ascertain the pattern of malignancy in our population so that awareness pertaining to the commonest type of gynecological malignancies, in our population can be formulated. This can help formulate screening guidelines for our region of the world and help us in planning programs to spread awareness for their early detection and recognition.\u0000Material and Methods:\u0000It was a retrospective review of 367 patients' data attending the Medical Oncology Clinic of Liaquat National Hospital from January 2016 to January 2020.\u0000Results: \u0000The most common site of gynecological cancer was Ovarian 181 (49.3%), followed by Uterine 124 (33.8). Cervix was the 3rd most common tumor with 33 (9%). 8 patients had dual ovarian and uterine primary tumors (2.2%). In the absence of a randomized control trial, pulmonary metastasectomy is commonly performed in an effort to improve survival. However, data from retrospective analysis and case series remain subject to selection bias. The aim of this study was to review the published data, especially to identify prognostic factors, to inform future clinical practice in the field of pulmonary metastasectomy for metastatic soft tissue sarcoma. We searched the SCOPUS database using the terms \"pulmonary metastasectomy\" or \"lung resection\" or \"lung metastasectomy\" or lung metastasectomies\" and sarcoma and survival in the title, abstract, and keywords of all documents. Over the study period (1976-June 2021), a total of 438 documents of published articles were retrieved and 192 were identified for bibliometric analysis, and of those, 128 articles were included in the content analysis. No randomized trial was identified. 'Bradford's law' was applied to classify journals into 'zones' and 6 journals appeared to publish 70/192 (36.4%) of all articles, receiving 52% of the citations. The vast majority of articles 64% were retrospective cohort analyses. A total of 49 articles reported on prognostic factors. Pulmonary metastasectomy is frequently performed for lung metastases originating from STS. The literature continues to evolve in the form of retrospective cohort studies.
