Among GISTs, duodenal localization occurs in less than 5% and usually presents with upper GI bleeding. A 45-year-old man presented in the outpatient department with complaints of epigastric discomfort, intermittent melena and undocumented weight loss for the preceding 3 months. Initial upper GI endoscopy showed mild duodenitis and no other upper GI pathology. For unexplained symptoms, a CT Scan was performed which demonstrated a well-defined solid lesion along the second part of the duodenum. An endoscopic ultrasound (EUS)-guided biopsy of a subepithelial lesion at D2 was performed. Immunohistochemistry findings were suggestive of GIST. Wedge resection of the duodenal mass was done.\u0000Duodenal GIST should be considered as a differential in cases of GI bleeding when other differentials have been ruled out. This is a double-edged sword as sleep disturbances then exacerbate cancer symptoms. \u0000\u0000Objectives: \u0000To determine the sleep quality in cancer patients as well as the relationship of sleep quality with socioeconomic status, financial burden, marital status, Eastern Cooperative Oncology Group (ECOG) performance status, pain, use of analgesia, and type of cancer.\u0000\u0000Materials and Methods:\u0000 An analytical cross-sectional study was carried out with 100 adult and pediatric cancer patients presenting to Mayo Hospital, Lahore. The Urdu version of the Pittsburg Sleep Quality Index (PSQI-U), Numerical Rating Scale (NRS), and Eastern Cooperative Oncology Group (ECOG) score were used to determine the quality of sleep, the intensity of pain, and performance status respectively. Statistical analysis was carried out using SPSS software version 26. Binary logistic regression was used to assess factors associated with sleep quality (defined by a cutoff value of ≥5 on the PSQI scale). Variables with p-value <0.25 were used to build the final multivariable regression model.\u0000\u0000Results:\u0000A total of 100 participants, with a mean age of 40.2 ± 19.7 years, and a male gender predilection (88%) were enrolled in the study. Non-hematological malignancies accounted for 62% of all patients while 80% of the participants were suffering from stage 4 cancer. The mean PSQI score was 11.56 ± 5.70. Sleep quality was associated with increasing age, marital status, employment status, pain incidence, increasing ECOG status, and undergoing chemotherapy on univariate analysis. On the multivariable model, increasing age and increasing ECOG scores were independent predictors of sleep quality. \u0000\u0000Conclusion:\u0000Cancer patients have poor sleep quality which is linked with advanced disease, aging, pain, and poor performance status. Usually, end-of-life decision-making takes place in a critical situation of the patient; therefore, health care providers, particularly, physicians and nurses play a major role in making a decision for the patient’s life with regards to updated knowledge and practice. In this view, health care providers face many challenges in end-of-life decision-making due to controversy among equally unfavorable solutions; particularly between two ethical principles i.e., patient autonomy and beneficence. Health care providers often overweigh beneficence over autonomy regarding less suffering for the patient and his/her family. This approach of health care providers raises a question for undermining patients' autonomy and violating the basic ethical right of a patient. To overcome these kinds of ethical challenges, it is imperative to equip health care providers with updated knowledge of advance directives for patients. In addition, patients and their families should be well informed from the beginning to the end stage of the patient stay in the hospital. Studies have shown differences in the expression of her2/neu as per ethnicity.\u0000\u0000Objectives: To assess the association of HER2/neu positive breast cancer with various ethnicities in Karachi, Sindh, Pakistan. \u0000\u0000Methodology: A prospective observational study was conducted at the Department of Oncology, Jinnah Postgraduate Medical Centre between June 2019 to January 2022. All patients referred to the oncology department from the outpatient department and emergency department for the management of breast cancer were eligible to partake in the study. Diagnosis of breast cancer was made on histopathology. Patients' socio-demographics including age, gender, occupation, education, ethnicity (Urdu speaking, Sindhi, Punjabi, Balochi, Pushtoons, and others), and clinical parameters including the site of the tumor, stage, and grade at presentation, and first-line treatment were documented. \u0000\u0000Results: A total of 550 patients were included.
