A case report.","authors":"Gustavo Frainer Barbosa,&nbsp;Daniele Pereira Dotto","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A non-ideal implant position and angulation may bring about some technical concerns because the abutment prosthetic possibilities may fail to correct the implant angulation or its consequent prosthetic restoration insertion plane. This case report describes a prosthetic solution for fixed full-arch maxillary prosthesis with six implants (Bone-Level Regular CrossFit®, Straumann AG, Basel, Switzerland) non-satisfactory position in maxilla; one of the implants showed buccal angulation and a divergent parallelism greater than 45° in relation to the other five implants. To correct the implant angulation and the insertion bar plane, a screw-retained RC non-engaging gold abutment for bridge was used in five out of six implants, and a combination of a screw-retained RC CrossFit™ gold abutment for crown and a Screw Bloc System (CNG Soluções Protéticas, São Paulo, Brazil) was used for the right-hand-side anterior implant that showed a greater buccal implant angulation (>45°). A satisfactory outcome was obtained, which totally met the patient's expectations. On each cephalogram 5 linear, 3 angular skeletal and 6 linear soft tissue landmarks were traced and measured using AudaxCeph program. The radiographs were divided into the three groups according to the ANB angle: control group, with skeletal class I (ANB angle 1-5°), study group 1 with skeletal Class II (ANB angle >5°), study group 2 with skeletal Class III (ANB angle <1°). All measurements and relations between dento-facial complex anatomical structures were statistically evaluated using IBM SPSS Statistics software.</p><p><strong>Results: </strong>261 patients (156 females, 105 males, aged between 7 and 35 years) were selected. The skeletal class II group consisted of 114, class III group - 59 and class I - 88 patients' cephalograms. Hyoid bone distance both to cervical third vertebrae and mandibular symphysis, maxilla and mandibular length, inferior airways space significantly differed between the Angle classes. Inferior airway space had a significantly positive correlation with all measurements, associated with hyoid bone.</p><p><strong>Conclusion: </strong>Inferior airway space was significantly narrower and the hyoid bone localization was in more posterior superior position in the skeletal class II in comparison with other both groups. Studies show that even 77% of deaths from oral cancers occurred in less developed regions. Gladly, in some countries mortality from oral cancer is decreasing due to timely proper treatment. Concerning diagnostic and treatment progress, the ability of dental practitioners and physicians to recognize and diagnose oral cancer as early as possible and send patients to the appropriate specialist as quickly as possible is of great significance, and in order to achieve that, doctors must continuously improve their knowledge.</p><p><strong>Material and methods: </strong>The research was carried out across ten cities located in Lithuania and their districts. In the implementation of the main tasks, the subjects were divided into two groups: A - 256 randomly selected dentists; B - 114 randomly selected physicians. Equal questionnaires were compiled for both groups. The questionnaire divided into 3 parts: 1) demographic data 2) part is devoted to assessing experience in the field of primary oral cancer diagnostic (POCD). 3) part was intended to evaluate the knowledge of POCD and oncological vigilance.</p><p><strong>Results: </strong>The main results of the present study indicate that 208 dentists and 99 physicians (total n=307) answered that they had been visited by a patient with oral cancer. 200 dentists and 73 physicians (total n=273) answered that they had diagnosed or suspected a case of oral cancer. 211 dentists and 61 physicians (n=272) state that they examine the patient's oral cavity for oncodiagnostic reasons. 205 dentists and all surveyed physicians responded (altogether n=319) that they received enough knowledge about oral cancer from their university studies. All the surveyed physicians and even 247 dentists (altogether n=361) said they wanted to have an annual oral cancer diagnosis week at their workplace (free supplementary education and POCD). Most assessed doctors claim that their knowledge about the primary diagnosis of oral cancer is average (n=162) only 16.8% dentists and 25.4% physicians evaluate patient's alcohol usage, contrastingly even 68.4% and 73.7% respectively evaluate patient's tobacco usage in the anamnesis. Regarding the correctly answered questions concerning the most common type of oral cancer, the present study shows low results: 70.3% and 61.4% of dentists and physicians accordingly.</p><p><strong>Conclusions: </strong>Healthcare providers such as dentists and physicians take up a big part in POCD. Physicians as well as the majority of dentists in Lithuania demonstrate a lack of information regarding mean symptoms of oral cancer and do not perform as thorough anamnesis as foreign clinics, that is why they may often fail to identify oral cancer at an early stage. Due to free vascularised tissue transfers, radical cancer treatment and immediate reconstruction are feasible as a one-stage procedure. The aim of this paper is to evaluate retrospectively our experience with free flap reconstruction of oral defects for patients with oral malignancies in advanced stages.</p><p><strong>Material and methods: </strong>During the 8-year period, 153 patients with oral cancer had undergone simultaneous extirpation of tumor with or without suspicious lymph node removal and reconstruction of the defect with free flap. A total of 157 free-flap reconstructions had been performed for 153 patients. Accordingly, 116 patients had reconstruction of oral soft tissue defect, but 37 patients had complex soft tissue and bone defect reconstruction.</p><p><strong>Results: </strong>132 patients had successful free flap surgery with uneventful post surgery period. Flap success rate was 96.8%. Donors site was closed primarily in 58 cases, skin graft was used in 102 cases. Five patients (3.2%) died during post surgery period. Average hospital stay was 20,5 days (from 8 till 44 days). There was no statistically significant correlation found between the patient's age and hospital stay, but hospital stay increased with tumor size (p>0.05).</p><p><strong>Conclusion: </strong>The application of the best possible reconstruction method that contributes to the fulfillment of expectations and provision of a good functional and esthetical result is enabled by setting proper, realistic reconstruction goals. Twelve full text articles were selected and used in this review.</p><p><strong>Results: </strong>143 articles were found in the database using keywords: PEEK, prosthodontics, dentistry. Data on the suitability of PEEK polymer were organized according to mechanical, chemical, physical properties and PEEK surface preparation.</p><p><strong>Conclusions: </strong>PEEK polymer is suitable to use in prosthodontics. However, there are not enough statements about complications, biofilm formation on PEEK surface and its resistance to compression. The search was constrained to English language articles published during last five years and includes only clinical trials.</p><p><strong>Results: </strong>In total, 25 articles were included in this literature review. The studies were divided into five main groups, depending on the application: 1) utilization of autologous platelet rich fibrin at 3rd molar extraction sockets preservation, 2) any extraction socket preservation, 3) bone augmentation/sinus lift procedures with implant placement, 4) augmentation of peri-implant tissues, and 5) treatment of infrabony defects.</p><p><strong>Conclusions: </strong>The application of autologous platelet rich fibrin (PRF) show benefits in promoting soft tissue healing and bone regeneration in different surgical procedures. Furthermore, it is an autologous biomaterial which has low cost and does not need administration of any additives. {"title":"Relationship between unilateral posterior crossbite and mandibular asymmetry during late adolescence.","authors":"Kristina Lopatienė,&nbsp;Karolina Trumpytė","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>The aim of the study: </strong>Was to evaluate the relationship of mandibular condylar and ramal symmetry with unilateral posterior crossbite during late adolescence.</p><p><strong>Material and method: </strong>120 pre-orthodontic patients of the Lithuanian University of Health Sciences Department of Orthodontics were included in the study. The main inclusion criteria of the study group were the following: permanent dentition, age 15 to 18 years, unilateral crossbite. Panoramic radiographs database were analyzed, the following parameters were evaluated: mandibular condylar and ramal height, and asymmetry index according to Habets' technique. Statistical data analysis was performed using SPSS 22.0 statistical software.</p><p><strong>Results: </strong>In the study group the mandibular condylar height, ramal height, and ramal plus condylar height on the crossbite side were statistically significantly lower than those on the noncrossbite side. Silanated barium-aluminum-silica glass was used as inorganic filler. Camphorquinone (CQ) - 1 mol%, 2-(dimethylamino)ethyl methacrylate (DMAEMA) - 2 mol%, and two concentrations of DPI (0 or 0.5 mol%) were used as the photoinitiator system. Two light-curing units (LCUs) were used (a single-peak (Radii-Cal) and a polywave (Bluephase)). The CLD was indirectly assessed in a softening test by Knoop hardness indentation; FS and FM were measured by means of a three-point bending test.</p><p><strong>Results: </strong>DPI positively influenced Knoop hardness when compared with experimental resin without DPI. The flexural strength of experimental cements was lower than that found with Variolink II. The crosslinking density (obtained by reduction of Knoop hardness) of cement with 0.5% DPI was similar to that of dual - cured Variolink II.</p><p><strong>Conclusions: </strong>DPI showed an improvement in some of the properties tested but was inferior to the commercial dual-cured resin cement. This situation is of significant importance causing orthodontic, surgical and interceptive problems. Trauma in the incisor area is a possible cause of the displacement of the tooth bud of the permanent canine resulting in the deviated eruption path. The aim of this article is to present two clinical cases with diagnosis of maxillary canine displacement and impaction in the incisor region after a traumatic injury during childhood, discussing treatment possibilities and outcomes achieved after orthodontic treatment.</p><p><strong>Materials and methods: </strong>In this article ectopic maxillary canines migration in the central incisor area after a traumatic injury to the anterior upper teeth are described. The path of eruption of the canine can cause severe central incisor root resorption. Treatment strategy established for the case reports described involves orthodontic space opening, extraction of the central incisor, uncovering the impacted canine and orthodontic traction of the tooth in place of central incisor. Modification of crown morphology and color were essential to obtain an aesthetic and function outcome.</p><p><strong>Results: </strong>The patients finished treatment with normal and stable occlusion, an adequate width of attached gingiva and good aesthetic results.</p><p><strong>Conclusion: </strong>Authors suggest that might have been a possible connection between the trauma of primary dentition causing the displacement of the tooth bud of permanent canine and the deviated path of eruption. {"title":"Comparison of skeletal anchorage distalizers effect in maxillary buccal segment: A systematic review.","authors":"Lior Levin,&nbsp;Arūnas Vasiliauskas,&nbsp;Juste Armalaite,&nbsp;Kristina Kubiliute","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>The aim of this systematic review was to evaluate and compare distalization effect in maxillary buccal segment between Palatal Skeletal Anchorage (PSA) and Zygoma Gear Appliance (ZGA) in evidence-based way.</p><p><strong>Materials and methods: </strong>Relevant studies published between January 2007 till December 2017 in PubMed, ScienceDirect, AJO-DO and Scopus electronic databases were identified. Inclusion criteria were: English language, study performed on humans, randomized or nonrandomized clinical trials, assessment of buccal segment distalization by Palatal or Zygomatic skeletal anchorage and patient's clinical preoperative and post-operative evaluation measured by cephalometric analysis. Quality assessment of included studies was performed.</p><p><strong>Results: </strong>A total of 357 scientific publications, articles, clinical trials related to the used keywords were identified during the search. Thirteen articles fulfilled our inclusion criteria. PSA system showed maxillary molar distalization distance range between 1.8 mm to 6 mm. ZGA presented molar distalization range from 4.37 mm to 5.31 mm.
