{"title":"Dental roots' and surrounding structures' response after contact with orthodontic mini implants: A systematic literature review.","authors":"Greta Gintautaitė,&nbsp;Giedrius Kenstavičius,&nbsp;Alė Gaidytė","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The objective of this article was to conduct a systematic literature review about the consequences and recovery of dental roots' and surrounding structures' after iatrogenic mini implant (MI) contact based on peer reviewed publications of 2008-2017 January. The Cochrane and PRISMA references were used while searching for scientific literature in six data bases. The inclusion criteria to select articles were: 1) root contact evaluation associated with the use of orthodontic MI, 2) the diameter of MI was ≤2.5 mm, 3) the research sample was ≥20 MI, 4) the extent of dental root or surrounding structures damage, the regeneration/degeneration and their progress was described, 5) the condition of dental roots and surrounding structures should be evaluated immediately after damage with MI, after longer contact with MI and after a certain period of time, 6) articles published in 2008-2017 January. Two authors independently reviewed and extracted data from the selected studies and a methodological quality assessment process was used to rank the studies classifying them as low, moderate or high quality. 13 articles met the selection criteria of the research. The success of damaged dental root or surrounding structure regeneration was influenced by the damage extent: the cementum, dentin or periodontal ligament may regenerate; the regeneration of damaged pulp is uncertain. The loss of pulp vitality, root resorption, root fracture, ankylosis or osteosclerosis are rare complications. Processing the information recorded on it and integrating the esthetic evaluation with biologic and functional parameters will really allow dentist to formulate a correct diagnosis and to choose most suitable treatment plan for the patient. The aim of this study was to evaluate the prevalence of facial and dentolabial parameters among students of the Faculty of Dentistry of Lithuanian University of Health Sciences and to identify relationship of all facial and dentolabial parameters results.</p><p><strong>Material and methods: </strong>The study is carried out at the Department of Dental and Maxillofacial Orthopedics of Lithuanian University of Health Sciences. The facial and dentolabial parameters of students are examined by visual examination using medical gloves and rulers. Participants were sitting on a chair at a distance of 150 cm in front of the researchers.</p><p><strong>Results: </strong>The thick type of lips was the most popular for normal profile face, of thin lips - concave, of medium lips type - convex. The most popular of profile face types were normal and convex. The most popular tooth exposure among all smile line types was less than half teeth. {"title":"Uses of maxillary sinus lateral wall bony window in an open window sinus lift procedure: literature review.","authors":"Elvinas Juzikis, Algimantas Gaubys, Henrikas Rusilas","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>To review all of the possible uses for maxillary sinus lateral wall bony window in an open maxillary sinus lift procedure and to evaluate the influence of each method to the rate of sinus membrane perforations.</p><p><strong>Methods: </strong>A systematic literature review was performed of randomized control studies in English identified in MEDLINE (PubMed) and Cochrane online databases, published between 2007.09.01 and 2017.09.01. Surgeries had to be performed in vivo, for patients over 18 years old. A study had to have at least 10 sinus lifting procedures, had to detail how the bony window was used and had to report the number of Schneiderian membrane perforations.</p><p><strong>Results: </strong>922 publications were found, out of which 68 were selected for qualitative assessment. 29 of them were selected for quantitative assessment. 4 distinct uses for bony window were found: bony window is elevated into the sinus cavity under the membrane; removed and discarded; repositioned to its original position after the surgery; used as a graft material for sinus lift.</p><p><strong>Conclusions: </strong>there is a statistically significant difference of sinus membrane perforations between different uses of the lateral bony window of an open sinus lift procedure. It is very important to realize what obstacles appear in medical care professionals (dentists and general medical practitioners(GPs)) way for primary oral cancer diagnostics.</p><p><strong>Purpose: </strong>To find out the insight of primary oral cancer diagnostics in Kaunas city.</p><p><strong>Goals: </strong>to analise knowledge of society about oral cancer and its primary symptoms; to evaluate and compare the insight of POCD for general medical practitioners and dentists; to appraise the oncological awareness and evaluate the experience in oral oncology for mentioned medical care proffesionals.</p><p><strong>Materials and methods: </strong>Questionnaires were prepared for patients and medical professionals. Permission for investigation was confirmed by LUHS Bioethical centre. Questioning was done in Kaunas city (Lithuania) and its districts.</p><p><strong>Results: </strong>100 questionnaires were collected from random people, questioned at health care centers and 106 questionnaires were given to medical staff (64 for dentists and 42 for general medical practitioners); 81% of GPs and 75% of dentists claims to have low insight on POCD; 83,3% of GPs and 46.9% of dentists claims that they have not enough learning experience on POCD from graduated university. {"title":"Results of complex rehabilitation patients with temporomandibular joint disease and parafunction of masticatory muscles.","authors":"Roman Fadeyev,&nbsp;Vasiliy Parshin","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>Definition of the effective and appropriate essential use of myogymnastic exercises and orthopedic methods of posture correction in the complex rehabilitation of patients with TMJ pathology and parafunction of the masticatory muscles.</p><p><strong>Methods: </strong>In the study, 63 patients (11 men and 52 women) aged from 20 to 67, with the middle age 31±2.3 were treated, 43 of them had TMJ disease. In 51 patients, parafunctions of masticatory muscles were diagnosed. The treatment was carried out with the help of uncoupling the dentition rows of the splint, made after the TENS-therapy; used medicamentation treatment, using an individual plan for myogymnastic exercises and individual insoles for correction the posture.</p><p><strong>Results: </strong>Total effectiveness of the treatment is shown as the average of the findings. Analysis of treatment dynamics was due to only those data the results of which after treatment in the control and main groups were different. Treatment effectiveness of the main group in comparison with the control one is as follows - periodontal vessels dopplerography - 16.22%, tonus of the right chewing muscle - 11.45%, right temporal muscle tone - 4.8%, the number of hours per a day in case of increased tone of the chewing muscles - 12.9%, mouth opening amplitude - 17.1%. Alveolar ridge and surrounding gingiva deficiencies aggravates implant placement in the aesthetic area. This case report describes a technique for aesthetic single implant placement with simultaneous bone grafting and soft tissue thickening. At the time of implant surgery, allogenic (Maxgraft, Botiss Biomaterials, Germany) and xenogenic bone substitute (Cerabone, Botiss Biomaterials, Germany) was used for bone grafting, soft tissues were augmented simultaneously with collagen tissue matrix derivate membrane (Mucoderm, Botiss Biomaterials, Germany). After 4 months during second stage surgery the implant was exposed. Subsequently healing abutment was replaced with provisional crown for gingival contouring. An individual zirconia abutment was made and a cemented full-ceramic crown was placed for final restoration. The 12-month follow-up check-up revealed a pleasing aesthetic treatment outcome, as well as clinically healthy peri-implant soft tissues. Radiological examination showed a stable bone crest with minor bone remodelling around the implant platform. A case report.","authors":"Mustafa Ayna,&nbsp;Aydin Gulses,&nbsp;Thomas Ziebart,&nbsp;Andreas Neff,&nbsp;Yahya Açil","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Aim: </strong>The aim of this study was to describe the histological characteristics of a peri-implantitis case in the anterior maxilla.</p><p><strong>Case report: </strong>A dental implant inserted in the missing upper right lateral incisor region has been removed with its adjacent tissues. The samples were placed in 4% formalin for 10 days and, were embedded in methacrylate prior to sawing and grinding. The samples were processed with Donath´s sawing and grinding technique, stained with toluidine blue and mounted on high-sensitivity plates for histology and microradiography. The structure of the connective tissue revealed that there was a lack of collagen fibers running parallel to the implant surface. The connective tissue showed a loose granulation tissue with medium-density lymphocyte infiltration and neutrophilic leukocytes. In addition to the collagen loss in the infiltrated tissue, an excessive bone resorption was present. Peripherally, the light microscopy showed the osteoclasts and their adhesive apparatus which promote bone resorption.</p><p><strong>Conclusion: </strong>With the increasing number of implants being placed, peri-implantitis has become much more prevalent. Then, to achieve the correct diagnosis whether their presence is associated to a syndrome, some criteria must be followed. The present study aims to report a case of a 21 years-old male patient whose was referred with several radiolucent lesion on the maxilla-mandibular complex. The lesions were biopsied and the diagnosis of the histological exam hypothesized as odontogenic keratocyst. After complete evaluation, others abnormalities were also found such as calcification of falx cerebri, palmar and plantar pits, and multiple basal cell on feet. The patient was diagnosed with Gorlin-Goltz syndrome and was referred to other medical specialties to adequate follow-up. Aim of the study was to evaluate correlation between psychosocial factors and children's dental anxiety. Totally, 240 randomly selected children (mean age M=7.96, SD=2.61, range 4 to 12) and their parents took part in the study. Parents evaluated their own (MDAS) and their children's anxiety (CFSS-DS). Psychosocial factors were evaluated by a large questionnaire, developed for this study. Dental status was fixed and child's behavior in dental setting was evaluated with Frankl's scale. Pearson's correlation of CDA with all variables and stepwise linear regression with the correlating variables within the seven psychosocial factor blocks was performed. Dental experience and attitude factors (crying at dentist and dental treatment with difficulties) as well as Children's personality and behavior factors (general anxiety and children's behavior at dentist) gave the most effect on CDA, totally explaining 56% and 54% of variance, respectively. Children's medical experience and attitude factors (anxiety and caution towards doctors) as well as Parental/information factors (parental dental anxiety, promising prizes before treatment) explained 34% and 31% of CDA variance, respectively. Socio-economic factors (number of children and mother's age) explained 15%, but oral care habits and attitude (brushing as obligation) - 14% of CDA variance. Family distress factors had no correlation with CDA and were excluded of further analysis. Twelve full text articles were selected and used in this review.</p><p><strong>Results: </strong>143 articles were found in the database using keywords: PEEK, prosthodontics, dentistry. Data on the suitability of PEEK polymer were organized according to mechanical, chemical, physical properties and PEEK surface preparation.</p><p><strong>Conclusions: </strong>PEEK polymer is suitable to use in prosthodontics. However, there are not enough statements about complications, biofilm formation on PEEK surface and its resistance to compression.
