We report two cases of male siblings with CAH. Cases: The elder sibling was diagnosed late at 4 years old with signs of virilization, complicated with central precocious puberty (CPP). The younger sibling was diagnosed after baseline screening and stimulation test. Treatment with both hydrocortisone and fludrocortisone were commenced for both siblings once the diagnosis was made. The elder sibling however, required treatment with gonadotropin analogues to treat the CPP. Conclusion: These cases illustrate the importance of an early diagnosis of CAH and the benefit of screening siblings to prevent late presentations of CAH in high-risk families.\u0000\u0000\u0000\u0000

Methods: This is a retrospective study. Demographic and clinical data of children diagnosed with moderate and severe laryngomalacia who underwent supraglottoplasty between 2012 and 2021 were retrieved from the computerized patient record system of Hospital Sultanah Bahiyah. The surgical procedure of supraglottoplasty was mainly performed with cold instrument, with occasional assistance from a microdebrider. Study outcome includes complete stridor resolution, weaning from tube feeding, and weaning from respiratory support post supraglottoplasty. Results: 124 children were included in the study. Complete stridor resolution was achieved in 64.51% (n=80) 1-month post-surgery and 86.29% (n=107) 3-months post-surgery. Half of the children with tube feeding pre-operatively could be weaned off to oral feeding within 1-month post-surgery. All the children with oral feeding maintained oral feeding post-surgery. All the children with no or low respiratory support (nasal prong oxygen) pre-operatively were able to be weaned down to room air within 1-month post-surgery. Within the group of children with high respiratory support (high-flow nasal cannula, non-invasive ventilation, and endotracheal intubation) pre-operatively, half were weaned down to room air within 1-month post-surgery. None of the children with isolated laryngomalacia required tracheostomy after supraglottoplasty. All laryngomalacia patients who required tracheostomy after supraglottoplasty were associated with either synchronous airway lesions or medical comorbidities. Only 3 cases (2.42%) required revision supraglottoplasty with a maximum of one revision. Conclusions: We conclude that supraglottoplasty contributes to substantial clinical improvement in moderate and severe laryngomalacia. Supraglottoplasty facilitated weaning down of respiratory support and tube feeding in laryngomalacia with respiratory difficulty and feeding intolerance.\u0000\u0000\u0000\u0000

This study was conducted to determine the financial impact on the parents and risk factors for prolonged hospitalisation. Methods: A prospective, cross-sectional hospital-based study was conducted at the University Malaya Medical Centre (UMMC) among patients aged between 3 months and 12 years old with CAP. Parents were interviewed and medical records were reviewed. Results: A total of 95 patients were enrolled in this study with a median age of 13 months, of whom 52 were boys (54.7%) and 43 were girls (45.3%). The median (IQR) total expenses incurred by the parents was RM546.90 (USD 128.23), which constituted 12.6% of total household income. The median workday loss was 9 (4 – 29) days. Pre-existing chronic illness and underweight were found to be strong predictors of prolonged hospitalisation with odds ratio of 3.97 and 4.5, respectively. Conclusion: CAP resulted in a significant financial burden in terms of loss of productivity and costs incurred by parents.\u0000\u0000\u0000\u0000

The coping strategies practised by parents often play a major role in determining the level of parenting stress. Objective: This study aimed to identify the proportion of parents with parenting stress among those who have children with epilepsy, to compare the level of stress in different domains (parent, child, and parent-child interaction) as well as to examine the relationship between parenting stress and parental coping strategies. Methods: In this cross-sectional study, a total of 61 parents of children with epilepsy aged between 2 to 12 years old were recruited from the Pusat Perubatan Universiti Kebangsaan Malaysia (PPUKM) paediatric clinic over eight months. Validated questionnaires were used to determine the parenting stress level and coping strategies. Results: The proportion of parents with high stress scores (13.1%) was lower in this study compared to previous studies, except for non-Malay parents. Among the three domains, 29.5% of parents reported a high stress level under Parent-Child Dysfunctional Interaction (P-CDI). A negative correlation was observed between active coping and total parenting stress scores (TPSS) (r = -0.323, p = 0.011). In contrast, there was a positive correlation between self-blame coping and TPSS (r = 0.315, p = 0.014). Conclusion: Parents of children with epilepsy mostly reported low TPSS, except for non-Malays. Parents are encouraged to practise more active coping and less self-blame in managing children with newly diagnosed epilepsy.

Recently, there has been a concerning increase in total number of obese children in Malaysia. This study aims to predict the trend of obese children in Malaysia, especially in Kuala Lumpur. This study involved primary schools in Kuala Lumpur, Malaysia. A total of 4970 seven year old primary school children were involved in this study. Data were collected by the SEGAK test performed by the physical education teacher in the primary schools. In this study, weight and height were the variables that were used for BMI calculation. Underweight, normal, overweight, and obese were used to classify the children’s BMI following the WHO BMI index for Asians. Descriptive statistics, Normality test, and Discriminant Analysis were used for the statistical analysis. The result found that the BMI was distributed normally according to P-P and Q-Q plot graph, and discriminant analysis found that there were 231 children (4.65%) of the total sample of the children predicted to have an increase in BMI and a total of 236 (4.74%) predicted from normal BMI to underweight. The result predicted that there would be a slight increase in children concerned about the potential of becoming obese. Action should be taken regarding the prediction of the percentage as the number can increase either suddenly or slowly.

Methods: A 10-year retrospective record review was conducted involving paediatric patients who were diagnosed with SB. Baseline data was recorded, including type of SB, level of spinal lesion, neurological sequelae (bladder, bowel incontinence, ability to walk), renal function and imaging findings (ultrasound, MCUG, DMSA). The socio-demographic characteristics were described using descriptive analysis. Multiple logistic regression analyses were used to determine the association between significant predictors for renal scarring and chronic kidney disease (CKD). Results: A total of 60 SB children with a mean age 11.8±3.8 years during data collection were recruited. Most of the lesions were at L5 and below. The majority were complicated by urinary (88.3%) and bowel incontinence (93.3%). Despite the majority performing clean intermittent catheterization (CIC), almost two-thirds had symptomatic urinary tract infections (UTI). One-third of them had both hydroureter and hydronephrosis. 50% of them were shown to have vesicoureteric reflux. Half of the cohort progressed to CKD. The odds of developing renal scarring were 8.3 times higher in females compared to males (p = 0.046). The presence of hydroureter and hydronephrosis on ultrasound was identified as a significant factor in the development of CKD (OR 3.8; 95% CI 1.239, 11.385).
