Chishti, Mohammed Arif Mateen Khan","doi":"10.37184/lnjcc.2789-0112.2.4","DOIUrl":"https://doi.org/10.37184/lnjcc.2789-0112.2.4","url":null,"abstract":"Background & Objective:\u0000The study objective was to review the clinic-pathological characteristics of adnexal masses in the children and adolescent population in our institution.\u0000Methods:\u0000Retrospective analysis of 178 adolescent patients including neonates, presenting with adnexal masses between January 2000-December 2012, was performed. \u0000Results: \u0000Records of a total of 178 patients were analyzed. The mean age was 14 years. The most common symptom was abdominal pain noted in 146(82%) patients followed by nausea and vomiting in 28(15.7%) and incidental findings in 19(10.7%). The majority of the ovarian masses were benign. The follicular cyst was the most common histopathological type in 66 patients (37.1%) followed by a hemorrhagic cyst in 40(22.5%), benign teratoma in 21(11.8%), benign serous cyst in 20 patients (11.2%), endometrioma and mucinous cystadenoma in 13 patients (7.3%). The average tumor size observed in the present study was 7.84cms. However, the size varied in different age groups such as 4.27 cm size in neonates, and 7.1 cm in 1-14 years age group, and 8.61 cm in 15-19 years age group with a P-value of 0.009. Out of a total of 178 patients, 46(25.8%) were managed conservatively. However, 90(50.6%) patients underwent ovarian cystectomy, 26(14.6%) were managed by unilateral salpingo-oophorectomy, 10(5.6%) by unilateral oophorectomy. Aspiration and biopsy were done in 6(3.4%) patients.\u0000Conclusion:\u0000Incidence of adnexal masses increases with age with maximum cases between age 14 and above. Adolescent girls with adnexal masses can have variable presentations however abdominal pain is the most common symptom. Herein, we report the long-term outcomes of PABC from a single institution in an Arab country.\u0000\u0000Methods: \u0000Consecutive patients diagnosed to have PABC between 2005 and 2012 at a tertiary referral hospital from a Gulf cooperation council country were the subjects of the study. Long-term outcomes are reported, with a minimum follow-up of 8 years.\u0000 \u0000Results: \u0000A total of 16 patients were evaluable for long-term survival analysis. The median age at the time of diagnosis was 31.5 (26-40) years. Nine (56%) patients were multiparous (> 5 previous pregnancies). The mean gestational age at diagnosis was 19.7±7.4 weeks. Immunohistochemistry revealed the following phenotypes: Luminal A 3 (18.8%); HER-2 enriched 8 (50%); triple-negative 5 (31.2%). Three patients underwent modified radical mastectomy as the initial treatment, of which 2 received adjuvant chemotherapy during pregnancy. For patients who received neoadjuvant or palliative chemotherapy, the response rate was 75% (pCR 2; CR 1; PR 6). After a median follow-up of 60 months, median progression-free survival was 36 months (95%CI 24.2 to 47.8), while the overall survival was 59 months (95%CI 31.6 – 86.4). Age, marker status, Ki-67 score, clinical stage and differentiation grade did not affect the PFS or OS on univariate analysis.\u0000\u0000Conclusions: \u0000Fifty percent of the patient with PABC expressed HER-2/neu protein, and 1/3rd had triple-negative disease. The rate of response to chemotherapy, and long-term survival may help to set a benchmark for studies from the region. With reference to the time of occurrence, a second primary malignancy can be either synchronous or metachronous. The synchronous cancer of the breast can also be present in the same breast. Usually, in metachronous bilateral breast cancer (MBBC), the histologically same type of malignancy is seen at different periods. However, we present a case of MBBC in a 62-year-old female patient who had two histologically different types of breast cancer. She presented in hospital with a lump in her right breast and a fungating mass in her left breast. On biopsy right-sided lump was invasive ductal carcinoma while the left-sided mass was borderline phyllodes tumor, at two separate biopsies from renowned histopathology labs. Pakistan alone has the highest rate of Breast Cancer than any other Asian country as approximately 90000 new cases are diagnosed every year out of which 40000 dies. Obesity is also a critical public health problem growing with every passing year in Pakistan and worldwide. Research studies are being conducted worldwide regarding the relation between the two problems. \u0000\u0000Objective:\u0000The objective of this study is to determine the prevalence of obesity in breast cancer patients in a tertiary care hospital in Karachi, Pakistan.\u0000\u0000Methods:\u0000BMI is used as a screening tool for overweight and obesity. According to World Health Organization, a body mass index (BMI) over 25 is considered overweight, and over 30 is obese. \u0000A retrospective analysis of 262 patients diagnosed with Breast Cancer during 2019 and 2020 was performed. Patients’ hospital records in Oncology were reviewed. The weight in kilograms and height in centimeters of patients were reviewed. Their BMI was calculated and recorded using the SPSS system. \u0000\u0000Results:\u0000The median BMI was 28.25 kg/m2 with an interquartile range of 25.15 - 31.99 kg/m2. The indications for biopsy were suspicious digital rectal examination and abnormally high PSA. The procedure was performed either transurethrally, transperineally or transrectally, \u0000Results: \u0000A total of 268 patients were diagnosed with Prostate Adenocarcinoma, the age range was from 40 to 94 years. The median age was found as 68 years with an interquartile range of 15 years. The median Gleason score was found as 8 with an interquartile range of 7 scores. A negative weak corelation was found between the age of the patients at the time of diagnosis and the Gleason score (r= -0.013, p=0.832).\u0000Conclusion: Though there are many studies that support the existence of corelation between increasing age and Gleason Score, our study, like a couple of others, showed no association between the two and a weak positive corelation. Our conclusion of the study is that while studying the relationship between the two, other variables like PSA, DRE findings and TNM classification of the tumor are also important. It also improves the relationship between the doctor and the patient.\u0000\u0000Objective: Current study aims to determine the patient satisfaction level going for major gynecological surgery by detailed counseling and consent.\u0000\u0000Material and Methods: This is a cross-sectional study conducted at Liaquat National Hospital, Karachi. The duration of the study was one year (1st march 2018 to 28 February 2019). Informed consent was taken from participants fulfilling the inclusion criteria. The questionnaire was composed of demographic details of patients such as age, gender, marital status, education level, and socioeconomic status. The satisfaction rating scale was used to assess the impact of counseling on the patient. The scale contains different items regarding counseling and consent that assesses patient satisfaction, rated on a 1–5 scale (1 = strongly disagree; 5 = strongly agree). Higher scores indicate higher satisfaction.\u0000\u0000Results: In our study majority of the participants were from 41-50 (30%) years of age, and all were married and belonging to the middle class. The majority were from the secondary level which is 62%. Out of 156 participants, 87 agreed that counsellor listened to them, 96 understood the information, 91 were comfortable talking to the counsellor and 80 were more hopeful about their progress. Similarly, the majority of the participants agreed and were satisfied with the treatment after informed consent. Results found that 85% of participants showed a higher satisfaction level towards counseling and informed consent. \u0000\u0000Conclusion: Counseling is a useful supplementary technique for refining treatment adherence and disease condition. This, modern radiation delivery technique allows high-dose delivery to the target and minimum dose to the brain/critical organs resulting in better tumor control and perhaps an improvement in Neurocognitive functions (NCF). \u0000\u0000Objectives: To explore the effect of stereotactic radiosurgery/stereotactic radiotherapy on neurocognitive functions (NCF) in various brain tumors.\u0000\u0000Methods: A prospective observational study was conducted at Neurospinal and Cancer Care Institute (NCCI), Karachi. A total of 100 patients who had undergone multisession stereotactic radiosurgery /stereotactic radiotherapy were selected after taking their written informed consent by employing a convenient sampling technique. The type of tumor was classified and identified on a radiological and histological basis. Neurocognitive function evaluation was carried out through Mini-Mental State Examination (MMSE) score questionnaire provided to patients at baseline (pre-radiosurgery SRS/SRT), and 6 months after treatment (post-radiosurgery SRS/SRT). \u0000\u0000Results: The mean age of patients was 37.38±15.03 years and the majority (49%) belonged to the 27-44 years age group. The majority of them were males whereas the most common pathology found was glioma (31%). Moreover, both overall and in patients having prior surgery, the comparison of different MMSE score levels showed that there was a significant increase in patients with normal score levels and a significant decrease in patients with mild and moderate score levels after treatment (P=0.006 and P=0.046 respectively) at post-treatment time.\u0000\u0000Conclusion: Stereotactic radiosurgery results in the improvement of neurocognitive functions. We conducted a retrospective analysis of the three case series of pleural large fibrous tumors. Additionally, we will discuss our experience with pre-surgical Angioembolization, involving implanting coils and plugs, to treat spindle cell neoplasm.
