It is distinguished in particular by the diagnostic autonomy granted to the vascular physician, who prescribes, performs, and interprets functional tests. These include a range of non-invasive examinations, foremost among which is Doppler ultrasound. The major advantage of vascular medicine lies in the immediate correlation between clinical data and imaging, thanks to specific ultrasound training integrated into the DES. It addresses a wide spectrum of pathologies, ranging from arterial conditions to venous diseases, including microcirculation and certain rare diseases. It involves caring for acute or chronic patients-who are often in pain and anxious-with complex wounds and significant sensitivity in the affected areas of the body. In this context, nursing expertise plays a fundamental role in the quality and safety of care. This qualitative study compares the practices of sixteen nurses (ten registered nurses [RNs] and six advanced practice nurses [APNs]) in the context of chronic disease management. Results reveal two parallel trajectories: that of the RNs, who develop empirical expertise, and that of the APNs, who possess regulatory authority. Faced with its own dead ends, it also seeks leads in genes, the microbiome, and the search for biomarkers. This headlong rush reveals a fundamental difficulty in identifying, naming, and defining what the discipline actually treats. This article proposes to \"disorganize\" psychiatry-that is, to halt the search for the single organ it has never had-and to rebuild it around its true object: the person, within their environment, embedded in their history, and in the history of their environments.
