Primary reconstruction options are the superficial musculoaponeurotic system (SMAS) flap, the sternocleidomastoid muscle flap, free or microvascular grafts and biomaterials (Alloderm), among others. We present our experience in periumbilical abdominal free fat graft for primary reconstruction of the parotid defect. Material and methods: A observational study was performed in patients requiring a parotidectomy (superficial, total or radical) due to a primary parotid neoplasm or tumor recurrence. In all cases were used a free abdominal fat graft. The patients were operated by the same surgeon between May 2013 and January 2019. The study analyzes epidemiological and clinical aspects, approach, surgical time, hospital stay, post-surgical complications, sequelae, clinical follow-up and aesthetic results. Results: A total of 22 patients were incluyed after a conservative, total or radical superficial parotidectomy. Thirteen men and nine women aged between 13 and 79 years (46 years on average), with the initial diagnosis made by fine water aspiration puncture (FNA) and Computerized Tomography (CT) or Magnetic Resonance Imaging (MRI). Aesthetic result in all patients was excellent or very good. No long-term sequelae or syndromes Frey. No complications interest except minor postsurgical hematomas and seroma. Although PGCG is classified as a tumor-like lesion, it does not represent a true neoplasm, however it is often associated with a response of local irritation and trauma. Clinically, it appears as an exophytic growth with either sessile or pedunculated base, it can be located in gingiva or edentulous alveolar mucosa and usually does not exceed two cm in diameter. The following report describes the case of a 64-year-old female diagnosed with a PGCG of a particularly large size and rapidly evolving, located in the upper jaw area. The lesion was completely excised, and the diagnosis was confirmed by histopathology, there is no apparent recurrence in the area after 12 months of follow-up. A few cases of PGCG greater than 5 cm in diameter have been reported. Furthermore, exceptional cases might present an abnormal rapid growth as well. Barbier","doi":"10.20986/recom.2020.1140/2020","DOIUrl":"https://doi.org/10.20986/recom.2020.1140/2020","url":null,"abstract":"Objectives: To present the working protocol, clinical outcomes and upper airway changes of a 20patient cohort with moderate-severe obstructive sleep apnea/hypopnea syndrome undergoing maxillo-mandibular advancement with counterclockwise rotation. Material and methods: A multidisciplinary committee determines the surgical indication, which patients consent. Computed tomographies and polysomnographies are performed before and after surgery, as the usual clinical practice protocol. The clinical investigation ethics institutional review board approved the study. Lengths, volumes, minimum area, antero-posterior dimension, transverse dimension and other measurements are determined in the upper airway. Statistical analysis is descriptive and comparative by pairs with p < 0.05. Results: Planned movements are 10.40 mm of advance and 2.11 mm of anterior impaction. Apnea/hypopnea index reduces by 30.50 points and minimum peripheral capillary oxygen saturation increases by 5.00 points. Clinically and statistical significant findings are: 10.98 mm of shortening and 6.26 mm^3 of volume enlargement, especially in the retro-palatal compartment; 91.45 mm^2, 3.68 mm and 8.00 mm of area, antero-posterior dimension and transverse dimension widening respectively; hyoid bone advances 1.92 mm. Conclusions: Maxillo-mandibular advancement with counterclockwise rotation in moderatesevere obstructive sleep apnea/hypopnea syndrome achieves in short-term follow-up apnea/hypopnea index < 15 in 80 % and minimum peripheral capillary oxygen saturation ˃ 85 in 75 % of our series. Many infected people still require to be hospitalized. Around 10-15 % of hospitalized patients require intensive care, where they are intubated for a prolonged period, needing tracheotomies some weeks after the intubation. We will be conducting an observational study of the tracheotomies performed by our oral and maxillofacial Department to COVID-19 patients on intensive care units between March 17th and April 17th, 2020. This study will be analyzing the patients’ epidemiological and clinical aspects, surgical technique employed, surgical time, type of cannula used, postoperative complications and the patients’ clinical monitoring. A total of 22 patients underwent open elective tracheotomy. There were twenty-two males and three females aged between 40 and 77 (mean: 64,9 years-old). In all cases tracheotomy was carried out due to pulmonary process caused by COVID-19 bilateral pneumonia. Two patients presented pneumothorax in the immediate postoperatory care as a complication, one perished during the procedure and another did so after arriving to the Intensive Care Unit after the tracheotomy surgery. Even though tracheotomy is a ruled surgical technique, the special characteristics of COVID-19 patients make of this procedure a critical situation, mainly due to lung instability and quick desaturation of the patients.
