REV ESP CIR ORAL MAXILOFAC. 2020;42(1):40-46 41 INTRODUCCIÓN La cirugía maxilofacial (CMF) es una especialidad quirúrgica que nace del trabajo mancomunado de dentistas y médicos cirujanos para el tratamiento de la gran cantidad de pacientes heridos y con secuelas de traumas faciales, consecuencia de la primera guerra mundial. En el desarrollo histórico de la especialidad, precozmente, en Reino Unido se plantea la necesidad de una graduación odontológica y médica, incluyendo una formación adicional en cirugía general, a raíz del complejo quehacer quirúrgico y a la creciente responsabilidad en los casos a tratar1-3. Por otro lado en Norteamérica, y ante los eventos históricos de la época, se fortalecieron las posiciones gremiales que mantienen la CMF como una especialidad derivada de la profesión odontológica4 y se abordó la complejidad mediante el desarrollo de programas de doble titulación, que en 2007 llegaban al 50 % del total de programas en Estados Unidos (EE. UU.)1. La necesidad creciente de formación médica en ciertos ámbitos de la especialidad ha derivado en la apertura de múltiples vías de doble titulación en EE. UU. y Canadá. Hoy en día, la variada realidad legislativa, profesional y sociosanitaria de los distintos países determina una diversidad en los requisitos de grado académico al ingreso, en la organización curricular y en el tipo de graduación al egreso de los programas de formación en CMF. En 2017, Kumar revisó algunas de las vías de entrenamiento en CMF en el mundo, incluyendo grado académico y otros requisitos para el ingreso al programa, cantidad de años de formación y grado académico logrado al egreso del programa2, que están resumidos en la Tabla I. This is a rare tumour with uncommon characteristics when compared to other salivary gland tumours. The case reported here is of a 28-year-old female patient who presented in the ER due to a palpable mass in the left parotid region. She underwent a superficial parotidectomy with using a mini-lifting approach, with tumour resection, followed by radiotherapy. They belong to the central third of the face and thus, they have a direct impact on aesthetics. The difficulty in repairing these defects lies in the need to restore facial symmetry and three-dimensionality. In addition, there is a need to preserve the qualities of the skin of each subunit, since they have a different dermal thickness and different number of sebaceous glands. That is the reason why these defects should not be addressed using a single flap and they require a different reconstructive approach. Although composite central facial defects are common, there is not a unified approach to repair them. The cases presented in this manuscript involves patients who underwent surgical intervention to remove a skin cancer. The extirpation resulted in a cutaneous defect affecting both the nasal sidewall and medial cheek. There are some added problems, which have been conditioned by the increase in complications with fillers, that are the use of adulterated materials, injections of various types of substances in the same area or because administration by inexperienced/untrained professionals. Several studies suggest that high frequency ultrasound (HUS) is an economical, useful and non-invasive diagnostic tool to determine the nature and type of material and to identify the injection site and quantity of injected filler. Despite being a well-documented pathology with some predilection in young women between 15 and 35 years old, the exact cause and pathogenesis remain unclear. It has been associated with rheumatoid arthritis, connective tissue diseases, hyperdivergent pattern in the sagittal plane, dentoskeletal Class II, preexisting TMJ dysfunction or even after orthodontic treatment or orthognathic surgery.
