{"title":"Implant-supported prosthesis treatment planning aspects of Kaunas city dentists and dental specialists.","authors":"Henrikas Rusilas, Laura Jacinkevičiūtė, Jan Pavel Rokicki","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Rehabilitation of mastication using fixed or removable prosthesis on dental implants is a daily procedure in modern dentistry.The huge variety of diagnostic tools and methods, such as cone beam computed tomography or surgical guides help to avoid complications. A vast quantity of diagnostic tools and prosthesis require a tight communication between surgeons and prosthodontists in order to achieve better treatment plan.</p><p><strong>Methods: </strong>The questionnaire was approved by LUHS Committee of Ethics. A questionnaire of 17 demographic and specialized questions was composed. A randomized survey of dentists and dental specialists was conducted in Kaunas. A statistical analysis was performed using χ<sup>2</sup>test and Student's T-test criteria.</p><p><strong>Results: </strong>Most of correspondents believe that treatment plan should be created by the current dentist, regardless his/hers specialization. All correspondents performing dental implantation, use elevation of mucoperiosteal flap. The most common diagnostic tool among dentists and oral surgeons is panoramic x-ray. The most common diagnostic tools between prosthodontists are panoramic x-ray and analysis of dental stone castings. The most common complications among dentists and prosthodontists are improper adjustment of soft tissue and errors of dental technicians, among oral surgeons - improper adjustment of soft tissues and implant position.</p><p><strong>Conclusions: </strong>1. The creation of treatment plan does not depend on specialization of dentist. 2. The elevation of mucoperiosteal flap is more common than non-flap procedure. 3. The most used diagnostic method is panoramic x-ray. 4. {"title":"The oral health status and behaviour of methadone users in Lithuania.","authors":"Rokas Aukštakalnis, Tomas Jurgelevičius","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To find out the oral health of former heroin users in Lithuania, who are currently on methadone treatment.</p><p><strong>Materials and methods: </strong>Subjects of research are patients, who were receiving methadone as a medication for heroin addiction. The research was done in Centre for Addictive Diseases in Lithuania. Oral health examinations were carried out using a probe and dental inspection mirror. The decayed-missing-filled (DMF) index and Silness Loe plaque indices were used. Patients received a questionnaire of smoking habits, dentist visits, daily dose of methadone and symptoms of hyposalivation. All the data was entered to Microsoft Office Excel 2016 and analyzed by SPSS 25.0 software.</p><p><strong>Results: </strong>Study included 49 patients - 7 women and 42 men. The mean age of these subjects was 40.3±8.1, ranging from 26 to 58 years old. Mean number of decayed, filled, missing and residual roots were 3.1±2.9, 12.1±9.5, 0.3±1.3, 4.4±4.1, respectively. Mean score of Silness Loe plaque index was 1.7±0.9. The statistical difference (p<0.05) was found between patients DMF and their visits to the dentist. {"title":"Influence of glass fibre post cementation depth on dental root fracture.","authors":"Miglė Jakubonytė, Kęstutis Česaitis, Jonas Junevičius","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Introduction: </strong>Glass fibre posts that are newer and have better properties are gaining more popularity than metal posts. However, there is no consensus about the optimal cementation depth of glass fibre posts. In our study, we have attempted to assess fracture resistance of roots restored with glass fibre posts cemented at different root depths.</p><p><strong>Materials and methods: </strong>Specimens were formed with peeso reamers and a special reamer adapted for the cementation of glass fiber posts at different depths. Glass fibre posts were cemented using self-etching cement: in group 1 - at the depth of 2/3 of the working length of the root canal (11 mm), in group 2 - at the depth of 1/2 of the working length of the root canal (7.5 mm) and in group 3 - at the depth of 1/3 of the working length of the root canal (5 mm). Dental roots were standardized by preparing their walls to equal measurements with a mill. Specimens were embedded on a metal plate at a 45° angle and were vertically pressed with a hydraulic press in buccolingual direction. {"title":"Evaluation of the posterior maxillary teeth movements during Class II correction: 3-dimensional superimposition of casts.","authors":"Igor Griblasky, Dalia Latkauskiene, Gundega Jakobsone","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim and objectives: </strong>The aim of this study was to analyze the changes of the posterior dentition during Class II correction with the crown Herbst appliance (cHerbst) and intermaxillary Class II elastics.</p><p><strong>Sample: </strong>Class II patients were divided into 2 groups: those who were treated with the cHerbst appliances (n=40, mean age 14.1±1.3) and those who were treated with fixed appliances and Class II elastics (n=20, mean age 16.7±2.7).</p><p><strong>Materials and methods: </strong>The scans of pre- and post-treatment casts the patients were superimposed and registered using the freeware Blender 2.67. The linear measurements in the anteroposterior, transversal and vertical planes along with the rotation angles of the first molars were assessed.</p><p><strong>Results: </strong>In the cHerbst group clinically significant derotation (13.8±5.0°) and distalization (3.2±1.2 mm) of the upper first molars were recorded compared to the intermaxillary Class II elastics group (4.2±5.5°, -0.3±1.5, respectively). In the cHerbst group the second and first premolars followed the distal movements of the upper first molars. The premolars expanded slightly more than the molars in both groups. {"title":"Treatment options for odontogenic maxillary sinusitis: a review.","authors":"Rokas Aukštakalnis, Rūta Simonavičiūtė, Regimantas Simuntis","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>The aim of this article is to review the main treatment options for odontogenic sinusitis that are used today.</p><p><strong>Material and methods: </strong>Search on PubMed, Cochrane Library, PMC, Science Direct data bases. For a literature review search keywords were used: odontogenic sinusitis, odontogenic maxillary sinusitis treatment OR diagnosis, maxillary sinusitis of dental source OR dental origin OR etiology.</p><p><strong>Results: </strong>2886 articles were found in the databases using keywords. After duplicate citations screened, inclusion/exclusion criteria applied, excluded articles after titles, summaries and full-text reading 25 articles were included in the literature review.</p><p><strong>Conclusion: </strong>Although clinical symptomatology is not conspicuous among other types of sinusitis, the odontogenic maxillary sinusitis treatment regimen is different. It consists of eliminating dental infection and management of sinusitis. The usage of Caldwell-Luc approach is limited these days and recommended only when a better access to sinus is needed, for example, removing large foreign bodies. Endoscopic sinus surgery is widely used these days to remove the inflamed sinus mucosa, foreign bodies, displaced teeth, while preserving physiological function of the sinus. {"title":"Quality of life in patients with temporomandibular disorders. Keywords and their combinations were used to find relevant articles and publications concerning the subject.</p><p><strong>Results: </strong>A total of 320 publications were initially retrieved. After further examination 12 articles were selected due to their relevance to inclusion criteria and were included in the systematic review. The selected 12 articles published between year 2006 and 2016.</p><p><strong>Conclusion: </strong>In this systematic review it was found that there is a direct correlation between temporomandibular disorders and lower quality of life. Out of questionnaires used for identification of patient satisfaction SF-36 and OHIP-14 were most popular in these studies. {"title":"Correlation of temporomandibular joint clinical signs with cone beam computed tomography radiologic features in juvenile idiopathic arthritis patients.","authors":"Ilga Urtane, Iveta Jankovska, Hadeel Al-Shwaikh, Zane Krisjane","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Patients with juvenile idiopathic arthritis (JIA) have a high risk of temporomandibular joint (TMJ) involvement. Early detection of osseous destruction of the TMJ that can be seen radiographically is vital to provide appropriate treatment before significant craniofacial deformities and problems with occlusion arise. The aim of study was to evaluate whether there is a correlation between the clinical signs and cone beam computed tomography (CBCT) radiologic features of TMJ in patients with JIA.</p><p><strong>Material and methods: </strong>Study group consisted of 65 patients (46 females and 19 males) aged 10 to 17 years with a confirmed JIA diagnosis and mean disease duration 2 years 9 month, all patients underwent a clinical examination of the TMJ and masticatory muscles as well as a radiological assessment of the TMJ osseous structures by CBCT.</p><p><strong>Results: </strong>Majority of the patients' study population experienced 2 or 3 clinical signs with mean number 2.1 (standard deviation (SD) =1.00) and 3-5 radiological features related to the TMJ destruction with mean number 4.9 (SD=1.96). Statistically significant weak correlation only between pain and condyle surface flattening (Spearman`s Rho test (rho) =0.396; p value (p) =0.001) was found. No correlation between number of clinical signs and radiological features was found.</p><p><strong>Conclusions: </strong>There was no conclusive evidence found regarding correlation between TMJ clinical signs and radiological features of osseous destruction in patients with JIA - only weak correlation between pain and condyle surface flattening was observed. The number of clinical TMJ signs does not correlate with number of radiological features. {"title":"Clinical substantiation of the sparing and less invasive implant prosthetics of the edentulous lower jaw method.","authors":"Vladimir Trezubov, Yuri Parshin, Roman Rozov, Alexander Simonenko","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Proof of the effective implant prosthetics treatment of patients with the edentulous lower jaw by minimally invasive method.</p><p><strong>Materials and methods: </strong>As a sample of clinical research, 68 senior patients (30 men and 38 women) were diagnosed with edentulous lower jaws. The methods of the study included estimating the implant retention by frequency-resonance analysis of Osstell ISQ, the degree of retention and stabilization of the tissue supported and implant-supported dentures (author's method: patent for invention No. 2640375 of 28.12.2017) and a sociologic questionnaire \"Profile of the influence of dental health OHIP-14\".</p><p><strong>Results: </strong>For the first time strength indicators of the retention and stabilization degree of implant-retained mandibular overdentures with attachment to one and two implants were determined. A linear relationship was shown between the degree of fixation of the prosthesis and the change in the quality of life (a statistically significant correlation was p=0.022, r=0.52 in the first group and p=0.0015; r=0.75 in the second group). The implants stability during the year increased in the first group in 2.57±1.59%, and in the second group in 2.08±1.40%. After a year using implant-retained mandibular overdentures, the life quality level concerning dental health in patients from the first and second groups remained the same (there were no statistically significant differences, p1-2=0.8392).</p><p><strong>Conclusions: </strong>The performed survey of prosthetic treatment patients with the edentulous lower jaw showed the effect use of a sparing method of implantation prosthetics. The fixation quality achieved with a single support-retaining implant was sufficient to improve the functional and comfortable using of the lower jaw prosthesis.
