Dantas","doi":"10.5935/1676-2444.20200039","DOIUrl":"https://doi.org/10.5935/1676-2444.20200039","url":null,"abstract":"ABSTRACT Introduction: Clinical analysis laboratories are health care facilities that provide resources for outpatient and/or emergency diagnoses; they are regulated in Brazil by the Resolution of the Collegiate Board of Directors (RDC) no. 302, of October 13, 2005, of the Brazilian National Agency of Sanitary Surveillance (Anvisa). Objective: The aim of this study was to perform a normative evaluation in clinical analysis laboratories, based on RDC no. 302, of October 13, 2005. Material and method: This is a cross-sectional and descriptive study. Twelve clinical laboratories participated in the study. The data were obtained through a structured questionnaire and answered by the technical leaders of the participating laboratories, in the municipality of Macapa, Amapa, Brazil. Results: The average nonconformity found among the participants was 9.64%, which allowed an overall evaluation among them as satisfactory, in relation to the minimum requirements demanded by the standardization recommended by Anvisa. When the participants were analyzed individually, there was a variation from 4.82% to 21.69% of the nonconformities index. Other studies agree with the results, however there is disagreement in a research carried out in Porto Alegre, Rio Grande do Sul, Brazil, possibly due to the fact that the laboratories present in this study do not have a quality management system in their processes. Conclusion: Laboratories 2, 3, 4, 6, 9, 11, and 12 were rated as satisfactory. Laboratories 1, 5, 7, 8 and 10 were rated as partially satisfactory. Azevedo","doi":"10.5935/1676-2444.20200001","DOIUrl":"https://doi.org/10.5935/1676-2444.20200001","url":null,"abstract":"Objective: To describe demographic and clinicopathologic profile of oral squamous cell carcinoma (OSCC) cases from incisional biopsies collected at the Nova Friburgo Health Institute of the Fluminense Federal University [Instituto de Saúde de Nova Friburgo da Universidade Federal Fluminense (ISNF/UFF)] and at the Raul Sertã Municipal Hospital [Hospital Municipal Raul Sertã (HMRS)]. Methods: Oral Pathology Laboratory records from the ISNF/UFF were reviewed and all cases of OSCC were primarily selected. Cases from ISNF/UFF and HMRS were selected and had their demographic data collected from the medical records. Clinical characteristics were evaluated using the images of the lesions taken on the biopsy day and the laboratory files. The histological slides were later reviewed. Results: Eighty cases of OSCC were identified. Most patients were male (56.7%), with a mean age of 60 years, smoker (62.71%) and/or alcoholic (44.55%). The most affected anatomic site was the tongue (39.49%), presenting mainly as an ulcer (39.49%). Microscopically, the well-differentiated lesions were more common (35.44%), and the mean counting of mitoses was 5.5/10 high-power field. Conclusion: The profile of OSCC patients in Nova Friburgo partly reflects the world literature, with emphasis on the following differences: low average number of mitoses found in the histopathological analysis and prevalence of well-differentiated lesions. This is due to the increased incidence and detection of neoplasms, associated with diagnostic subspecialization and the advent of personalized medicine. There are numerous treatments available for different types of cancer, and the diagnosis must be dispensed quickly and accurately for each case. Deep learning is a tool that has been used in daily life, including image detection, and there is growing interest in its application in Medicine and especially in Pathology, where it has a revolutionary potential. Objective: In this article, we present deep learning, in particular convolutional neural networks, as a potential technique for the analysis of digitized images of histopathological slides, detecting identifiable patterns in an automated manner, introducing the possibility of applying this technology as an auxiliary tool in the diagnosis of neoplasms, especially in gastric cancer, the object of this preliminary study. Method: From a database of digitized images of histopathological slides representative of gastric cancer, we identified three morphological patterns of neoplasia, as well as non-neoplastic tissue patterns, with which we train a convolutional neural network algorithm, designed to identify and categorize similar images within these standards, in an automated manner. Results: The results of identification and automatic classification in the defined categories were satisfactory, with ROC curves above 0.9. Micronuclei (MN) have been addressed in the literature as biomarkers of genotoxic damage. These markers provide information on biological or biochemical changes in a target tissue at early stage, enabling a favorable prognosis. Objective: To compare whether the frequency of MN in active smokers is higher than that in nonsmokers. Material and method: The search was performed in the Medical Literature Analysis and Retrieval System (PubMeb), in the Latin American and Caribbean Literature in Health Sciences (LILACS), and in the Scientific Electronic Electronic Library Online (SciELO), for articles published in the last ten years. Randomized clinical trials, with a cross-sectional design, which compared the MN frequency in the oral mucosa of adult smokers and non-smokers, were selected. Results: A total of 52 articles were identified, four of them were removed due to duplicity. From the remaining 48 studies, after reading their titles and abstracts, 14 remained, which had their full texts read. Finally, eight articles remained for the qualitative analysis. Discussion and conclusion: Individuals who use tobacco present genotoxic and cytotoxic damages that interfere in the mitosis process, which leads to MN formation. A 44-year-old immunocompetent male with facial lesion and latent pneumonia was hospitalized and misdiagnosed with paracoccidioidomycosis. Computerized tomography scans showed pulmonary and neurological involvement, and cultures/China ink, cryptococcal antigen test and restriction fragment length polymorphism of urease gene ( URA5 -RFLP) confirmed C. neoformans genotype VNI. Hemoculture indicated ampicillin-resistant Klebsiella pneumoniae (healthcare-associated infection). Fluconazole was administered, but after resistance detection, amphotericin B was chosen (cumulative dose/1500 mg). The objective of this study is to report a case of oral lichen planus (OLP) with cutaneous manifestations and to discuss the clinical, histopathological aspects and the established treatment. A 61-years-old female white patient was referred for evaluation of white lesions in the oral mucosa. In the intraoral examination, multiple white lesions with striated appearance were observed in the jugal mucosa, tongue and border. The extraoral examination revealed scaly lesions on the arm, white spots on the legs, and nail dystrophy on feet. Based on biopsy of the oral lesions and the histopathological analysis, the diagnosis of OLP was confirmed. The patient underwent treatment with clobetasol propionate topical cream (0.5 mg), and was instructed to apply it to affected area, once or twice a day for four weeks. In the clinical follow-up after one month and 15 days, the improvement of the lesions could be analyzed. As OLP is a disease with an etiopathogenesis that is still poorly recognized, several factors may enable the development of this condition. Chest computed tomography revealed expansive lesions indicating right supraclavicular fossa lymph node enlargement and right pleural effusion. Toxoplasmosis serology was performed: positive for immunoglobulin G [(IgG) 180.6] and positive for immunoglobulin M [(IgM) 0.98]. Therefore, treatment for the infection was initiated and immunohistochemistry of left cervical lymph node revealed ALL. Consequently, chemotherapy treatment was introduced at an oncology center.
